The Candida genus is comprised of over 150 species of asporogenous ‘yeast-like’ fungi. Members of this
genus are ubiquitously distributed, persisting as saprophytes in soil and aquatic environments, as well as
colonising several animal reservoirs (1_3). The majority of Candida species are unable to grow at 378C
and are, therefore, not normally associated with human colonization (4). However several species do
persist as commensal microorganisms within humans and these can act as opportunistic pathogens in
debilitated individuals (Table 1). Candida albicans is the species most frequently associated with normal
oral carriage in humans, occurring in the mouths of up to 80% of healthy individuals (13).
A change from the harmless commensal existence of Candida to a pathogenic state can occur following
alteration of the oral cavity environment to one that favours the growth of Candida. The causes of such
changes are the so-called predisposing factors for Candida infection (candidosis) and most often these
relate to a weakening of host immune defences (Table 2).
Infections caused by Candida are most frequently superficial, occurring on moist mucosal surfaces in
individuals suffering with a mild debilitation. In severely immunocompromised patients, infections can be
systemic and are significant because of their associated high mortality. To highlight this, in intensive care
unit patients the mortality rate for individuals with systemic candidosis is approximately 30_50% (24).
Recent decades have seen a significant increase in the incidence of all forms of candidosis and this
reflects changes in medical practice with a greater use of invasive surgical procedures, a more widespread
use of immunosuppressive therapies as well as broad-spectrum antibiotics. Key to the increase in oral
candidosis has, however, been the escalation of HIV-infection and AIDs (25).
The most prevalent Candida species involved in human infection is C. albicans. In oral candidosis, C.
albicans generally accounts for around 50% of cases (26, 27) and whilst a similar prevalence of C.
albicans occurs in systemic candidosis, in recent years higher incidences of non-C. albicans Candida
(NCAC) species have been reported (28, 29). The reasons for the increasing incidence of NCAC species
in human infection are multifactorial and undoubtedly improved diagnostic methods, changes in medical
practices such as the more widespread use of invasive medical procedures (e.g. use of indwelling
catheters, organ transplants) and immunosuppressive therapy may all have facilitated increased rates (30).
Oral candidosis is not a single infection and generally four primary oral forms are described based on
clinical presentation (Fig. 1). Pseudomembranous candidosis (oral thrush) presents as creamy white
lesions on the oral mucosa and a diagnostic feature of this infection is that these plaques can be removed
by gentle scraping leaving behind an underlying erythematous mucosal surface (31, 32). Histological
examination of recovered pseudomembranes reveals desquamated epithelial cells together with yeast and
filamentous forms of Candida.
The infection has traditionally been regarded as an acute condition often affecting newborn babies where
there is
an immature immune system. In older individuals, acute pseudomembranous candidosis often occurs
when there
is a nutritional limitation, local immune suppression (e.g. steroid inhaler administration for the treatment
of asthma), or an underlying disease most notably HIVinfection and AIDS (26, 33, 34).
In the case of AIDS, or other instances where the individual is immunocompromised long-term, chronic
pseudomembranous candidosis can develop and whilst antifungal therapy can temporarily resolve the
condition,
recurrent infection is frequent. Of additional concern with chronic forms of pseudomembranous
candidosis is the subsequent progression of infection to oesophageal involvement, which in turn can lead
to difficulties in swallowing and chest pains.
In addition to pseudomembranous candidosis, another acute form of Candida infection is acute
erythematous candidosis. This form of oral candidosis frequently occurs after receipt of a broad-spectrum
antibiotic, which, by
lowering the oral bacterial population, facilitates subsequent overgrowth of Candida by alleviating
competitive
pressures. The infection can occur on the buccal mucosa, but most frequently presents as reddened lesions
on the
dorsum of the tongue and also the palate (32, 35).
Erythematous candidosis is the only form of oral candidosis that is consistently painful. Where antibiotic
treatment
has been associated with predisposition, cessation of treatment leads to spontaneous resolution of the
lesions once the bacterial population of the mouth recovers to pretreatment levels (22).
Chronic forms of erythematous candidosis can also occur and traditionally these infections encompassed
the atrophic lesions associated with angular cheilitis and denture stomatitis. However, both infections are
also linked to bacterial colonisation and, therefore, these are now often categorised under Candida-
associated lesions (see later). Chronic erythematous candidosis is prevalent in HIV-positive individuals
and AIDS patients, and depending on the study can represent over a third of the Candida lesions
encountered (36).
Chronic hyperplastic candidosis (occasionally referred to as candidal leukoplakia) can present on any oral
mucosal surface and appears either as smooth (homogenous) or nodular white lesions (37). Unlike the
lesions of pseudomembranous candidosis, those of chronic hyperplastic candidosis cannot be removed by
gentle scraping.
Most frequently, chronic hyperplastic candidosis occurs bilaterally in the commissural regions of the
buccal mucosa with highest prevalence in middle-aged men who are smokers (38). A characteristic
feature of this form of oral candidosis is the penetration of the oral epithelium by C. albicans hyphae,
which are detected in biopsy sections following Periodic Acid-Schiff (PAS) or equivalent staining
methods (Fig. 2). Histological examination is indeed the diagnostic tool for this infection and in addition
to the presence of Candida hyphae, an inflammatory cell infiltrate is invariably present within the lamina
propria together with marked variations in epithelial thickness (39). An important feature of chronic
hyperplastic candidosis is its possible association with malignant transformation (40). A confirmed link
between Candida and the development of oral cancer remains to be established, although it has been
shown in vitro that yeast can generate the carcinogenic nitrosamine, N-nitrosobenzylmethylamine from
suitable precursor molecules (41).
Secondary forms of oral candidosis can also occur and are frequently described as Candida-associated
lesions. Angular cheilitis is a condition where lesions manifest at the angles of the mouth and
microbiological sampling of these sites frequently reveal the presence of C. albicans, often in
combination with the bacterium Staphylococcus aureus (42). The exact role of Candida in this infection
remains unclear, but it is worth noting that angular cheilitis often occurs in patients with existing forms of
intraoral candidosis and where the Candida load is already high.
Median rhomboid glossitis is a chronic condition that presents as a diamond-shaped lesion at the posterior
midline on the dorsum of the tongue (31). High levels of Candida can be recovered from these lesions,
which are often associated with individuals who frequently use steroid inhalers or who are tobacco
smokers (43).
The most prevalent oral infection involving Candida is Candida-associated denture stomatitis, which
occurs in up to 65% of denture wearers, often asymptomatically (44). As this name suggests, denture
wearing is the major predisposing factor, particularly in cases where the denture is not adequately
cleansed or is retained overnight in the oral cavity. Under these conditions, the stagnant area above the
upper fitting surface of the denture provides an ideal environment for the growth of Candida, which is
adept at adhering to acrylic used as the base material for the denture (45). Furthermore, the limited flow
of saliva at this location means that loosely adherent Candida may not be efficiently removed from the
site, as would be the case at other oral locations. In situations where there is an illfitting denture, frictional
irritation can damage the normally protective mucosal barrier, allowing infiltration of Candida into the
tissue thereby promoting infection. Candida have also been associated with linear gingival erythema,
which, whilst rare, is occasionally seen in HIVinfected patients (46, 47). The precise role of Candida in
part be due to the frequent use of systemic antifungal agents in HIV patients.
In a discussion on the oral forms of candidosis, mention needs to be made of chronic mucocutaneous
candidosis (CMC), characterised by the widespread occurrence of superficial candidosis of the skin, nails,
and mucosal membranes (including those of the oral cavity) of infected individuals (48, 49). CMC is
primarily associated with disorders resulting in the incorrect functioning of T-lymphocytes (48).
