REQUEST FOR QUOTATION DOCUMENT for

1. INTRODUCTION Council has received funding from the Australian Lions Foundation, Crookwell Lions Club, Gullen Range Windfarm, Stronger Communities Programme, and Veolia Mulwaree Trust for supply and installation of a new amenities building (toilet block) in Clifton Park, Crookwell.
The supply and delivery of the toilet block has already been awarded to Modus Australia and the pre-fabricated toilet building (in kit form) and associated fixtures are expected to be delivered at project site by 26 September 2018. Under the scope of this quotation, Council is seeking quotes from appropriately licensed Contractors to undertake the works identified in accordance with the scope of works.
Scope of Works
A summary of the works is as follows, but not limited to;
To be completed by Contractor:
• Installation of the new toilet block broadly includes: excavation for the concrete pad, completing concrete pad, installation of the toilet block elements, fitting all the fixtures within the building, reinstating the disturbed ground.
• Plumbing works from the internal toilet fixtures to the connection point for water and sewer line.
• Installation of the toilet block as per the attached installation guide and drawings.
• Standard landscaping on left and right part of the toilet building. In addition to that back side landscaping should be done with such bushes that compliment the landscape and hide the back view of the building from the residents’ house.
To be completed by Contractor:
• Survey and set out for the purpose of installation work.
• Sewer and water connection from toilet block to main sewer and water line.
• Supply of prefabricated toilet building to the project site.
• Dial before you dig investigations.
General Construction Notes
• All relevant documentation to be supplied by the contractor unless noted otherwise.
• All services are to be located safely by the contractor.
• A Traffic Control Plan will be required to allow safe vehicle access to the Clifton Park. Work shall not hamper use of any of the park facilities.
• Work will not hold up local vehicle access to the nearby McIntosh road for a period exceeding 5 minutes. If required, detours may be implemented to redirect traffic.

• All work areas must have temporary safety fencing to prevent public access. Any areas where this is not possible will need to be made safe for public access/egress and warning/advisory signage erected.
• All work shall comply with applicable legislations.
• All building works are to be carried out by a suitably licenced person(s)
• All plumbing works are to be carried out by a suitably licenced person(s).
• All electrical works are to be carried out by a suitably licenced person(s).
• All work associated with this project is to be completed within 4 weeks from the date of contract award.
Work Program
• The successful company is to supply a work program in a GANTT Chart format outlining the key items and delivery time for each item, including handover date.
2. DOCUMENTATION The documentation to be supplied to Upper Lachlan Shire Council under this project comprises, but not limited to the following components;
• Prior to engagement; - Completed Price Schedule (attachment 1) - Insurance details as per section 3. - Business ABN - Company/Contractor Licence details
• Prior to associated works commencing; - Dial Before You Dig report - Trades licence details - Safe Work Method Statements (SWMS) to be approved by Council - Traffic Control Plan (to allow access of vehicles to nearby roads) to be approved by Council
NOTE: All business and trades accreditations/licences must be relevant and in date. A “stop work order” may be implemented by Upper Lachlan Shire Council where documentation has not been provided prior to associated work commencing.
3. INSURANCES Insurances required for this engagement are Public Liability ($20m), Professional Indemnity ($5m) and Workers Compensation. Certificates of currency for these will be required from the successful contractor.

Please complete the following Pricing Schedule. This engagement will be for all inclusive hourly rates of staff, plant and equipment. Materials costs will be to supply all materials with a margin for handling and administration. Completed Price Schedule to be returned to Council by 17th September 2018.

toilet block to Council.

YARRA 3 Assembly Instructions
STEP II• Starting at panels 1 & 2 put the two wall sections together using the
appropri-ate joining channel & bolts provided. • Then place corner angle over corner thread & align walls with lines on
concrete.• Wind nut & washer onto thread inside base of corner angle. Don’t fully
tighten.• Place all wall panels in the sequence as shown using the numbers on
panels matching up to the numbers shown on the panel layout drawing.
STEP I• Mark out wall centerlines on concrete pad in according to plan supplied.• Mark fixing point centre’s & drill 12mm diameter holes to 100mm deep.• Remove dust from holes as shown on chemical anchor tube.• Place a thread beside each hole.• Apply chemical anchor into holes & place 10mm s/s threaded rod (125mm
long) with 100mm into concrete & 25mm above.
Please note: work with one hole at a time as chemical anchor sets quite quickly.
STEP III• Continue placing wall panels & joining channels and fixing nut & washes
to threads.• Place all wall panels in the sequence as shown using the numbers on
panels matching up to the numbers shown on the panel layout drawing.

STEP IX• Drill and fasten verandah columns
STEP VIII• Bolt fascia purlin to barge boards. • Connect columns to fascia purlin.• Columns to lean forward on a 7 degree angle.
STEP VII• Place 3 purlins over joining channels & apply bolts in locations
provided. Don’t fully tighten. • Bolt barge boards to purlins in locations provided.• All bolts can now be fully tightened.
