Course Description
This course is the required obstetrics and gynecology clerkship for 3rd year medical students. During
this course, students will encounter pregnant patients during their antepartum, intrapartum, and
postpartum care. Ambulatory time will provide students with participation in office obstetrics and
gynecology, including prenatal care, contraception, and preventive medicine. Inpatient
gynecology/gynecologic oncology places the student on the inpatient team, providing surgical,
postoperative, and non-surgical inpatient care. Case based learning and simulations will augment the
clinical experience.

Rotation Overview
The Obstetrics and Gynecology clerkship is designed as a combination of clinical learning experiences as
well as didactics in the form of case-based learning and simulations. The goals of the clerkship are:
1. To provide opportunities that will help students develop knowledge, skills, attitudes, and
principles that are essential to the ObGyn physician.
2. To provide a representative sample of the range of common problems and their presentations
encountered in Obstetrics and Gynecology.
3. To use community-centered clinical experiences as authentic contexts for students’ mastery of
the competencies of Obstetrics and Gynecology.
4. To provide integration of obstetrical and gynecological content in the M3 curriculum.
ObGyn physicians provide comprehensive and continuing health care to females, regardless of age or the
nature or presentation of the problems encountered. Students will have opportunities to apply their
clinical knowledge and skills to a wide and diverse range of patient problems and presentations. They
will incorporate medical, psychosocial, and preventive aspects into the differential diagnoses and
management plans. Case based learning, simulations, and use of provided online quiz modules will
supplement the clinical component of this clerkship.

Required Activities
Each student will be provided a day-to-day schedule for their clinical rotations. Our AHEC and Riverside
students will be provided their schedules from their clinical sites. There are additional sessions required
of all students during the Obstetrics and Gynecology Clerkship. These sessions are required unless
otherwise stated. Please note that these sessions take precedence over any scheduled activities at all
clerkship sites. Also note that the day of the week is subject to change depending on university holidays.
In the SARS – CoV2 environment most of these will be by web-based platform.
1. Clerkship first and second days of orientation/lectures (TTH 2nd floor Legacy, Academic
Classrooms and TTH ObGyn Simulation Room, see schedule as locations may vary)
a. first and second days of the rotation
i Monday or Tuesday (First Day – web-based via BlackBoard Collaborate)
ii Tuesday or Wednesday (Second Day – partially web-based and on-site sessions
(see schedule for more details)
2. Case Based Learning (CBL) sessions (web-based until further notice, check schedule for
location, dates, and times)
a. These are mandatory for all students including AHEC and Riverside.
i If a session is missed the corresponding worksheet must be completed and turned
into the Clerkship Coordinator with 5 days of the missed session.
b. First and second day of the rotation during orientation and second, third Thursday
mornings and fourth Friday afternoons of the rotation.
i If the second Thursday falls on a Grand Rounds day (the second Thursday of the
Month) those 2 hours will be made up in an evening session that same week
ii The fourth Friday afternoon following OSCE, Breast and Pelvic (see schedule)
1 If OSCE, Breast and Pelvic evaluations fall on a Monday CBL’s will be
schedule either the fourth Thursday am or Friday pm. (see schedule)
c. Will be scheduled with consideration for other educational activities.
d. Completion of APGO uWise Modules and turning in the quizzes with > 70% that
correlate to CBL topics.
e. AHEC and Riverside students will be required to attend via Blackboard Collaborative.
3. ObGyn Grand Rounds and M&M Conferences (location: Zoom meeting)
a. 8:30am Second Thursday morning of each month (additional information sent by
Clerkship Coordinator)
b. Riverside and AHEC students are required to attend.
4. Tumor Board (Location: Teleconference) while on Gyn/GynOnc Inpatient service

however attend Tumor Board if their site holds them.