Candida virulence factors
Given the high prevalence of Candida as a harmless commensal in humans, it is not surprising that no
single primary virulence factor has been identified with the organism. However, a number of putative
virulence factors (Table 3) have been proposed that in the event of host debilitation contribute to tissue
damage and persistence of the organism within the host.
The ability of Candida to adhere to host surfaces is a prerequisite for both successful commensal carriage
as well as persistence during active infection.Within the oral cavity, removal of loosely attached Candida
by the physical flushing action of salivary flow or the process of sloughing off of epithelial cells from
mucosal surfaces are important factors in host defence against Candida overgrowth. An enhanced ability
to overcome these removal mechanisms can therefore be regarded as a virulence attribute.
Adherence of Candida to human epithelial cells is initiated through weak and reversible interactions
involving hydrophobic and electrostatic forces (50). The nature of these forces are in part governed by the
local environment in the oral cavity; however, studies have shown that strain variation does occur in
terms of cell surface hydrophobicity that may give certain isolates greater pathogenic potential (51).
Specific adhesion between Candida and receptors on host tissues results in a stronger covalent
attachment. Candida genes of the ALS (agglutinin-like sequence) family (52) and also HWP1 (hyphal
wall protein) (53) encodes for cell wall associated glycoproteins that promote adhesion of C. albicans to
epithelial surfaces. In addition, specific interaction of Candida with oral bacteria has also been
demonstrated that could encourage biofilm formation on dentures and in dental plaque (54).
Biofilms can be defined as microbial communities that are often (but not necessarily) attached to a solid
surface; invariably the biofilm microorganisms are encased within an extracellular polysaccharide matrix
that has been
generated by the organisms themselves. Candida species are adept at forming biofilms (55, 56) and this
has been
linked to both increased expression of virulence factors (57) as well as reduced susceptibility to
antimicrobial
agents (58). Candida biofilms in human disease are widely recognised and occur on tissue surfaces as
well as the
biomaterials of medical devices. Candida biofilms on artificial voice box prostheses have been identified
as a major cause of their failure. Urinary tract infections caused by Candida biofilms in catheterised
patients are recognised, whilst biofilms at other sites including on prosthetic heart valves, intravascular
catheters, and replacement joints have also been linked with infection (59). Candidal biofilms in
haemodialysis and peritoneal dialysis catheters are a common occurrence and associated with an infection
rate of up to 20% in patients undergoing treatment (55).
In terms of oral candidosis, biofilms on an oral prosthesis, most commonly a denture, are a major
predisposing factor to chronic oral candidosis. Candida readily adhere to the polymethylacrylate material
of dentures, and also exploit micro fissures and cracks within the material to facilitate retention (60).
Biofilm formation on denture surfaces is further promoted by poor oral hygiene and retention of dentures
in the mouth whilst sleeping.
Several Candida species are described as polymorphogenic (61) and whilst in nutrient rich culture media
at 378C, yeast growth would appear to be prevalent, filamentous growth can occur and indeed is
frequently seen in clinical situations. Both C. albicans and Candida dubliniensis are species associated
with the generation of true hyphae (62). True hyphae are distinct from pseudohyphae (which are
effectively elongated yeast cells), as the former lack constriction at yeast/hyphal junctions and also
possess septa within the filaments (Fig. 3). The pathogenic significance of being able to produce hyphae
could relate to the greater resistance of hyphae to phagocytosis compared with yeast, enhanced adherence
to host surfaces, and the ability of hyphae to invade epithelial layers resulting in tissue damage (Fig. 2).
Hyphal production is also linked with enhanced expression of several putative virulence genes, some of
which are not involved in the morphogenic process itself (63). It should be noted that both filamentous
and yeast forms are encountered in the oral cavity of healthy individuals, highlighting the importance of
host factors in controlling Candida and the fact that no single predominant virulence factor is associated
with the organism (64).
A phenomenon referred to as ‘high frequency phenotypic switching’ is evident in vitro for certain strains
of C. albicans when cultured on appropriate agar media. Colonies generated by isolates that are ‘high
frequency switchers’ exhibit variation in terms of colour, shape, and translucency. The cause of these
colony switch variants relates to cell morphology and differential gene expression by the variants (56,
61). The so-called white-opaque switching is associated with mating in C. albicans as well as the up-
regulation of biofilm genes (65, 66). High frequency switching strains have been proposed to have greater
virulence, as replication of this property in vivo might aid in evading recognition by adaptive immune
responses as well as promoting biofilm formation. Candida produces several extracellular enzymes that
could have a locally damaging effect on host structures (67). Significant attention has been given to the
secreted aspartyl proteinases (SAPs) of Candida. C. albicans possesses at least 10 different genes that
encode for SAPs, and the resulting proteins are in turn designated SAPs 1_10. SAPs 1_8 are released
extracellularly, whilst SAPs 9 and 10 are thought to be membrane bound. SAP enzymes belong to the
same class of proteinases as the HIV aspartyl proteinase as well as human pepsin and renin. All Candida
SAPs are inhibited by the agent pepstatin A. Candida SAPs have optimal activity at acidic pH (pH 2_7
activity range), although distinct differences in pH optima are evident with SAPs 1_3 being most active at
the lower end of the pH scale and SAPs 4_6 at higher pH. This heterogeneity in activity optima of the
SAPs could benefit Candida in its survival under different environmental conditions. Of the Candida
SAPs, the most widely studied are SAPs 1_6. From
research it has been shown that SAP 2 is induced when C. albicans is cultured in media containing
proteins as the only nitrogen source (68). SAP 1 and SAP 3 are specifically expressed during switching of
C. albicans, whilst SAPs 4_6 are involved in promoting hyphal growth (69). C. albicans proteinases have
keratinolytic activity that can both serve to facilitate initial penetration of keratinised cells as well as
providing a valuable source of nitrogen during colonisation (70, 71).
In terms of virulence, SAP activity can therefore directly induce damage to host cells, facilitate hyphal
growth for invasion of tissue, increase adherence following exposure of receptor sites, and also degrade
host immunoglobulins and other defence proteins (55, 72_75). Another group of hydrolytic enzymes
produced by Candida species are the phospholipases (PLs) and seven distinct encoding genes have been
identified in C. albicans. These genes have been given the designations PLA, PLB1, PLB2, PLC1, PLC2,
PLC3, and PLD1 (76). Through the hydrolysis of ester linkages of phospholipids,
PLs can effectively degrade the membrane of host cells leading to cell lysis and death. By this process,
both adherence of Candida to receptor sites and its subsequent penetration of damaged tissue can be
facilitated.
The role of extracellular lipase and esterase production by Candida in pathogenic processes is less well
understood
(77). Both lipases and esterases share the ability to hydrolyse the ester bonds in glycerides, although the
latter only act on soluble substrate molecules. A total of 10 Candida lipase (LIP1_10) genes have been
identified in C. albicans and sequence-related genes found in C. tropicalis, C. parapsilosis, and C. krusei
(78). Lipases of C. albicans have recently been shown to exhibit cytotoxic effects on host cells (79) and
LIP gene expression detected in oral candidosis (80).
Haemolysins are substances that lyse red blood cells and their production by Candida is considered an
important attribute in promoting survival within the host through an increased ability to sequester iron.
Luo et al. (81) demonstrated a- and b-haemolysis by clinical isolates of C. albicans, C. dubliniensis, C.
kefyr, C. krusei, C. zeylanoides, C. glabrata, C. tropicalis, and C. lusitaniae. Candida haemolysin
production positively correlates with glucose concentration and that this could be a predisposing factor in
candidosis of poorly controlled diabetics where higher blood and salivary glucose levels occurs (76, 82).
