daughter___________________________to participate the JOINT BSP/GSP CAMPORAL and

ENCAMPMENT 2014 on January 22-24,2014 at Matuog Elementary School,Sitio Matuog,

Brgy. Basak, Guihulngan , Negros Oriental.

I understand that the school would give priority for the safety of my child. However,

they may not be held responsible for any untoward incidents that might happen beyond their

control.
