Introduction
During this presentation we will be looking at the 2002 Potters bar
disaster and in particular the set of points(Points 2182A) that caused
the disaster.
We will also be looking at the aftermath of the disaster as well as what
has changed as a result of the crash and the events leading up to thecrash.

Background
On May 10th2002, the rear coach of a four coach passenger train
traveling from Kings cross to Kings Lynn derailed killing 7 people and
injuring 71 leaving 11 in a critical condition and 60 walking wounded.
As shown in the picture below the train did not come to a stop until it
lodged itself
underneath the roof of the
platform.

Background cont
Out of the people that died, 6 of them, Austen Kark. Emma Knights,
Jonael Schickler, Alexander Ogunwusi, Chia Hsin Lin and Chia Chin
Wu were on the train and 1 person, an 80 year old woman named
Agnes Quinlivan was killed by falling debris.

The lead up to the crash.

Prior Concerns:
Passenger had complained of a violent lurching of the trains as they
pass over the set of points responsible for the incident.
Reported by passengers and station announcer on various occasions.
There was a full inspection of the points on the 1st May 2002.
A maintenance team were dispatched on the 9thMay 2002.

The incident.
At 12:45 the West Anglia Great Northern train left
Kings Cross station bound for Kings Lynn station in
Norfolk.
At 12:55 travelling at approximately 97mph the train
crossed the points and the fourth carriage derailed.
The derailment caused the carriage to hit the under
bridge and roll up to, and getting wedged between the
platform and the roof of the station.

The Cause
A fault with set of points 2182A caused the last carriage to derail.
Two nuts were missing at this set of points putting excessive strain on
the lock stretcher bars.
This caused fractures allowing the points to switch as the vibration fromthe train passed over the points.

The damage caused.

The physical damage caused was the damage to the rail and points
as well as the bridge, carriage and the station.
There was the cost of human life.
The corporate damage was extensive and damning.

Investigations by the British transport police and HSE
Victims of the incident called for a public inquiry but were never givenone
20% of points in the area found to have loose bolts
HSE told railtrack to review the design of them
Jarvis and railtrack found to be at blame

Bolts in adjustment bars notbeing fitted found to be the cause
This was because Jarvis had apoor maintenance routine
Also partly railtracks fault forgiving wrong instruction
Could have been avoided by
regular visual inspection byJarvis and correct informationgiven by railtrack

Railtrack took most of the maintenance in house
A review of the design of the points was carried out