Host response to oral candidosis
Immunocompetent individuals rarely suffer from oral candidosis even when Candida is present in the oral
cavity. Prevention of mucosal infection by Candida is mediated primarily by the functions of the innate
immune response. In particular, neutrophils and macrophages are key to successful phagocytosis and
killing of Candida. Professional phagocytes recognise Candida through pattern recognition receptors
(PRRs), which interact with specific molecules (pathogen-associated molecular patterns; PAMPs)
exposed on the surface of Candida (83).
Following recognition, these cells release cytokines and chemokines to further modulate the immune
response. Dendritic cells (DCs) are professional antigen presenting cells that provide a sentinel role in
mucosal tissue. Interaction of DCs with Candida leads to DC activation and phagocytosis. Following
phagocytosis, DCs migrate to the lymph nodes where the Candida antigen is processed and presented on
the surface of the DC to naı¨ve CD4_ T-cells (84_88). Interaction between DCs and T-cells cause the
latter to differentiate into mature effective T-cells (89). The type of T-cell generated is thought to be
under direction of the DC and examples of effective T-cells include T-helper 1 (Th1), T-helper 2 (Th2),
T-helper 17 (Th17), and regulatory T-cells (Tregs). Previously it was generally accepted that a Th1
elicited response was a protective one, whilst Th2 responses were implicated in infection. More recently,
evidence suggests that a Th17 response is predominant in protection of mucosal surfaces (90).
Treatment and management of oral candidosis
A priority in the treatment of oral candidosis is the alleviation of any identifiable predisposing factor.
Acquiring a thorough medical history is, therefore, an essential component in the management process.
An example of this are the recommendations made to patients suffering from chronic erythematous
candidosis in order to improve oral hygiene through adequate denture cleansing. Such recommendations
would include the regular and frequent use of a denture cleanser with anti-candidal properties such as 1%
sodium hypochlorite preparations, together with the removal of dentures overnight. Chlorhexidine (0.2%)
should be used if the denture has metal components since hypochlorite will otherwise cause
discolouration.
However, it should also be noted that discolouration of teeth and mucosal surfaces have been reported
with the use of chlorhexidine (91). Certain predisposing factors are, however, more difficult if not
impossible to eradicate such as where there is an underlying disease (e.g. leukaemia or AIDS). In these
cases, targeted antifungal therapy, as discussed later,
plays an important role in the management strategy. Both the physical and chemical reduction of Candida
load in the oral cavity can be achieved by good oral hygiene practices including tooth brushing and the
use of antimicrobial mouthwashes. Manual tooth brushing is limited to accessible oral surfaces, although
powered or electrical tooth brushing may be more effective as cavitation within surrounding fluids could
disrupt Candida biofilms at otherwise inaccessible sites (92).
Several mouthwashes exhibit anti-candidal activity including triclosan, chlorhexidine gluconate, and
essential
oil formulations. The latter tend to contain natural plant extracts such as thymol, eucalyptol, and
bioflavanoids and these can have a direct anti-candidal activity in vitro through cell membrane disruption
and enzyme inhibition (93, 94).
When compared with antibacterial agents, the availability of antifungal agents is significantly lower. The
reason for this relates to the relatively recent recognition of the significance of human fungal infections
and also the fact that fungal organisms are eukaryotic and share many features with mammalian cells
making selection of suitable targets for antifungal agents problematic. Classification of antifungal agents
is based on the target of activity, and in the treatment of candidosis the two classes most commonly used
are the polyenes and the azoles (Table 4).
Polyenes include the drugs amphotericin B and nystatin and their mode of action is through direct binding
to the sterol ergosterol found within fungal cell membranes. Polyene binding to ergosterol induces
leakage of cytoplasmic contents leading to fungal cell death (95). The equivalent mammalian sterol is
cholesterol, which has a lower binding affinity for polyenes and this makes host cells less susceptible to
their toxic effects. Nevertheless, at higher therapeutic concentrations polyenes do exhibit a degree of
toxicity in humans. The use of polyenes is limited further as they are poorly absorbed through the gut and,
therefore, topical application in the form of lozenges and oral suspensions are the principle means of
administration in oral infection. Polyenes are frequently used in the treatment of chronic erythematous
candidosis, and oral suspension of amphotericin B may be employed in treating refractory oral candidosis
frequently seen in HIV-infected and AIDs patients.
Unlike the polyenes, azole antifungals are fungistatic rather than fungicidal (96). The mechanism of
action is by inhibiting the enzyme lanosterol demethylase that is a cytochrome P-450 3-A dependent
enzyme involved in the synthesis of ergosterol. Subsequent depletion of ergosterol in the fungal cell
results in inhibition of fungal growth and impairment of membrane permeability. Since azoles are
fungistatic, complete resolution of the infection will be aided by simultaneously addressing predisposing
host factors. The two most frequently administered azole antifungals in the treatment of oral candidosis
are fluconazole and itraconazole and these drugs have the advantage of being readily absorbed through
the gut with the result that oral administration is an effective means of delivery (97). Furthermore,
fluconazole is secreted in high
levels in saliva making the agent particularly suitable for treating oral infection (98).
Unfortunately, in recent years Candida resistance to azole antifungals has been detected and this can arise
through several mechanisms including the over production of the lanosterol demethylase enzyme, an
alteration in the demethylase enzyme structure that makes it less susceptible to azole inhibition, the use of
multi-drug transporter pumps to remove azoles from the cell, and the incorporation of alternative sterols
to ergosterol within the cell membrane (99). Several NCAC species are
inherently more resistant to azoles than other species. For example, 35% and 75% of C. glabrata and C.
krusei isolates exhibit resistance to fluconazole and this could be a reason why the prevalence of certain
NCAC species in human disease has increased in recent years (29, 100).
As the need to expand antifungal options increases, newer azole drugs such as itraconazole have been
used in the treatment of oral candidosis, whilst others including voriconazole and pozoconazole are
alternatives for invasive infections by Candida. Recently, the echinocandin class of antifungals have
emerged as alternatives to the azoles and polyenes (101). Echinocandins act through inhibition of the D-
glucan synthase, which is an enzyme required for the synthesis of the fungal cell wall. This enzyme is
absent from mammalian cells thereby reducing potential host cell toxicity. Whilst echinocandins such as
caspofungin, micafungin, and anidulafungin are fungicidal against Candida, their use is somewhat limited
by their large molecular size that dictates the need for intravenous administration. Echinocandins are
again primarily used in the treatment of invasive fungal infections.
Successful treatment of candidosis can be hampered where there is an established biofilm. Candida
biofilms exhibit significantly higher tolerance to both antimicrobial mouthwashes and also traditional
antifungal agents. As a consequence, alternative strategies have been suggested to combat such infections.
These have included the modification of biomaterials such as those used in denture prosthesis to inhibit
adherence of Candida. Examples include the use of coatings with silanes, chlorhexidine, histatins,
antifungals, as well as the incorporation of surface-modifying groups (102_105).
Alternative strategies could exploit quorum-sensing agents to promote biofilm disruption with the agent
farnesol already having been shown to induce instability in Candida biofilms (106). Other possible
strategies could involve the use of probiotics (107), which would induce an added microbiological
pressure on Candida within the oral cavity and may also promote local immune function. The potential
benefits of probiotics in the management of Candida biofilms have already been reported for indwelling
voice box prostheses (108), as well as in lowering the Candida prevalence in the oral cavity (109).