Required Clinical Experiences
During the Obstetrics and Gynecology Clerkship, students are required to evaluate patients in the
following diagnostic categories representing the common problems seen in a ObGyn practice setting.
This provides the core of the ObGyn experiences. Patients may be seen in ambulatory, outpatient, or
inpatient settings. Students are required to keep their logs up to date and the logs will be monitored
weekly to ensure adequate experiences. If multiple problems are addressed with one patient at a given
encounter, then up to 2-3 appropriate diagnostic categories can be entered for that patient. Students will
use the 5 Level of Involvement checklist in Med Ed to determine if a patient encounter meets the
minimum level of involvement to count towards this requirement.
Level of involvement:
In addition to seeing patients in the diagnostic categories listed below, how the students are engaged in
the encounter is also an important factor in helping students achieve the objectives for this clerkship.
Level of involvement is likely to include various types of interactions with patients and the health care
team and should be monitored to ensure a complete experience. Levels of involvement will be indicated
for logged patient encounters. The logs will be reviewed mid-block to ensure that students have a range
of experiences in both in-patient and/or out-patient settings.

If the student is unsure of the appropriate category for a given diagnosis, they can discuss it with their
preceptor or with the Clerkship Director.
Students are required to use RocketMed, the electronic, web-based database, to keep a log that documents
the types of patients seen and the level of responsibility. Procedures must also be logged. Students are
expected to log in to the RocketMed portal, https://rocketmed.utoledo.edu/, for each patient they
encounter, and two to three diagnoses can be entered for each patient. Once the required cases have been
entered (i.e., one per diagnostic category required), students are expected to log at least one procedure or
diagnosis category per day. Students are encouraged to log cases that are particularly interesting or
educational. All seven procedures and/or diagnosis categories for the week can be entered on Sunday.
The expectation is that by Monday morning of each week the cases will be updated.

Textbook: Beckmann and Ling’s Eighth Edition is the recommended textbook. The department has
copies available to loan. Please contact the Clerkship Coordinator to arrange to borrow.
Another recommended textbook is Hacker and Moore’s Essentials of Obstetrics and Gynecology 6th

This website contains the uWise modules, worksheets and quizzes that will be used throughout the
ObGyn Clerkship. Reviewing these modules and completing the worksheets and quizzes will enhance
your understanding of these problems and your ability to apply the essential competencies of Obstetrics
and Gynecology to them. Students can register on the site for free; they must use the above link and their
University of Toledo email address. Registering on the site allows students access to a multiple-choice
board review 100 question test which, although not designed for preparing for the NMBE Obstetrics and
Gynecology Subject Exam, are successfully used by students across the country to help prepare for the
exam.

Mid-Clerkship Feedback
During the 1st and 2nd weeks of the rotation students will be given formative feedback by their preceptor/s.
Preceptors will complete the Mid-Clerkship Formative Feedback Form (provided during orientation) and
review it with the student. The student is required to have two feedback forms completed by the mid-
clerkship evaluation (schedule provided during orientation and available on Blackboard), one observed
history form and the portfolio notes appropriate for the first half of the clerkship. The forms should be
uploaded to BlackBoard. It is the student’s responsibility to ensure they receive the completed signed
feedback forms from their preceptors. The Clerkship Director will review these forms, case log entry,
and other relevant materials with each student during their mid-clerkship evaluation. AHEC and
Riverside Mid Clerkship meetings will be held via phone with the Clerkship Director and onsite.
Grading
Calculation of grades is as follows:
Clinical Competence Score (Evaluations) 50%
Subject Exam 30%
Departmental Grade 20%
1. Clinical Competency Score (50%)
A composite clinical grade is based on clinical evaluations received from attendings, residents and
advanced practice practitioners with whom the student has worked with at the various clinical sites. The
evaluations are time weighted and use a 1-5 scale:
5 Significantly above expected competency
4 Above expected competency
3 At expected competency
2 Below expected competency
1 Significantly below expected competency
Students are not to discuss their evaluations with the preceptors. Students should direct all comments and
questions to the Clerkship Director.
24 or less points gained for clinical portion of the competence grade will result in a grade of Fail.
Failure due to poor clinical competence evaluation will require completion of the clerkship in its entirety.

