Why Lockout Tagout?
• Equipment may accidentally be energized while servicing
• Sudden release of stored energy can cause:
• Extreme injury, loss of limb, death, etc.
• Releasing energy and locking out removes the risk
• Tagging equipment lets others know why it is down
• Lockout Tagout is for your safety, do not neglect it!
Energy Sources• Gravity
• Pumping Unit Weights
• Racked Pipe
• Pressure• Valves
• Electricity• Wire terminals
• Power Switches
• Mechanical• Motors
• Pumps
LOTO Procedure
• STEP 1: Stop the equipment
• STEP 2: Release stored energy
• STEP 3: Place a lock on the start switch
• STEP 4: Place a tag on the lock
• STEP 5: Clear area of other people
• STEP 6: Try to start the equipment, equipment must fail to start
• Consult equipment SOP if available
Tag Information
• Always place the following information on the tag:
• A description of the work being done
• Whose lock is on the equipment
• When work will be complete
• How to contact the person doing the work