telah dilaporkan dari spesies non - C . Candida albicans ( NCAC) ( 28 , 29 ) . Alasan meningkatnya
insiden spesies NCAC pada infeksi manusia multifaktorial dan tidak diragukan lagi ditingkatkan
diagnostik metode , perubahan dalam praktek medis seperti penggunaan lebih luas dari invasif prosedur
medis ( misalnya penggunaan kateter , transplantasi organ ) dan terapi imunosupresif semua mungkin
meningkatkan ratio ( 30 ) .
oral kandidosis bukan infeksi tunggal dan umumnya dalam empat primary oral form dijelaskan
berdasarkan presentasi klinis (Gambar 1 ) . Kandidosis pseudomembran ( oral thrush ) muncul sebagai
creamy white lesions pada mukosa mulut dan fitur diagnostik infeksi ini adalah bahwa plak ini dapat
dihilangkan dengan gesekan lembut meninggalkan permukaan dasar mukosa yang eritematosa ( 31 , 32 ) .
Pemeriksaan histologis pseudomembranes pulih mengungkapkan sel epitel desquamated bersama dengan
ragi dan bentuk filamen dari Candida .
Infeksi secara tradisional dianggap sebagai kondisi akut sering mempengaruhi bayi yang baru lahir di
mana ada
sistem kekebalan tubuh yang belum matang . Pada individu yang lebih tua , kandidosis pseudomembran
akut sering terjadi ketika ada adalah keterbatasan gizi , penekanan kekebalan lokal (misalnya administrasi
steroid inhaler untuk pengobatan asma ) , atau penyakit yang mendasari terutama infeksi HIV dan AIDS (
26 , 33 , 34 ) .
Dalam kasus AIDS , atau contoh lain di mana individu immunocompromised jangka panjang , kandidosis
pseudomembran kronis dapat mengembangkan dan sementara terapi antijamur sementara dapat mengatasi
keadaan tersebut , Infeksi berulang sering terjadi . Perhatian tambahan dengan bentuk kronis kandidosis
pseudomembran adalah perkembangan selanjutnya dari infeksi keterlibatan esofagus , yang pada
gilirannya dapat menyebabkan kesulitan menelan dan nyeri dada .
Selain kandidosis pseudomembran , bentuk akut lain infeksi Candida adalah kandidosis eritematosa akut .
Bentuk kandidosis mulut sering terjadi setelah menerima antibiotik spektrum luas , yang , dengan
menurunkan populasi bakteri oral, memfasilitasi pertumbuhan Candida berlebih berikutnya dengan
mengurangi kompetitif tekanan . Infeksi dapat terjadi pada mukosa bukal , tetapi paling sering muncul
sebagai lesi kemerahan pada dorsum lidah dan juga langit-langit ( 32 , 35 ) . Kandidosis eritematosa
adalah satu-satunya bentuk kandidiasis oral yang secara konsisten menyakitkan . Dimana pengobatan
antibiotic telah dikaitkan dengan predisposisi , penghentian pengobatan menyebabkan resolusi spontan
lesi sekali populasi bakteri mulut pulih ke tingkat pretreatment ( 22 ) .
Bentuk kronis dari kandidosis eritematosa juga dapat terjadi dan tradisional infeksi ini meliputi lesi atrofi
terkait dengan cheilitis angular dan denture stomatitis . Namun, kedua infeksi juga terkait dengan
kolonisasi bakteri dan , oleh karena itu , ini sekarang sering dikategorikan di bawah lesi Candida terkait
( lihat nanti ) . Kandidosis eritematosa kronis adalah lazim pada orang HIV - positif dan pasien AIDS ,
dan tergantung pada studi ini dapat mewakili lebih dari sepertiga dari lesi Candida ditemui ( 36 ) .
Kandidosis hiperplastik kronis ( kadang-kadang disebut sebagai leukoplakia candida ) dapat hadir pada
setiap permukaan mukosa mulut dan muncul baik lesi putih ( homogen ) atau nodular halus ( 37 ) .
Berbeda dengan lesi kandidosis pseudomembran , orang-orang dari kandidosis hiperplastik kronis tidak
dapat dihapus oleh gesekan lembut . Paling sering, kandidosis hiperplastik kronis terjadi bilateral di
daerah commissural dari mukosa bukal dengan prevalensi tertinggi pada laki-laki paruh baya yang
perokok ( 38 ) . Sebuah fitur karakteristik dari bentuk kandidosis oral penetrasi epitel lisan oleh C.
albicans hifa , yang terdeteksi di bagian biopsi berikut Periodic Acid -Schiff ( PAS ) atau metode
pewarnaan yang setara (Gambar 2 ) . Pemeriksaan histologi memang alat diagnostik untuk infeksi ini dan
di samping kehadiran Candida hifa , infiltrasi sel inflamasi adalah selalu hadir dalam lamina propria
bersama dengan variasi ditandai ketebalan epitel ( 39 ) . Sebuah fitur penting dari kandidosis hiperplastik
kronis adalah hubungan yang mungkin dengan transformasi ganas ( 40 ) . Sebuah link dikonfirmasi antara
Candida dan perkembangan kanker mulut masih harus dibentuk , meskipun telah terbukti secara in vitro
bahwa ragi dapat menghasilkan nitrosamine karsinogenik , N - nitrosobenzylmethylamine dari cocok
prekursor molekul ( 41 ) .
Bentuk sekunder dari kandidosis mulut juga dapat terjadi dan sering digambarkan sebagai lesi Candida
terkait . Angular cheilitis adalah suatu kondisi dimana lesi nyata di sudut mulut dan mikrobiologi sampel
dari situs ini sering mengungkapkan adanya C. albicans , sering dalam kombinasi dengan bakteri
Staphylococcus aureus ( 42 ) . Peran yang tepat dari Candida infeksi ini masih belum jelas , tetapi perlu
dicatat bahwa cheilitis angular sering terjadi pada pasien dengan bentuk yang ada dari kandidosis
intraoral dan di mana beban Candida sudah tinggi .
Median rhomboid glossitis adalah suatu kondisi kronis yang muncul sebagai lesi berbentuk berlian di
garis tengah posterior pada dorsum lidah ( 31 ) . Tingginya kadar Candida dapat pulih dari lesi ini , yang
sering dikaitkan dengan individu yang sering menggunakan inhaler steroid atau yang perokok tembakau
( 43 ) .
Infeksi oral yang paling umum melibatkan Candida adalah Candida terkait denture stomatitis , yang
terjadi di hingga 65 % dari pemakai gigitiruan , sering asymptomatically ( 44 ) . Seperti nama ini
menunjukkan , gigi tiruan mengenakan merupakan faktor predisposisi utama , terutama dalam kasus di
mana gigi tiruan tidak cukup dibersihkan atau disimpan semalam di rongga mulut . Dengan kondisi
tersebut , daerah stagnan di atas permukaan pas atas gigi tiruan menyediakan lingkungan yang ideal untuk
pertumbuhan Candida , yang mahir mengikuti akrilik digunakan sebagai bahan dasar untuk gigi tiruan
( 45 ) . Selain itu , aliran air liur terbatas di lokasi ini berarti bahwa longgar patuh Candida mungkin tidak
efisien dihapus dari situs , seperti yang akan terjadi di lokasi mulut lainnya . Dalam situasi di mana ada
gigitiruan illfitting , iritasi gesekan dapat merusak penghalang mukosa biasanya pelindung , yang
memungkinkan infiltrasi Candida ke dalam jaringan sehingga meningkatkan infeksi . Candida juga telah
dikaitkan dengan eritema gingiva linear , yang, sementara jarang , kadang-kadang terlihat pada pasien
HIVinfected ( 46 , 47 ) . Peran yang tepat dari Candida sebagian disebabkan oleh sering menggunakan
agen antijamur sistemik pada pasien HIV .
Dalam sebuah diskusi tentang bentuk-bentuk oral kandidosis , menyebutkan perlu dibuat dari kandidosis
mukokutan kronis ( CMC ) , ditandai dengan terjadinya luas kandidosis superfisial kulit , kuku , dan
membran mukosa ( termasuk orang-orang dari rongga mulut ) dari terinfeksi individu ( 48 , 49 ) . CMC
terutama terkait dengan gangguan yang mengakibatkan fungsi yang salah dari T - limfosit ( 48 ) .
