Call the National Passport Information Center at 1-877-487-2778

Print your application online at travel.state.gov

Form DS-11 if you are a first time applicant or under age 16*

Form DS-82 if you are renewing your passport

Bring proof of U.S. Citizenship

Bring a photocopy (front & back) of valid photo identification

Bring one 2”x 2” color passport photograph

Bring applicable passport fees

We accept personal check, cashier’s check, money order, credit card, or cash (exact change only)
