HehasbeenworkingwithbothNHSEnglandandPublicHealthEnglandtobringaboutatransformationofcarewiththeaimofincreasingvalueforbothpopulationsandindividualsandpublishedabookcalledHowToGetBetterValueHealthcarein2007.ThemeansofdoingthisthroughSystemsisnowcalledPopulationHealthcare.Theaimofpopulationhealthcareistomaximisevalueandequitybyfocusingnotoninstitutions,specialtiesortechnologies,butonpopulationsdefinedbyacommonsymptom,conditionorcharacteristic,suchasbreathlessness,arthritis,ormultiplemorbidity.ThisworkrequiresculturalchangenotstructuralchangeandthedevelopmentofPopulationHealthcareandPopulationMedicine,astyleofclinicalpracticewhichfocusesonallthepeopleinneedinthepopulationandnotjustthosewhoreferthemselvesorhavebeenreferredishiscurrentfocusbothinEnglandandinanumberofothercountries.

DrMelanieHiornsisaconsultantradiologistandDivisionalDirectorofMedicine,DiagnosticandTherapeuticServicesatGreatOrmondStreetHospitalforChildren(GOSH)inLondon.Herclinicalworkisfocussedonbodyradiologyandshehasaspecialinterestinthegenitourinarysystemandgastroradiology.Shehasbeeninaseniormanagementroleforeightyearsandhasresponsibilityforover700staffanda£50Mbudget.
GOSHwasanearlyadopteroftheISASaccreditationschemeinRadiologyandcontributedextensivelytothepilotprojectwork,andsubsequentlybecamethethirdhospitalintheUKtogainaccreditation,andtheonlyspecialistchildren’shospital.InparallelwiththisshebecameaNationalAssessorforISASseveralyearsago,andcontinuestodothisworkassessingradiologydepartmentsagainstthecriteriasetoutbyISAS.

Radiologydevelopedasamedicaldisciplineduringthe20thcenturyandembodiesthemid-centurynotionthathumanproblemscanbeaddressedthroughtheapplicationoftechnology.Overthelast30years,UKmedicalpracticehasbecomeincreasinglyreliantonimagingasameansofdiagnosis,triage,surveillanceandreassurance.
Inthisregard,radiologycanbeseenasagreatsuccesswithagrowthindemand,whichfaroutstripstheGDPgrowthoftheUK,morecloselyparallelingthegrowthofsoutheastAsianeconomies.Thishoweverputspressureonradiologydepartmentsandonindividualradiologiststoincreasethenumberofexaminationsundertakenandtodecreasethetimeframeinwhichtheyarebothperformedandreported.
Nationallycommissionedevidence-basedguidelinesarewidelyemployedinUKmedicinetopromotesafeandcost-effectiveworking.However,itisveryrareforNICEguidelinestosuggestlessimagingaspartofbestpractice.NICEguidelinestendtobedisease-basedandhaveacumulativeimpactonradiologyasadisciplinethatintersectswithdiverseclinicalpathways.Therehasalsobeenanincreaseinexaminationcomplexityandrequesterexpectation.Theriseinexamnumbersisoftenaccompaniedbyadropinthepositivity-rateforagiventest,whichhasimplicationsforthosereportingthem.

Despitethesestresses,radiologyhasimprovedoutcomesforpatientsandcanhelpsecondarycaretackleongoingproblemsofcapacityanddemandinemergencyandinpatientservices.ItalsoremainsanattractivecareeroptionforUKmedicalgraduates.

Therehasbeentremendousprogressmadeoverthelastfortyyearsduetothesecondhealthcarerevolution,withthefirsthealthcarerevolutionhavingbeenthepublichealthrevolutionofthenineteenthcentury.Hipreplacement,transplantation,andchemotherapyareexamplesofthehightechrevolution.Howevertherearestillfiveoutstandingproblemswhicharefoundineveryhealthservicenomatterhowtheyarestructuredandfunded:Oneoftheseproblemsishugeandunwarrantedvariationinaccess,quality,costandoutcome,andthisrevealstheotherfour:

Inadditiontheserviceswillhavetocopewithrisingneedanddemandwithoutadditionalresources.
Moreofthesame,evenbetter,quality,safercareisnottheanswer.Thefocushastobeonvalue,onbettervalueforindividualsandpopulations.Toachievethisweneed:
• Developmentofcollaborativesystemsandnetworkswithpatients&carersasequalpartners
• Personalisedserviceforallthepeopleaffectedinthepopulation• Cliniciansactingthestewardsofthepopulation’sresources
Anewparadigmisneedednotanewstructure.Inanerainwhichtherewillbeinsufficientmoneytomeetneedanddemandweneedtoharnesstheforcesdrivingthethirdhealthcarerevolution–citizens,knowledgeandthesmartphone
Weneedatransformationwiththenewfocusontriplevalue.Thiswillnotbebroughtaboutbymoremoney,evenifmoremoneywereavailable.Norwillitresultfromstructuralreorganisationbecausethebureaucracieswhicharenecessaryareorganisationsthatoperateinalinearfashionwhereashealthandhealthcareissuesarecomplexor‘wicked’problemsandrequirenewtypesofintervention.

Educationalaim:inspiredebateaboutfuturetrainingbasedonpastdevelopmentandcurrentpolitical/medicalimperatives.
Radiologytraininghaschangedwiththeadventofnewtechnologiesandtechniques.ThecurriculaforClinicalRadiologyandInterventionalRadiologyarereviewedandupdatedonanannualcycletomaintaincurrencywithtechnologicalchangesaswellastocomplywiththerequirementsoftheGMCandotherregulatorybodies.
TheShapeofTrainingreviewisunlikelytoimpactonthecurrentstructureoftraininginradiology.CorecompetencesshouldbemaintainedthroughouttrainingwithtraineesatCCT/CSThavingcompetencesingeneralradiology,oneortwospecialinterestareasorsubspecialtycompetenceinInterventionalRadiology.
Trainingundertakenasanapprenticeshipmodel,orblendedwithclassroomlearningasintheAcademies,islikelytoremain,withdurationoftrainingat5or6years,coveringthewholecontentofthecurriculumpreCCT/CST.
Credentialingofareasofpracticealreadyoccurs:forexamplebreastphysiciansandbreastimaging,cardiologistsandcardiacCTandMRI.CredentialingislikelytogoaheadevenifimplementationofthewholeoftheShapeofTrainingreviewdoesnot.Developmentofcredentialsinareasofpractice,withtheRCRasthecredentialingbody,toenablepostCCT/CSTflexibledevelopmentoftheworkforcewillbethenextchallenge.