Faktor virulensi Candida
Mengingat tingginya prevalensi Candida sebagai komensal yang tidak berbahaya pada manusia , maka
tidak mengherankan bahwa tidak ada faktor virulensi utama tunggal telah diidentifikasi dengan organisme
. Namun, sejumlah faktor virulensi diduga ( Tabel 3 ) telah diusulkan bahwa dalam hal host kelemahan
berkontribusi terhadap kerusakan jaringan dan ketekunan organisme dalam host.
Kemampuan Candida untuk mematuhi tuan permukaan merupakan prasyarat bagi keberhasilan kereta
komensal serta ketekunan selama aktif infection.Within rongga mulut , penghapusan longgar melekat
Candida oleh aksi pembilasan fisik aliran saliva atau proses peluruhan off sel epitel dari permukaan
mukosa merupakan faktor penting dalam pertahanan host terhadap Candida pertumbuhan berlebih .
Kemampuan ditingkatkan untuk mengatasi mekanisme penghapusan ini karena itu dapat dianggap
sebagai atribut virulensi .
Kepatuhan Candida pada sel epitel manusia dimulai melalui interaksi lemah dan reversibel yang
melibatkan pasukan hidrofobik dan elektrostatik ( 50 ) . Sifat kekuatan ini sebagian diatur oleh
lingkungan lokal di rongga mulut ; Namun , penelitian telah menunjukkan bahwa variasi regangan tidak
terjadi dalam hal hidrofobik permukaan sel yang dapat memberikan tertentu isolat potensial patogen yang
lebih besar ( 51 ) .
Adhesi spesifik antara Candida dan reseptor pada host hasil jaringan dalam lampiran kovalen kuat .
Candida gen dari ALS ( urutan agglutinin - seperti ) keluarga ( 52 ) dan juga HWP1 ( hifa protein
dinding ) ( 53 ) mengkode untuk dinding sel glikoprotein terkait yang mempromosikan adhesi C. albicans
pada permukaan epitel . Selain itu, interaksi spesifik Candida dengan bakteri mulut juga telah
menunjukkan bahwa dapat mendorong pembentukan biofilm pada gigi palsu dan plak gigi ( 54 ) .
Biofilm dapat didefinisikan sebagai komunitas mikroba yang sering (tapi tidak selalu ) melekat pada
permukaan padat ; selalu mikroorganisme biofilm yang terbungkus dalam suatu polisakarida matriks
ekstraselular yang telah
dihasilkan oleh organisme itu sendiri . Spesies Candida mahir membentuk biofilm ( 55 , 56 ) dan ini telah
terkait dengan kedua peningkatan ekspresi faktor virulensi ( 57 ) serta mengurangi kerentanan terhadap
antimikroba
agen ( 58 ) . Candida biofilm dalam penyakit manusia diakui secara luas dan terjadi pada permukaan
jaringan serta
biomaterial perangkat medis . Candida biofilm pada buatan prostesis kotak suara telah diidentifikasi
sebagai penyebab utama kegagalan mereka . Infeksi saluran kemih yang disebabkan oleh Candida biofilm
pada pasien dengan kateter diakui , sementara biofilm pada situs lain termasuk pada katup prostetik
jantung , kateter intravaskular , dan sendi pengganti juga telah dikaitkan dengan infeksi ( 59 ) . Biofilm
candida di hemodialisis dan kateter peritoneal dialisis adalah umum terjadi dan terkait dengan tingkat
infeksi hingga 20 % pada pasien yang menjalani pengobatan ( 55 ) .
Dalam hal kandidosis oral, biofilm pada prosthesis oral, paling sering gigitiruan , merupakan faktor
predisposisi utama kandidosis mulut kronis . Candida siap mematuhi bahan polymethylacrylate gigi palsu
, dan juga mengeksploitasi celah mikro dan retak dalam materi untuk memfasilitasi retensi ( 60 ) .
Pembentukan biofilm pada permukaan gigi tiruan ini lebih dipromosikan oleh kebersihan mulut yang
buruk dan retensi gigi palsu dalam mulut sementara tidur .
Beberapa spesies Candida digambarkan sebagai polymorphogenic ( 61 ) dan sementara di nutrisi media
kultur yang kaya di 378C , pertumbuhan ragi akan muncul menjadi lazim , pertumbuhan berfilamen dapat
terjadi dan memang sering terlihat dalam situasi klinis . Kedua albicans C. dan Candida dubliniensis
adalah spesies yang terkait dengan generasi hifa sejati ( 62 ) . Benar hifa berbeda dari pseudohyphae
( yang efektif memanjang sel ragi ) , sebagai mantan penyempitan kurangnya di persimpangan ragi / hifa
dan juga memiliki septa dalam filamen (Gambar 3 ) . Signifikansi patogen mampu menghasilkan hifa
bisa berhubungan dengan resistensi yang lebih besar dari hifa ke fagositosis dibandingkan dengan ragi ,
kepatuhan ditingkatkan menjadi tuan permukaan , dan kemampuan hifa untuk menyerang lapisan epitel
yang mengakibatkan kerusakan jaringan (Gambar 2 ) .
Produksi hifa juga terkait dengan peningkatan ekspresi beberapa gen virulensi diduga , beberapa di
antaranya tidak terlibat dalam proses morphogenic itu sendiri ( 63 ) . Perlu dicatat bahwa bentuk-bentuk
baik berserabut dan ragi yang dihadapi dalam rongga mulut orang sehat , menyoroti pentingnya faktor
tuan rumah dalam mengendalikan Candida dan fakta bahwa tidak ada faktor virulensi dominan tunggal
dikaitkan dengan organisme ( 64 ) .
Sebuah fenomena yang disebut sebagai ' frekuensi tinggi fenotipik beralih ' jelas in vitro untuk strain
tertentu dari C. albicans pada medium media agar sesuai. Koloni yang dihasilkan oleh isolat yang variasi
pameran ' frekuensi beralih tinggi ' dalam hal warna, bentuk , dan tembus . Penyebab varian beralih koloni
berkaitan dengan morfologi sel dan ekspresi gen diferensial dengan varian ( 56 , 61 ) . Yang disebut
beralih putih buram dikaitkan dengan kawin di C. albicans serta up- regulasi gen biofilm ( 65 , 66 ) .
Tinggi frekuensi beralih strain telah diusulkan untuk memiliki virulensi yang lebih besar , sebagai
replikasi in vivo ini properti mungkin membantu dalam pengakuan menghindari oleh imun adaptif
tanggapan serta mempromosikan pembentukan biofilm . Candida menghasilkan beberapa enzim
ekstraseluler yang dapat memiliki efek merusak secara lokal pada struktur host ( 67 ) . Perhatian yang
signifikan telah diberikan kepada proteinase aspartil disekresikan ( SAP ) dari Candida . C. albicans
memiliki setidaknya 10 gen yang berbeda yang menyandikan untuk SAP , dan protein yang dihasilkan
pada gilirannya ditunjuk SAP 1_10 . SAP 1_8 dirilis ekstrasel , sementara SAP 9 dan 10 dianggap terikat
membran . SAP enzim milik kelas yang sama proteinase sebagai proteinase HIV aspartil serta pepsin
manusia dan renin . Semua SAP Candida dihambat oleh agen Pepstatin A. Candida SAP memiliki
aktivitas optimal pada pH asam ( pH kisaran aktivitas 2_7 ) , meskipun perbedaan yang jelas dalam pH
optima yang jelas dengan SAP 1_3 yang paling aktif di ujung bawah dari skala pH dan SAP 4_6 pada pH
yang lebih tinggi . Heterogenitas ini dalam kegiatan optima dari SAP bisa menguntungkan Candida dalam
kelangsungan hidup di bawah kondisi lingkungan yang berbeda . Dari SAP Candida , yang paling banyak
dipelajari adalah SAP 1_6 . dari
penelitian telah menunjukkan bahwa SAP 2 diinduksi ketika C. albicans dibiakkan dalam media yang
mengandung protein sebagai satu-satunya sumber nitrogen ( 68 ) . SAP dan SAP 1 3 secara khusus
disajikan selama beralih dari C. albicans , sementara SAP 4_6 terlibat dalam mempromosikan
pertumbuhan hifa ( 69 ) . C. albicans proteinase memiliki aktivitas keratinolytic yang dapat baik berfungsi
untuk memfasilitasi penetrasi awal sel keratinised serta menyediakan sumber yang berharga dari nitrogen
selama penjajahan ( 70 , 71 ) .
