The ULTIMATE protocol has been developed and clinically validated in partnership with a multi-centre team of dental surgeons. This protocol provides an innovative approach to the preparation of the implant site using the latest technological advances in cutting instruments for bone surgery.Designed for maximum adaptability whatever the bone density, the ULTIMATE surgery protocol provides an effective response to the most demanding requirements. The very gradual drilling sequence which is homothetic to the shape of the In-Kone® range of implants enables an initial close contact between the bone and the titanium to be obtained, which encourages uniform distribution of the primary stability.
The ULTIMATE protocol is compatible with all 3.5 - 4 - 4.5 and 5 mm diameter In-Kone® implants

The vertical positioning of the In-Kone® implant depends on a number of parameters: appearance, tissue thickness and available vertical height. The sealed connector of the In-Kone® implant enables it to be embedded subcrestally, if the bone volume permits Embedding can be recommended to limit the stresses on the bone and for ease of management of the prosthesis. For example, if the practitioner wants to embed a 10 mm long In-Kone® implant 1.5 mm subcrestally, he can use the 11.5 mm height markings on the drills or the 11.5 mm height series of depth stops for reference. If there is a great deal of resistance when inserting the implant, remove the implant, replace it in its titanium support, check the drilling depth with the gauge or the drill, and consider additional widening of the site if necessary.
1 - Hold the implant with the contra-angle implant driver (DCPICACI2 or -L) or manual implant driver (DCPIMCI2-1.2 or -L). Always check that the driver is engaged in the implant. If necessary rotate the driver slightly axially until the hexagonal
end is fully inserted in the indexation in the base of the connector.
2 - Screw into the mouth. Start at 25 N.cm and gradually increase. For 6 mm long implants, start at 35 N.cm.
3 - Screw in manually with the implant driver (DCPIMCI2-1.2 or -L) and the ratchet wrench (DCCLIC-2). Position one of the facets of the implant driver in a vestibular position so that the indexed prosthetic pillar abutments can be repositioned easily.
4 - Fit the healing screw or cover screw with the manual implant driver (DCPIMCI2-1.2 or -L) or with the hex screwdriver (DCM1.2, C or L). Tighten manually (10 N.cm maximum).

The kit is read from left to right with the coloured rubber ring identifying the final drill for the required implant diameter (ill.1). The preceding drill offers an under-drilling solution in low density bone and the next drill can be used for over-drilling in high density bone (ill.2).
There are several possible protocols for each implant diameter (3.5, 4.0, 4.5 and 5.0) according to the bone density encountered. The engravings on the drills do not take account of subcrestal positioning of the implant. It is therefore necessary to go to the next graduation to position the implant subcrestally (for example marking engraved at 11.5 mm for a 10 mm long implant positioned 1.5 mm subcrestally).

! To avoid damaging the internal construction of the implant, it is advisable not to exceed the contra-angle tightening torque of 50 N.cm for In-Kone® Universal & In-Kone® PRIMO implants. Use a surgery motor with regularly calibrated torque control for this or the torque wrench supplied in the kit (if final tightening is carried out manually, do not exceed 70 N.cm).
