ADDENDUM NO. 1

(SUPPLY, DELIVERY, INSTALLATION, CONFIGURATION, TESTING AND COMMISSIONING OFE2M RELATIONAL DATABASE MANAGEMENT SYSTEM WITH AUDIT, SECURITY AND DATABASE

APPLIANCE)

This Addendum No. 1 dated March 11, 2015 is issued to clarify, modify or amend items in theBid Documents. This shall form an integral part of the Bid Documents.

ISSUES CLARIFICATION/ AMENDMENTS

A. Cost of bid documents EighteenThousandFive Hundred Pesos(Php 18,500.00)

B. Section Ill. Bid Data Sheet

Item 5.4 Bidders shall include in their bidsa copy of Single Largest CompletedContract or Purchase Order and thecorresponding proof of completion, suchas (i) Certificate of Final Acceptance orCompletion from the bidder's client; or (H)Official Receipt of the bidder for thecontract.

Failure to submit a copy of Single LargestCompleted Contract with proof ofCompletion or a failure against theveracity of such shall be a ground fordisqualification of the bidder for awardand forfeiture of the bid security

For this purpose, similar contract shallrefer to supply of consumables forprinters.

Item 17.1Bids will be valid until June 22,2015.

Item 18.1The bid security issued in favorof the DBMshall be either in the followingforms and amount:

»- The cost of bid documents is Twenty FiveThousand Pesos(Php 25,000.00).

»- Bidders who participated in the first (t")bidding are required to pay the balance inorder to participate and obtain the biddocuments including the Supplemental! BidBulletin.

»- Item 5.4 Bidders shall include in their bids aphotocopy of the Single Largest CompletedContract (SLCC)and the corresponding proofof completion, such as: (i) Certificate of FinalAcceptance or Completion from the bidder'sclient; or (H) Official Receipt of the bidder forthe contract.

Failure to submit a photocopy of SLCCwithproof of completion or a failure against theveracity of such shall be a ground fordisqualification of the bidder for award andforfeiture of the bid security

For this purpose, similar contract is an ITproject actually completed and performed bythe bidder which includes the supply, deliveryand commissioning of RDBMS.

Bidswill be valid until July 21, 2015

The bid security issued in favor of the BOCshall be either in the following forms andamount:

Item 18.2The bid security shall be validuntil June 22, 2015

Item 29.2B VAT Returns (Form 2550Mand 2550Q) or Percentage Tax Returns(2551M) with proof of payment coveringthe months from July 2014 to December2014.

Reminders:

The bid security shall be valid until July 21,2015

VAT Returns (Form 2550M and 2550Q) orPercentage Tax Returns (2551M) with proofof payment covering the months from August2014 to January 2015

~ This item "Freebies" shall be deleted.
