cVDPV2 update from Environmental Samples
1.4 KM
• Sequencing results for the ES sample
collected from Hodan district
ES/SOM/BAN/HDN/EGH-1/18/008 are
genetically linked to the VDPV2 sequence
from ENVID
ES/SOM/BAN/WBR/21OCT/17/003,
(Waberi district, Twenty One October
ENV site).
• SOM/BAN/HDN/EGH-1/18/008 is
classified as cVDPV2, as described in
GPEI Guidelines for Reporting and
Classification of Vaccine-derived
Polioviruses.
• The sample was collected on 22 Feb.
2018 at 8:00 AM and sent to KEMRI on
the same day. The PCR result was
received from KEMRI on March 08. and
final sequencing result on 21 March 2018
• This site was established on 23 November
2017. The sewage is only for the hospital
• Egyptian hospital ES sites is 1.4 KM in the
upstream side of the 21October ES site.
Actions in Egyptian Hospital
Follow-up continued on Egyptian hospital following VDPV2 isolation
• The objective is to identify if the source of cVPDV2 is a patient / caregiver / employee in the Egyptian Hospital
• 5 samples collected from healthy children coming to the hospital for health service on 25th March, 2018 and sent to
KEMRI on the same day.
• Discussion continued with Health workers and hospital managers in the Hospital and collect relevant information on
demographics, travel, immunization status etc. of patients.
• Retrospective active case search continued reviewing medical records from the previous 6 months to see if there were
any missed AFP cases is going on.
• A standard protocol and checklist is developed to conduct which includes standard questioner and data collection
will start soon. The survey will be conducted by “external reviewers” selected and trained for this activity by
WHO
Review meeting Objectives:
1. Review overall cVDPV2 response activities in south and central zones in general and the 3 affected regions in in
particular
2. Review performance and quality of the 2 rounds of mOPV2 campaigns in Banadir, L. Shabelle and M. Shabelle
3. Review surveillance activities in South and Central zones
4. Evaluate the level of preparedness for the IPV campaign: review micro-plan & monitoring plans
5. Discuss with the team and partners on the draft “cVDPV2 outbreak response and surveillance enhancement
action plan”

- Sahil region from Somaliland is silent and has not reported for 8 consecutive weeks. The regional team has requested to collect community samples from health children in health facilities.

Of the 63 districts reporting AFP cases in
2018, 62 have annualized NP-AFP rate above
2 per 100,00 and also Stool adequacy rate is
above 80%.
The remaining 52 districts have not reported
any AFP case in 2018; However, there is no
district in Somalia which is silent for more
than 52 consecutive weeks.

• 95% (n=70/74) cases reported in 2018 were notified within 7 days of onset of paralysis. 3 AFP cases from Bay and 1 from Lower Jubba regions had delayed notification.

Vaccines and Cold Chain Management
• CCEOP operational development plan (ODP) on going and draft plan expected by end of
March 2018
• Plans are under way to deliver the 10-dose IPV to Mogadishu before the proposed start
date of campaign (1st April 2018).
• Final hand over of three regional cold chain stores in Central South to new partners (NGOs)
completed in Kisamyo, Dusharmreb and Baidoa. This hand over was from Swisso Kalmo to
HIRDA ( Lower Juba & Galgadud) and INTERSOS (Bay) respectively
• Updated the IPV vaccine management response plan and shared with WHO and CDC
• Completed the Measles vaccination campaign in Central South with no major logistical
problems reported.