2. NBME Subject Examination (30%)
Students take the Subject Exam on the last day of the clerkship.
Students will take the NBME Subject Examination as a component of the overall grade in this course.
• Students must earn a minimum passing score of the 5th percentile based on the prior’s year
national performance date (<5th percentile is a failing score.)
• That percentile will be set based on the NBME entire academic year data.
• A score of less that the 5th percentile on the NBME Subject Exam will require remediation of
the examination and the final grade will be a DEFER until it is completed.
The Subject Exam is scheduled on the last day of the clerkship. The exam is multiple choice and is
created and administered electronically by the NBME. According to NBME protocol, all students must
start the exam at the same time; therefore, if you are late and the exam has started, you will be unable to
sit for the exam at that time. Students will need to reschedule the exam through the Office of Student
Affairs. For time and location, please refer to the Orientation Packet schedule.
Failure to achieve the 5th percentile will require the student to retake the subject examination and a grade
of DEFER will be assigned. A second attempt to complete the NBME subject examination must be
completed within one year from the first attempt. If the student achieves the 5th percentile or higher on
the second attempt a grade of PASS will be assigned. PASS is the highest grade that can be achieved
after a DEFER grade due to an initial failure to achieve the 5th percentile on the NBME Subject
Examination. A second failure of the NBME subject examination (See policy #10-04-00008-706)
requires a 5-week remedial clerkship to be completed.
The ObGyn Clerkship final grade scale is as follows:
• Minimum Passing score for the Obstetrics and Gynecology Subject Exam > 64
• Scale for Clerkship Final Grade: High Pass > 83, Honors > 89 This is subject to
change as we are waiting for AY2022 numbers from the Department of Medical
Education.
3. Departmental Grade (20%)
The Clerkship Educational Program is composed of the various components described earlier in the
handbook. Points are awarded for successful completion of the projects along the following guidelines:
OSCE - 4 points.
Students will have a history based OSCE focused on an ambulatory issue. You will be expected to gather
a focused history, discuss with the patient your thoughts and the next best step and conclude the visit in a
professional manner Each station will request some portion of expected tasks, forming a checklist for
proper completion of the station. Students receive credit for each expected task they do at each station
and this goes towards the raw OSCE score. NOTE: these “credits” are used to determine their raw OSCE
score and are not the same as the points used in calculation of the clerkship grade. The raw OSCE score
is then converted to a percentage of tasks completed and then to the points awarded.
Minimum passing score for the OSCE is 60% of the tasks required.
Provided scoring is available, this will be reviewed at the end of clerkship meeting with each student.
Review is for feedback only; no scoring alterations will be made.

Breast and Pelvic Exam Evaluation - 4 points.
The breast and pelvic exams are evaluated based on the checklist provided at the beginning of the
clerkship and taught during orientation. The number of points awarded is based on the number of tasks
on the checklist accomplished.
Minimum passing score for the Breast and Pelvic Evaluation is 60% of tasks required.
Provided scoring is available, this will be reviewed at the end of clerkship meeting with each student.
Review is for feedback only; no scoring alterations will be made.
Contraceptive Counseling – 1 Point
This can be accomplished with role play with a provider (i.e., APN, CNM, Attending, or Resident) or
direct observation by a provider in the clinical arena. Role play is an interactive activity between student
and provider. A performance checklist is provided. Students should evaluate a patient’s eligibility for
different contraceptive options, discuss with the patient (resident or attending) the options available based
on the patient’s own context and preferences, risks/expectations/benefits and identify those methods
contraindicated (if any) for that patient. A form with specific scenarios is provided. Direct observation in
the clinical environment by a provider will be evaluated on the Contraceptive Counseling in the Clinical
Environment Card. The completed card or form will be uploaded to BlackBoard to be graded.
CBL/Orientation and Associated APGO Quiz Modules (total of 27) – 4 points
Students are required to attend all CBL’s and turn in quiz modules for each topic with > 70% score.
• Completed modules due by the Monday after at each of the CBL sessions.
o Tracked through uWise by coordinator.
• Number of points awarded will be percentage of quiz modules turned in with required score.
Completion of Logs and Assignment Cards – 2 points.
Student will be required to log their hours and required experiences in the RocketMed portal. They will
also have three assignments cards to complete.

All cards must be signed off by preceptor, which include faculty, resident, mid-wife and CNP. RN’s,
LPN’s or MA’s are not accepted signatures (except for interpretation of fetal tracing, obtaining fetal heart
tones).
All assignment cards must be turned in/submitted on BlackBoard no later than 5pm the day prior to the
student taking their NBME. Anything turned in after this will not receive credit.
All observed procedures (i.e., deliveries and hysterectomies) should be logged in the RocketMed portal.

