IntroductionA genetic study of disease based in the genetic
sequencing, allows to know the problem at the
source, looking to raise an specific solution or at
least a better way to treat people with the disease.
Charcot-Marie-Tooth disease has no cure, but
knowing the sequence of it in the patients, allows to
identify clinical variables for a better patient care.

Peripheral NeuropathyThis Comes from the Greek words, “neuro” that means nerves and “pathy” that means abnormal.
There are 2 types of nerves, the ones that relay information from the CNS to the periphery, and the nerves that relay information from the periphery to the CNS

Peripheral Neuropathy
Peripheral neuropathy occurs when these nerves don’t work properly, and this can cause several symptoms like pain, loss of sensation, inability to control muscles and others.

Pacientes
El estudio se realizo en una familia de 10
integrantes.
Madre, padre 4 hermanos afectados y 4
hermanos no afectados con CMT tipo I.

El diagnostico del paciente control y de sus
tres hermanos afectados se realizo por
examen físico y estudios electrofisiológicos.

SNP-Single Nucleotide Polymorphism
El paciente estudiado tiene un total de
3.420.306 SNP de los cuales en los cuales
hay 121 mutaciones sin sentido y 27
mutaciones en las cuales se cambia el codón
Stop por cualquier otro codón.

Polymerase Chain Reaction. PCRPor medio de PCR se amplificaron los exones 5
y 11 del gen SH3TC2 y se secuenciaron
directamente en todos los miembros de la
familia en estudio.
Para verificar la mutación de aminoácidos
Arg954ter (R954X) que corresponde a una
mutación de GA en el DNA genómico en el
exón 11 del gen SH3TC2 en el cromosoma 5.

Polymerase Chain Reaction. PCRSe generó también un fragmento de PCR de
312 Pb que se incubo con la enzima de
restricción TaqI. La mutación del nucleótido
resulta en la eliminación del punto de
restricción para esta enzima.
Esta enzima hace cortes pegajosos entre T y
C.

En adicion al fenotipo de CMT se encontro por medios electrofisiologicos una neuropatia axonal en un padre y un abuelo del individuo estudiado.
Ademas se encontro mononeuropatia del nervio mediano en la muñeca MMM en todos los abuelos y ambos padres sin embargo con un patron de herencia poco claro.

Estudios de Conducción NerviosaSe hayan tres tipos de MMM. El Abuelo carpintero con un fenotipo normal pero MMM severa.
Abuela y madre con una MMM leve.Y una MMM severa asociada con una
polineuropatía axonal.Esta ultima es similar a la neuropatía heredada
atribuida a la haploinsuficienda de PMP22.La duplicación de este gen causa CMT tipo IA, la
forma mas comun.

Variación Genética
Se encontraron 3.420.206 SNPs
Se usaron dos enfoques para determinar el
numero de copias variantes.
Se identificaron 234 variantes en un rango de
tamaño de 1690 pb a 1.627.813 pb pero
ninguna con relacion a neuropatias.

Variación Genética
Se observo entonces los SNP no-sinonimos detectados
por la secuencia completa comparados con una base
de datos de mutaciones previamente observadas.
De las 174 SNP no sinomimos 159 tenian rasgo
hereditario.
De estas, 21 causaban enfermedades mendelianas.
De estas 16 eran heterocigoticas.

Variación Genética
Las 5 homocigoticas que sobraron eran
errores de asignacion de mutaciones de
enfermedad lo que explica porque 4 de ellos
son homocigoticos en el examen pero no
estaban afectados.

Variación Genética
Se examinaron mutaciones en 40 genes
relacionados con neuropatias. Esto dio como
resultado 3148 SNPs de los cuales 54 codificaban.
De estas 54, 2 estaban en el locuas de SH3TC2 y
eran una mutacion con cambio de sentido y una
mutacion sin sentido.

La mutacion sin sentido R954X ya se habia
implicado con CMT
La mutacion con cambio de sentido (AG en el
cromosoma 5 en el nucleotido 148.402.474)
corresponde a la mutacion del aminoacido

Relacion Genotipo/Fenotipo
Se analizaron las mutaciones por separado pero se comprobo
que solo los hijos (4) que presentaban los dos alelos mutados
poseian CMT.
Se encontro Tambien que los que poseian la mutacion Y169H
tenian una neuropatia axonal.
Mientras que la mutacion R954X se asocia con el sindrome
del tunel del carpo sin importar si estaba solo o con el otro
alelo mutante.

A Representa el Familiograma, se muestra de
color negro los individuos afectados con CMT y de color gris los que tienen neuropatía axonal.Solo los individuos con los dos alelos mutados presentan la enfermedad. Los que tienen solo el alelo Y169H presentan neuropatia axonalLos que tienen solo el alelo R954X presentan sindrome del Tunel del Carpo.

restriccion TaqI. La mutacion elimina el sitio de corte de la enzima. Los individuos no mutados presentan dos bandas mientras que los individuos que tienen la mutacion R954X presentan tres bandas de las cuales la de arriba representa el DNA no cortado.

Conclusions 1. There are cases when 2 different mutations combine to develop
a disease.
2. The protein SH3TC2 is necessary to the order of ranvier nodes
without this the patient present malfunction conduction.
3. Sequencing diseases can help to the diagnosis of the primer
cause of the disease, so the tratment can be more effective.
4. Although the mutations are the same, the fenotype in each
patient is different, one could be harder than another one.
