2) BankStatementorCertificationfromthebankstatingyouhaveenoughmoneytopayfor

thesemester

Step1:Downloadthe Financial Statement and Certification file

Step2:Filloutthehighlightedfields:


Filloutthegreenhighlightedareaifthebankaccountyouareprovidingdocumentationfromisinyourname.Thisrequiresyoursignature.

Filloutthepurplehighlightedareaifthebankaccountyouareprovidingdocumentationfromisinsomeoneelse’sname.Thisrequirestheirsignature.

Onlyfilloutthelastsectionifyouarereceivingascholarshiporgrantfromanoutsideagencyorfromyouruniversity.Inthiscase,youwoulduploadacopyoftheawardletterfromthatagencyoraletterfromyourhomeuniversity.Ifthefundingdoesnotcoverallexpenses,youneedtoincludeacopyofabankstatementasnotedabovetoshowthatyouhavetheremainingfundsavailable..

Step3:SavethisdocumentanduploadittotheUCBerkeleyExtensionwebsiteasan“AdditionalDocument”.Seebelowforinstructions.