Completion of 12 non-CBL APGO Quiz Modules with ≥ 70% score – 2 points
Student will be required to complete 12 APGO Quiz modules that are not covered during the CBL
sessions. Highly recommended a minimum of three modules per week should be completed. This will
be tracked through uWise by the coordinator. All non-CBL quiz modules must be completed no later
than 5pm the day prior to the student taking their NBME. Anything turned in after this will not receive
credit.
Portfolio Notes – 3 points.
Students will be required to complete 3 portfolio notes. They must be 3 of the 7 listed below and only 1
from each category.
• Gyn Problem Focused, Office Visit
• Gyn Post-Op
• OB H&P
• D/C Summary
• Annual Well Woman Exam
• Post-Partum Note
• Antepartum Note
All notes must be edited/critiqued by preceptor (faculty, resident, midwife, APN) and signed. Any
corrections or suggestions must be made to the note/s. Students will then turn in both the original
marked-up note and a clean copy (original plus incorporated suggestions) to Blackboard making sure all
patient identifiers are removed. If no corrections are made by preceptor, student will turn in original with
preceptor/s signature. NO ASSIGNMENT WILL BE ACCEPTED WITH HIPPA IDENTIFIERS,
INCLUDING PATIENT INITIALS.
All notes must be turned in no later than 5 pm the day prior to the student taking their NBME. Anything
turned in after this will not receive credit.
Failure of the departmental educational program (< 9 points) requires a repeat of the component until it is
successfully passed. A grade of DEFER will be submitted to the Registrar’s Office pending successful
remediation of the failed component(s) of the departmental educational program.
A minimum of 10 accumulated points must be achieved for the Departmental Grade to pass the clerkship
When a student has remediated all failed components of the departmental educational program, a final
grade no higher than PASS will be submitted to the Registrar’s Office.

• Student Professionalism
Medical Students are held to the highest standards of professionalism and have a number of professional responsibilities that they are obligated to uphold. Breach of a professionalism standard will result in a professionalism behavior report based on the institutional policy.
• Conflict of Interest – Separation of Roles
The health professionals who provide sensitive medical and healthcare services to medical students will have no involvement in the academic, professionalism, or disciplinary evaluation, promotion, or dismissal of students receiving those services. Inform the Coordinator of any such relationships within the department prior to the start of the clerkship.
• Assigned Education Hours
To ensure that there is an appropriate balance of assigned educational activities and self-directed learning, students’ scheduled activities should:
o Not exceed 80 hours per week, with no more than 30 consecutive hours,
o Have at least 24 consecutive hours (1 day) of non-scheduled time per week.
o Notify the clerkship coordinator if they are approaching 70 hrs/week.
• Blood Borne Pathogen Exposure Policy/Protocols
In the event of blood or body fluid exposure:
When at The University of Toledo Health Science Campus:
1. Flush area thoroughly for 15 minutes; wash with soap if applicable.
2. Notify supervising faculty member or preceptor.
3. Always report to UTMC Emergency Department for initial assessment of injury and exposure.
4. Immediately obtain and submit all lab results from both you and the primary source of contact to
Ruppert Health Center, Family Medicine.
5. Complete a UTMC employee injury report at utoledo.edu/depts/safety/Forms.html and send to
Safety and Health.
6. After hours/weekends, call UT Emergency Department at 419.383.3888 to address questions or
concerns.
7. Notify clerkship director of incident.
Whenever exposure occurs at a non-UTMC site:
Contact the supervisor or preceptor at site to find out individual protocol. Follow up treatment after
initial treatment may be obtained at occupational health which is located in the Emergency
Department of University of Toledo. Exposed medical student needs to bring appropriate records
from outside facility to avoid delays in treatment. Occupational health can be reached at 419-383-
5598. Also inform the clerkship office.
COMLS Policy Supervision of Medical Students
Excerpts from policy:
When performing any procedures or clinical activity which poses any risk to patient safety,
students must be directly supervised by a faculty member or resident or fellow at the hospital or other clinical facility.
In certain clinical situations, Advanced Practice Providers (APP) such as Physician Assistants, Nurse
Practitioners and Midwives may provide supervision, if noted to be within their scope of practice.
*Direct Observation/Supervision: the observing/supervising physician is physically present with the student and patient.
