When are we available?

Most Student Health Services are currently available: 8-7 Monday through Thursday

(Fall, Winter, Spring Quarters)

8-5 Friday

9-1 Saturdays (closed before holidays and no clinic on

Saturday OSU home football game days)

Eligibility -&- Costs

All enrolled students at the University are eligible to use Student Health Services Fees are Charged for Services Office visits

Health History - Screening Form is completed at Student Health Center

Required for all OIA-registered students

Problems with your health Medicines Allergies

PHOTO ID REQUIRED

Your Medical Records If you have past health records you plan to submit for your medical chart at SHS, please be sure the information has been translated into English

International Student Health Screening

Tuberculosis - Screening Have you had a previous TB test? If you had a “positive” Mantoux TB test previously,

do you have a “normal x-ray” report within the past year?

Have you had any recent vaccines?

Tuberculosis Test If at risk, blood taken at screening program (QTF-G

Test) Positive blood test, student will be contacted to

return in approximately one week to discuss result, chest x-ray will probably be done at health center

Negative blood test, result will be mailed to the student