Dalam hal virulensi , kegiatan SAP oleh karena itu secara langsung menyebabkan kerusakan pada sel
inang , memfasilitasi pertumbuhan hifa untuk invasi jaringan , meningkatkan kepatuhan mengikuti
paparan dari situs reseptor , dan juga menurunkan imunoglobulin host dan protein pertahanan lainnya ( 55
, 72_75 ) . Kelompok lain dari enzim hidrolitik yang dihasilkan oleh spesies Candida adalah
phospholipases ( PL ) dan tujuh gen encoding yang berbeda telah diidentifikasi dalam C. albicans . Gen
ini telah diberi sebutan PLA , PLB1 , PLB2 , PLC1 , PLC2 , PLC3 , dan PLD1 ( 76 ) . Melalui hidrolisis
ikatan ester dari fosfolipid ,
PL dapat secara efektif menurunkan membran sel inang menyebabkan lisis sel dan kematian . Dengan
proses ini , baik kepatuhan Candida ke situs reseptor dan penetrasi selanjutnya dari jaringan yang rusak
dapat difasilitasi .
Peran ekstraseluler lipase dan esterase produksi oleh Candida dalam proses patogenik kurang dipahami
( 77 ) . Kedua lipase dan esterase berbagi kemampuan untuk menghidrolisis ikatan ester pada gliserida ,
meskipun yang terakhir hanya bertindak pada molekul substrat larut . Sebanyak 10 Candida lipase
( LIP1_10 ) gen telah diidentifikasi dalam C. albicans dan gen - urutan terkait ditemukan di C. tropicalis ,
C. parapsilosis , dan C. krusei ( 78 ) . Lipase dari C. albicans baru-baru ini telah ditunjukkan untuk
menunjukkan efek sitotoksik pada sel inang ( 79 ) dan ekspresi gen LIP terdeteksi dalam kandidosis oral (
80 ) .
Haemolysins adalah zat yang sel-sel darah merah dan produksi melisiskan mereka dengan Candida
dianggap sebagai atribut penting dalam mempromosikan kelangsungan hidup dalam host melalui
peningkatan kemampuan untuk menyerap zat besi . Luo et al . ( 81 ) menunjukkan - dan b - hemolisis
oleh isolat klinis dari C. albicans , C. dubliniensis , C. kefyr , C. krusei , C. zeylanoides , C. glabrata , C.
tropicalis , dan C. lusitaniae . Candida produksi hemolisin positif berkorelasi dengan konsentrasi glukosa
dan bahwa ini bisa menjadi faktor predisposisi pada kandidosis penderita diabetes yang kurang terkontrol
di mana kadar glukosa darah dan saliva lebih tinggi terjadi ( 76 , 82 ) .
Respon terhadap kandidosis mulut tuan rumah
Individu imunokompeten jarang menderita kandidosis mulut bahkan ketika Candida hadir dalam rongga
mulut . Pencegahan infeksi mukosa oleh Candida dimediasi terutama oleh fungsi respon imun bawaan .
Secara khusus, neutrofil dan makrofag adalah kunci untuk sukses fagositosis dan pembunuhan Candida .
Fagosit profesional mengakui Candida melalui reseptor pengenalan pola ( PRRs ) , yang berinteraksi
dengan molekul tertentu ( pola molekul patogen terkait , PAMPs ) terkena pada permukaan Candida ( 83 )
.
Setelah pengakuan , sel-sel ini melepaskan sitokin dan kemokin untuk lebih memodulasi respon imun .
Sel dendritik ( DC ) adalah antigen presenting sel profesional yang memberikan peran sentinel dalam
jaringan mukosa . Interaksi DC dengan Candida menyebabkan aktivasi DC dan fagositosis . Setelah
fagositosis , DC bermigrasi ke kelenjar getah bening di mana antigen Candida diolah dan disajikan pada
permukaan DC ke Nai ¨ ve T - sel CD4_ ( 84_88 ) . Interaksi antara DC dan T - sel menyebabkan kedua
untuk berdiferensiasi menjadi sel - T yang efektif dewasa ( 89 ) . Jenis T - sel yang dihasilkan dianggap di
bawah arahan dari DC dan contoh T - sel yang efektif termasuk T - helper 1 ( Th1 ) , T - helper 2 ( Th2 ) ,
T - helper 17 ( Th17 ) , dan peraturan - sel T ( Treg ) . Sebelumnya itu umum diterima bahwa respon

Pengobatan dan pengelolaan kandidosis mulut
Prioritas dalam pengobatan kandidosis oral pengentasan faktor predisposisi yang dapat diidentifikasi .
Mendapatkan riwayat kesehatan menyeluruh , oleh karena itu , komponen penting dalam proses
manajemen . Contoh dari hal ini adalah rekomendasi yang dibuat untuk pasien yang menderita kandidosis
eritematosa kronis dalam rangka meningkatkan kebersihan mulut melalui gigi tiruan pembersihan yang
memadai . Rekomendasi tersebut akan mencakup penggunaan teratur dan sering pembersih gigi tiruan
dengan sifat anti - kandida seperti persiapan 1 % sodium hipoklorit , bersama dengan penghapusan gigi
palsu dalam semalam . Chlorhexidine ( 0,2 % ) harus digunakan jika gigi tiruan memiliki komponen
logam sejak hipoklorit jika tidak akan menyebabkan perubahan warna .
Namun, juga harus dicatat bahwa perubahan warna gigi dan permukaan mukosa telah dilaporkan dengan
penggunaan chlorhexidine ( 91 ) . Faktor predisposisi tertentu , bagaimanapun , lebih sulit jika tidak
mustahil untuk memberantas seperti di mana ada penyakit yang mendasarinya ( misalnya leukemia atau
AIDS ) . Dalam kasus ini , ditargetkan terapi antijamur , seperti dibahas kemudian,
memainkan peran penting dalam strategi manajemen . Kedua pengurangan fisik dan kimia dari Candida
beban di rongga mulut dapat dicapai dengan praktik kebersihan mulut yang baik termasuk menyikat gigi
dan penggunaan obat kumur antimikroba . Gigi menyikat Manual terbatas pada permukaan mulut
diakses , meskipun listrik atau listrik menyikat gigi mungkin lebih efektif sebagai kavitasi dalam cairan
sekitarnya dapat mengganggu Candida biofilm di situs tidak dapat diakses ( 92 ) .
Beberapa obat kumur menunjukkan aktivitas anti - candida termasuk triclosan , chlorhexidine glukonat ,

eucalyptol , dan bioflavanoids dan ini dapat memiliki aktivitas anti - kandida langsung in vitro melalui sel

Alasan untuk ini berkaitan dengan pengakuan yang relatif baru tentang pentingnya infeksi jamur manusia
dan juga fakta bahwa organisme jamur eukariotik dan berbagi banyak fitur dengan sel mamalia membuat
pilihan target yang cocok untuk agen antijamur bermasalah . Klasifikasi agen antijamur didasarkan pada
target kegiatan , dan dalam pengobatan kandidosis dua kelas yang paling umum digunakan adalah poliena
dan azol ( Tabel 4 ) .
