Vessel working in North Sea backloading deck cargo. After disconnecting, the deck crew reported a shackle lying on the edge of the lift which had just been sent down. This shackle is a “green-pin” 6.5 tonne shackle bearing an ID tag 140111.

After back-loading 20ft half height H702, the AB’s noticed a shifting spanner on top of the lift, and immediately reported it to the Bridge Fortunately this was a Near Miss and not a Dropped Object Incident. The potential for a very serious incident is obvious.

As mini container AMD 3787 was being swung over the vessels deck to be backloaded a piece of wood was seen to fall from the container towards the deck landing on top of container TC2.

Vessel deck crew noticed a 14” Pipe Wrench lying on the deck beside air compressor A767 which had been back-loaded a few minutes before.

Reported immediately to DP3 deck crew.

When 40’ basket EL-28 was lifted off the deck it touched the adjacent completion basket ISP8 and objects were seen to fall to the deck from a height of approx 4-5’. It was not clear where the items actually fell from.

Vessel discharging deck cargo at Mearns Quay, Aberdeen, when stevedores had discharged some gas racks the scaffold pole (10ft) was discovered on the frame of cargo unit STX-CEL 0001.

This had been back loaded from the Rig at night, this may explain why it was not spotted by the AB’s when landed on the deck of the vessel.

As tank No. CT363 was being backloaded it struck a mud skip, deck crew noticed when hooking on a another lift the length of angle iron on deck.
