1. Introduction
The Western Australian Poisons Information Centre (WAPIC) commenced operation at Princess Margaret Hospital in 1966. In late 1999, the WAPIC moved to Sir Charles Gairdner Hospital (SCGH), a move that reflected a change in the characteristics of poisonings, with fewer serious paediatric cases and a significant increase in adult poisonings. At SCGH, the WAPIC became closely associated with the Emergency Department and the developing Clinical Toxicology Unit. A Medical Director was appointed to the centre in 1999. In early 2000, the South Australian (SA) PIC closed and the WAPIC took over its coverage to include SA and the Northern Territory, thus expanding our service region to more than half the geographical area of Australia.
2. Emergency and Information Services
The Western Australian Poisons Information Centre provides telephone consultation to the general public and medical professionals in cases of poisoning or suspected poisonings. The centre also provides advice on poisoning prevention, drug information, first-aid management of exposures, and the identification of toxic agents. Complex cases are referred to the on-call Clinical Toxicologist based at Sir Charles Gairdner Hospital, Royal Perth Hospital and Fremantle Hospital. In 2010 the WAPIC referred 1443 calls to the WA Clinical Toxicology Group.
The WAPIC serves Western Australia, South Australia, and the Northern Territory; a total population of 4.2 million 1. Operational hours: 08:00 to 22:00 h WST (14 hours), five days per week and 08:00 to 04:00 h (20 hours) two days per week. Outside these hours calls are diverted to the NSW, Queensland or Victorian PIC’s who share out-of-hours service.
Access to the service is via the 13 11 26 phone number which is charged at the cost of a local call from any landline in Australia.
3. Preventative activities
Our centre collaborates with Kidsafe WA and the Injury Control Council of Western Australia. Printed material was supplied to community groups, including child health centers, day care centers and to interested members of the general public. Information is provided on first-aid management of poisonings and envenoming, the safe storage and use of household chemicals including pesticides and poisonous plants.
4. Training activities for professionals
On-going training and education was provided through attendance of fortnightly case conferences held by the Clinical Toxicologists and weekly education sessions. In addition, a representative attends the bi-monthly national clinical meetings held at the NSWPIC. At these meetings case presentations are made and consensual case management guidelines are formulated.
Clinical placement was provided for two, final year Curtin University, Bachelor of Pharmacy students and lectures were given to Curtin University, Post-graduate Diploma of Pharmacy students.

Call details are entered directly into a computer database, the INTOX Data Management System which was developed and is maintained by the International Programme on Chemical Safety and the World Health Organisation ,Geneva, Switzerland.
Incoming communications are classified as Cases, Incidents or Requests. A case is defined as a record of exposure or potential exposure of an individual to an agent. 2 Requests include drug information calls including drug information in pregnancy and lactation, advice on chemical safety, details on product constituents and requests for printed information. An incident is defined as an event leading to exposure or potential exposure involving one or many victims. At the time of the phone call specific individual victim details are not available in incidents.

A total of 40532 phone calls were handled by the WAPIC in 2010. This number is slightly lower than previous years; 41344 in 2009 and 42597 in 2008. The majority of the callers were either the victim (18.5%) or members of the victim’s family (50%). Health professionals accounted for 21% of the callers.
Incoming phone calls are classified as cases, incidents or requests (CIR’s). One or more phone calls can be recorded for each CIR. In 2010, 30718 (80%) of the calls concerned cases.

(k) Comments
Our centre handled 29068 cases of human exposure in 2010. At the time of the phone call, 42.8% of the victims were either displaying clinical features or biochemical evidence of poisoning (established poisoning). Our centre does not routinely follow-up calls and so final severity is not known in the majority of our cases.
Children were the victims in 59.3% of all human cases, with toddlers (1 to 4 years) involved in 40.6% of all reported exposures. The most common childhood exposures were accidental, occurred in the home and involved pharmaceuticals or common household products.

Adults (adults and elderly) were the victims in 40.7% of cases. Forty two percent of adult exposures resulted from unintentional accidents and 19.6% from attempts of deliberate self harm (intentional suicide). Cases of deliberate self poisoning reported to the WAPIC have increased 18% since 20073. The most common agents involved in suicide attempts were pharmaceuticals.
The most common route of exposure was ingestion (62% of cases). Bites and Stings accounted for 9.4% of exposures, eye exposures 5.4 %, and mucosal exposures (predominately oral) were common in toddler cases involving exploratory tastes.
The agents (substances) involved in each case are categorised into one of 18 categories that include pharmaceuticals for human use, pharmaceuticals for veterinary use, household products, industrial products, cosmetics, pesticides, environmental contaminants, microorganisms, plants and animals. A total of 31,393 agents were involved in the 29,068 cases of human exposures.
At the time of the phone call to the WAPIC 86% of the victims were at the location of poisoning and 14% had either reached a health care facility or were in transit. It is noteworthy that 79% of the victims that were at the location of the exposure at the time the phone call was made to the WAPIC were able to be managed at home, due to the advice of the Poisons Centre. This represents a large saving of health care resources in terms of limiting unnecessary hospital attendances.