Poliena termasuk obat amfoterisin B dan nistatin dan modus tindakan adalah melalui mengikat langsung

suspensi oral amfoterisin B dapat digunakan dalam mengobati kandidiasis mulut tahan api sering terlihat

Berbeda dengan poliena , antijamur azol yang fungistatic daripada fungisida ( 96 ) . Mekanisme tindakan
adalah dengan menghambat enzim lanosterol demethylase yang merupakan sitokrom P - 450 3 -A enzim

echinocandin antijamur telah muncul sebagai alternatif untuk azoles dan poliena ( 101 ) . Echinocandins
bertindak melalui penghambatan sintase D - glucan , yang merupakan enzim yang diperlukan untuk
sintesis dinding sel jamur . Enzim ini tidak hadir dari sel mamalia sehingga mengurangi toksisitas sel
inang potensial. Sementara echinocandins seperti caspofungin , micafungin , dan anidulafungin adalah
fungisida terhadap Candida , penggunaannya agak dibatasi oleh ukuran molekul yang besar yang
mendikte kebutuhan untuk pemberian intravena . Echinocandins yang lagi terutama digunakan dalam
pengobatan infeksi jamur invasif .
Keberhasilan pengobatan kandidosis dapat terhambat di mana ada biofilm didirikan . Candida biofilm

Kriteria inklusi dan eksklusiKedua uji klinis dan tinjauan sistematis oleh penulis lain yang memenuhi syarat untuk dimasukkan . publikasi yang termasuk dari hasil pencarian atas dasar bahwa judul dan abstrak mereka sesuai dengan kriteria broadinclusion : ( i) judul / abstrak yang relevan dengan tujuan tinjauan ; dan ( ii ) artikel itu diterbitkan dalam bahasa Inggris , Jerman, Portugis atau Spanyol . hasilDari hasil penelusuran awal , 112 artikel diidentifikasi , 25 di antaranya dipilih untuk diperiksa. Materials and MethodsSearch strategyThe literature search covered nine Anglophone databases: Biomed Central, Cochrane Oral Health Reviews, Cochrane Library, Directory Of Open Access Journals, Expanded Academic ASAP PLUS, Meta Register Of Controlled Trials – mRCT, PubMed, Science-Direct, Research Findings Electronic Register – ReFeR and two Lusophone databases: Bibliografia Brasileira Em Odontologia – BBO, Literatura Latino-Americana E Caribenha Em Ciências Da Saúde – LILACS. In order to search databases, strings of search terms were constructed, consisting of relevant text words and Boolean links. The string of English search terms: “(GIC sealant* OR Glass ionomer cement sealant) AND (caries OR tooth decay)” was used to search the Anglophone databases and the string of Portuguese search terms: “SELANTE” [Palavras] and “CIMENTOS DE IONOMEROS DE VIDRO” [Palavras] and “CARIE” [Palavras]” was used to search the Lusophone databases. All publications listed in the databases until 15 January 2008 were included in the search.Inclusion and exclusion criteriaBoth clinical trials and systematic reviews by other authors were eligible for inclusion. Publications wereincluded from the search results on the basis that their titles and abstracts were in accordance with broadinclusion criteria: (i) titles/abstracts were relevant to the review objective; and (ii) the article was published in English, German, Portuguese or Spanish. Where only a relevant title without a listed abstract was available, a full copy of the publication was assessed for inclusion. In accordance with published recommendations (17), included articles were reviewed independently by two reviewers. Disagreements were resolved through discussion and consensus. After review, articles were accepted only if they complied withall the exclusion criteria described in Table 1. In cases of multiple reports regarding the same trial, the report covering the longest period and lacking the exclusion criteria was accepted. For the systematic reviews, only a descriptive analysis was attempted.Data extraction from accepted trialsThe outcome measure of the caries preventive effect was the caries absence on sealed teeth. Two reviewers (VY and SM) independently extracted data from the accepted articles, using a pilot-tested data-extraction form that included information contained in Table 2. Wherever possible, missing data was calculated from information given in tables and text of trials in order to complete the2 2 table for meta analysis. Disagreements between reviewers during data extraction were resolved through discussion and consensus.It was anticipated that the majority of studies eligible for inclusion would be split-mouth in design. The splitmouth study design is commonly used in dentistry to test interventions and has the advantage of having an individual serve as both the experiment and control. In this study design, one or more pairs of teeth (e.g. primary molars) form the unit of randomization. Strictly, these pairs are not independent and should be analyzed as “paired data” on a patient basis. However, similar to other reviews where split-mouth trials are included (2), it was decided to analyze the pairs
independently as it would have meant that most trials considered for inclusion here would have been excluded.Quality of trialsThe quality assessment of the included trials was undertaken independently by two reviewers (VY and SM).The quality assessment process was piloted using trials not included in this review and subsequently; quality assessment rating scored by both the reviewers was derived by consensus within the review group. Four main quality criteria were examined:1) Generation of randomization sequence (Allocation), recorded as:(A) Adequate – e.g. computer generated random numbers, table of random numbers.(B) Unclear.(C) Inadequate – e.g. case record number, date of birth, date of administration, alternation.2) Allocation concealment, recorded as:(A) Adequate – e.g. central randomization, sequentially numbered sealed opaque envelopes.(B) Unclear.(C) Inadequate – e.g. open allocation schedule, unsealed or non-opaque envelopes.3) Blind outcome assessment, recorded as:(A) Yes.(B) Unclear.(C) No.(D) Not used/possible.4) Completeness of follow up (clear explanation for withdrawals and loss-to-follow-up in each treatment group) assessed as:(A) Yes, drop outs less than 30%.(B) Yes, drop outs more than 30%.(C) No explanation.Meta-analysisThe caries absence and caries presence in sealed teeth were treated as dichotomous data. Trials were assessed for their clinical and methodological heterogeneity following Cochrane guidelines (13). Trials were consideredhomogenous, if they did not differ substantially in the following clinical and methodological aspects: age ofpatients; follow-up period; type of sealant material used; frequency of sealant material application; as well asmeasured outcome. Only trials considered to be clinically and methodologically homogenous were included for meta-analysis, for which the fixed effects model of the metaanalysis software, RevMan 4.2 was used. The differences in the caries preventive effect were computed on the basis of odds ratios (OR) from each trial and the respective 95% confidence interval (CI). Studies were assigned a Mantel-Haenszel weight in direct proportion to their sample size.ResultsFrom the initial search results, 112 articles were identified, 25 of which were selected for review. Independent review of these 25 articles resulted in further exclusion of 2 reviews (8,18) and 12 trials (19-30). Table 3 provides information on the reasons for exclusion. Four trials (19,20,23,29) were excluded because the drop-out rates of participants were greater than 33%. The trial by Boksman et al. (21) was abandoned 6 months into the 3- year trial period, because only 1.7% of the GIC fissure sealants placed were available for evaluation.
