Removed short link and lowered to f loor wi t h main winch

Picked up long link and raised to near monkey board withmain winch

St opped winch and manoeuvred link past t op drive &monkey board fingers

Varco operator on monkey board called to operator atcrown for clearance to recommence hoist ing

On receipt of clearance he called the Varco Supervisor onthe floor who passed the inst ruct ion to the winchoperator

Load raised approx. 1/2m before a scream heard from the

IP at t he crown at approx. 18:30 hrs

IP incurred a de-gloving inj ury with subst ant ial damageto forearm tendons

Subsequent ly had four f ingers of left hand amputat ed atf irst knuckle f rom palm plus surgery t o forearm to address

tendon damage Total 89 days lost t ime

Jingemia 6 - North Perth Basin Investigation

Permit to Work issued at 08:30 hrs, incident occurred at

approx. 18:30 hrs Varco crew on shif t from 06:00 hrs. Permit cont inued through crew change, Permit Authority

change out , dusk JHA prior to issue of PTW consisted of 6 lines and was not

reviewed at any t ime between issue and the incident JHA did not address issue of potential language difficulty

though this was informally discussed

Change out of link was not subj ect t o a JHA Discussions had t aken place but were not documented IP had been in t he mast for approx. 3 hrs

Operations suspended for 36 hrs and trauma counselling

provided for ODE & Varco crews Held meet ing wi th all sit e personnel prior t o

recommencing operat ions Re-it erat ed and re-enforced the need for detailed JHAs;

the JHA for subsequent installation of the hanger linkalone was 18 l ines

Re-it erat ed and re-enforced requir ement for JHAs to bereviewed when condit ions change
