HOSPITAL DUTY – LR/ DR GENERAL OBJECTIVES: At the end of second year, the student shall have acquired the holistic understanding of the human person as a bio – psycho cultural being focusing on the concept of health and illness as it is related to the care of the mother and child in varied settings Specifically, the student shall: 1. describe the health care delivery system . 2. demonstrate ethico – moral, legal responsibilities in the care of the individual, family and community as a whole 3. demonstrate the beginning skills in the provision of independent and collaborative nursing function 4. discuss the role of economics as it impacts on health and illness 5. relate the stages of growth and development in the care of clients 6. demonstrate beginning akills in the preparation of healthy and therapeutic diets in varied client cases 7. explain the dynamics of the disease process cause by microbes and parasites and the environment 8. imbibe the core values cherished by the nursing profession such as love of God, caring, love of country and of people 9. design a plan that will focus on health promotion and risk reduction to clients 10. utilize the nursing process in the care of high risk mother and child in the family ACTIVITIES: pre – conference with Clinical Instructor housekeeping gather/ prepare supplies/ instruments for delivery, initial newborn care and for DC as needed

HOSPITAL DUTY – LR/ DR

GENERAL OBJECTIVES:

ACTIVITIES:

pre – conference with Clinical Instructor housekeeping gather/ prepare supplies/ instruments for delivery, initial newborn care and for DC

as needed

Mother:- Monitor progress of labor. - Takes FHT and BP of the parturient.- Notes and record the duration, interval, frequency and intensity of uterine contractions.- Check if STAT medications are available – oxytocin, methergin and sutures- Handle delivery with DR staff- Aftyer care of instruments including kelly pad and placenta- Chart Nurse's Notes, TPR sheet and BP of the mother. Charting must be checked by CI and countersigned by staff nurse- Transport the mother safely to OB ward with the infant

Assist:- Check the mother's chart is at the labor room with FHT and BP properly recorded- Prepare mother's clothing asnd adult diaper- coaches the mother to bear down during contractions- check BP and VS of the mother after delivery of placenta- administer prescribed drugs (methergin IM) to mother- applies packs over the mother's hypogastrium- goes to the OB resident on duty for post partum doctor's order- after care of DR table and mops the floor

Newborn:- Cord Care

perform initial newborn care ( suctioning, initial bathing, cord care, take v/s and anthropometric measurements: head, chest and abdominal circumference, Crede's prophylaxis, administering vitamin K and Hepa B through IM as ordered, keeping newborn warm with adequate clothing and linen)

assist in latching - on Chart in newborn's chart TPR, HC, CC, length, weight, apgar score(1

and 5 minutes after delivery), footprint and meds given. Charting must be checked by CI and countersigned by staff nurse

Recording of data in the logbook ( Newborn, Delivery and Immunization)

After care of used equipment and supplies

Conduct a post conference and case discussion Give post test