Eight trials (31-38) and three literature review articles (2,11,12), were accepted and thus formed the basis for the evaluation of evidence regarding the caries-preventive effect of GIC versus that of resin-based fissure sealants.Description of accepted reviewsThree literature reviews (2,11,12) were accepted. The Cochrane systematic review (2) sought to evaluate thecaries preventive effect of resin and GIC cements in trials comparing these two interventions with each other or with a placebo (or no treatment). The strict inclusion and exclusion criteria meant that 40 of the 56 studies included for review were excluded, e.g. split-mouth trials, in which the authors did not present data in a paired way were excluded in this review without the attempt to calculate the missing data from available information. These criteria added to the strength of methodological rigor of this review 377 but resulted in similar findings in the review presented by Mejare et al. (12): although there was evidence regarding the effectiveness of resin sealants, the evidence related to GIC based sealants was perceived to be less convincing or incomplete. Moreover, the results from the comparison of resin sealants and GIC sealants were conflicting, as twoof the assessed trials (23,31) were in favor of resin, while one trial (35) reported that GIC fissure sealants performed significantly better at 44 months after placement. As the results of these trials differed substantially, the authors did not attempt a meta-analysis.The second review by Mejáre et al. (12) did not include trials comparing one type of fissure sealant material with another. Therefore, trials that pitted GIC fissure sealants against resin-based sealants for a variety of outcome measures were excluded. All of the 13 studies assessed in the review by Mejáre et al. (12) contained control groups that did not receive any intervention (i.e., fissure sealant caries preventive effect per tooth/child was compared to ‘no treatment’). Of these studies, none was graded as providing “high value” evidence; only 2 were graded as offering “moderate” evidence and most were rated as having “limited value”. The main outcome measures were relative risk reduction (the number of decayed occlusal surfaces in the controls minus the number of decayed surfaces in the sealed teeth, divided by the number of decayed surfaces in the controls) or prevented fraction (caries increment in the control minus caries increment in the sealed group, divided by the caries increment in the controls). The relative risk reductions reported were variable; ranging between 4% and 93% for all of the studies assessed. A meta-analysis, reporting on the cariespreventive effect of a single application of resin-based fissure sealants on the occlusal surfaces of 1st molars, showed that the relative risk of developing caries in a fissure-sealed tooth in relation to an untreated control was 0.67 (Confidence interval 95% CI: 0.55-0.83), which corresponded to a relative risk reduction of 33%. Only 2 of the 13 studies in the Mejáre et al. (12) review dealt specifically with GIC-type fissure sealants (31,39). Both trials reported significant caries preventive effects for GIC sealants but the strength of the evidence was rated as being of limited value. Consequently the authors’ concluded that the evidence regarding use of GIC fissure sealants wasincomplete.The systematic review by Beiruti et al. (11) was critical of the Cochrane (2) and Mejáre et al. (12) reviews, as the former excluded many trials and the latter only considered trials in which the control groups did not receive an intervention. Beiruti et al. (11) also limited their search to Medline and PubMed database entries to December 2004 and analyzed articles published in English only (94 publications identified and 12 analyzed). Of these, only randomized-control trials (RCT) were analyzed, from which a relative risk (RR) or an attributable risk (AR) could be calculated as an outcome measure for a caries-preventive effect. The GIC materials were categorized as medium viscosity, low-
viscosity, and low-viscosity resin-modified (cavity liner). The resin-based materials were grouped into ‘auto-cured’ and ‘light-cured’. Although such methodology was conceived as being more appropriate for reviewing trials comparing GIC and resin based sealants, the conclusions reached were similar to that regarding theCochrane Review: that no evidence is provided regarding the relative superiority of resin-based or GIC sealants materials in preventing caries development in pits and Table 3 Excluded articles and main reasons for exclusion 378 fissures over time.Description of accepted trialsOf the 8 clinical trials (31-38) included in this systematic review (Table 2), 7 followed a split-mouth study design (32-38) and 1 was a parallel-group study (31). In the splitmouth trials, the unit of randomization was the tooth. The split-mouth trials reported significantly different followup periods and sample sizes. All teeth under investigation were 1st permanent molars in children 6 to 11 years old, except in the trial by Kervanto-Seppälä et al. (38). In this trial, the caries-preventive effect of GIC versus resin sealants was investigated in the 2nd molars only, of children aged 12-16 years. In all split-mouth studies except the trial by Tostes (33), the interventions were randomly allocated to tooth surfaces within each pair of teeth per patient (either 1 or 2 pairs of molar teeth). In contrast, the trial by Tostes (33) randomized the teeth of each child in order to receive 3 interventions, with the fourth selected molar serving as a control (Table 2).With the exception of the Kervanto-Seppälä et al. (38) trial, where children were clinic attendees, all the other trials (31-37) covered children recruited from local schools. All the trials provided a clear description of the interventions given (Table 2) but only 2 trials (31,35) provided information on baseline caries prevalence in the form of DMFT/dmft scores: DMFT 1.81 ± 1.84 for 12-13 year olds (31) and dmft 1.64 ± 2.45 for the mean age 7 years (35). Two trials (31,36) reported a fluoride concentration ranging from 0.1 to 0.7 ppm in the water supply. Five trials (32-35,38) provided no information about the water fluoride concentration. Only three trials (31,36,38) gave information about inter/intra-examiner reliability by means of kappa scores and none of the included trials examined the effect of potential confounders on their reported results. Only 2- year data was accepted of one trial, which also reported on 4-year results. The 2-year data was chosen due to the high drop-out rate (49%) after 4 years (36).Quality of accepted trialsTable 4 provides information about quality aspects assessed for included studies. Only one study (31) couldbe regarded as a randomized controlled trial with a parallel group design. All the others were split-mouth studies, which are regarded as quasi-randomized. Details about lossto- follow-ups were reported in all included studies. Treatment allocation was rated A (Adequate) (36,37) in two trials, B (Unclear) in five (32-34) and C (Inadequate) in the remaining two (35,38).Studies that compared GIC with Resin SealantsOf the 8 accepted trials (31-38) that compared the cariespreventive effect of GIC and resin sealants, 4 trials were found in favor of resin sealants (31,36-38), 3 trials (32- 34) found that both were effective, and 1 trial (35) favored GIC over resin sealants. The Songpaisan et al. trial (31) compared GIC, resin and 0.5% hydrofluoric acid against a control group receiving no treatment. However, resin was applied only in childrenaged 12-13 years, whereas the other interventions were placed in children 7-8 years old and 12-13 years old.Although each intervention was only compared against the control group, data presented in tables in this trial enabled this research team to compare resin and GIC sealants. It was found that resin sealants performed significantly better than GIC sealants when mean DFS scores were compared at 24 months (Table 2).
The Kervanto-Seppälä et al. (38) trial studied 2nd permanent molars only, and the GIC sealant was applied only once in a 3-year follow-up period, while the resin sealants were resealed during annual evaluations, in theevent of being defective or lost.The trials by Lovadino et al. (32) and Arrow et al. (35) reported significantly greater retention rates for GICsealants when compared to resin sealants. However, all the other trials reported exactly the opposite; i.e., significantly lower retention rates for GIC sealants. Tostes (33) found no statistically significant difference in the caries preventive effect between the intervention and control groups after 2 years.Meta-analysisThe assessment for clinical and methodological heterogeneity between trials showed that the two trials(31,38) differed substantially from the others. The Songpaisan et al. (31) trial had DMFT/DFS increment asthe outcome measure. The Kerrvanto-Seppälä et al. (38) trial used repeated application of the resin-based sealant material throughout the investigation and included older children (aged 12-16 years). Therefore, neither trial was included in the meta-analysis.All six of the other trials (32-37) used split-mouth design, had caries incidence on sealed teeth as the outcomemeasure, used single material application during the investigation, included children aged between 6 to11 years and compared a low-viscosity GIC against a resin-based sealant material. These trials were consequently included for meta-analysis. Data was not presented in a paired way in 3 trials (34,36,37). However, it was possible to calculate the missing data from information provided in the tables (36,37) and in the results section of these articles (34). The result of the meta-analysis is shown in Figure 1. The pooled odds ratio (0.96, 95% CI 0.62-1.49) suggests that neither material is more effective in preventing dental karies in pits and fissures.
