Objectives
ECLIPSE stands for: “Electronic Checking Leading to Improved Prescribing Safety & Efficiency”. It is utilised in 4000 surgeries in England and we have subscribed to the advanced “LIVE” version to allow true Risk Stratification, Safety Alerts, Centralised Project Management, Integrated Care and Automated Patient Care Plans. The aim is to make your life easier whilst enhancing the safety of our patients. The system is an online interface found at www.eclipsesolutions.org
Risk Stratification: The Eclipse Live system is currently the most sophisticated risk stratification system available with the ability to run thousands of safety algorithms against all of your patients. It will reliably identify those patients at risk of complications / admission and generates user-friendly lists for easy management.
Integrated Care: The Eclipse Live system allows complete integration of care whilst protecting the security and confidentiality of the Patient. GP Practice, Community Services, Pharmacy, Hospital and Patient / Carer can interface with ease through our system with the creation of a completely functionally integrated generic team.
Self-Management Plans and personalised records: These automated self-management plans were first launched in 2007 and were immediately able to reduce admissions in diabetes by more than 50%1. Eclipse Live now can create plans for multiple conditions, enabling patients and carers to be both empowered by education and choices. Patients are able to access their medical records electronically or via paper summary printouts.

Eclipse Live : Practice Commitment
Introduction The Eclipse Live audit is a fully automated process that allows a practice to participate in the elp project easily and with minimal effort. In general terms the process is as follows. The practice signs an authorisation form to permit three things to happen:
Remote installation of necessary software if required Automated running of the elp SQL Suite query Remote login to provide technical support if required
Once set up, the elp query will be scheduled by Apollo to run automatically as required. When run, the Eclipse Live query will produce a .csv datafile containing anonymised clinical data (no names, dates of birth, addresses or personally identifiable data). This file will be automatically encrypted and transmitted to the secure Eclipse Live database. The Process Installation Once a signed authorisation form is received from a practice whose clinical system is compatible with Apollo SQL Suite, Apollo will carry out checks to determine if the practice already has a working version of SQL Suite; if this is the case, Eclipse Live query is scheduled via Apollo’s A3 system to run at the next opportunity. If not, Apollo will contact the practice to arrange installation of the necessary components and then schedule the automated run as above. Automated Running The Eclipse Live query will normally run during the early hours of the morning (typically 0200 – 0700) and does not interfere with system processes such as backups etc. If the query is unable to complete it will simply re-try on successive days until it does complete. For most practices, there is a longer “run window” on a Sunday, typically 0900 – 1900 so even if the query cannot complete during the week it will almost always complete by the Sunday following its scheduled run date. Audit Criteria Patients are selected for inclusion based on the criteria outlined in the extraction specification document.

Eclipse Live / Diabetes Manager Audit: Practice Commitment On behalf of the practice named below (“Practice”), I confirm that: I authorise the provision of the Eclipse Live audit in accordance with the conditions specified within this document. I have read the conditions applying to the provision of the Eclipse Live audit and understand and accept the role and responsibilities of the Practice in connection with the provision of the Eclipse Live audit, especially the necessity to leave switched on at all times, the Practice PC onto which the software is installed. I have made my partners and other stakeholders at the Practice aware of the Eclipse Live audit and the conditions relating to its provision. I am authorised to approve the Eclipse Live audit on behalf of the whole Practice. I confirm I have received information about the type of data that will be extracted from the practice system and transmitted to the Eclipse Live database, to facilitate reporting on the extracted data. The Practice agrees to the installation and operation of the Eclipse Live audit software and accepts the terms of the Software licence agreement. I understand that it may be necessary for an Apollo technician to remotely install and maintain the Eclipse Live audit software components and I authorise, when required, remote access to the Practice system to allow installation of the software and for the provision of any necessary technical support.

Eclipse Live Authorisation Document: End User Agreement END USER REQUIREMENTS AGREEMENT This End User Requirements Agreement (the “Agreement”) is made between Apollo Medical Systems Ltd (“us/we”) and you as the end user (“you”). “Software”: means the Apollo SQL Suite, Version [] (and such updates that we may provide to you from time to time) and associated documentation. By installing, or using this software, you agree to be bound by the terms of this agreement. If you do not agree to the terms of this agreement, do not install or use the software. 1. OBLIGATIONS 1.1 We hereby grant to you a non-exclusive, non-transferable, limited right to use the software for your own internal purposes only, provided that you must not: 1.1.1. decompile, reverse engineer, disassemble, or otherwise reduce the software to a human readable form, except as permitted by law; 1.1.2. modify, sell, rent, transfer, resell for profit, distribute or create derivative works based on the software or any part of it; or 1.1.3. make copies of the software other than one copy in machine readable form solely for back-up purposes. 1.2 You hereby give us express permission to disable the software remotely in the event of your unauthorised use of the software and/or following the expiry of this agreement. 2. OWNERSHIP 2.1. We own the copyright in the software and of any authorised copies you make. 2.2. You own only the CD-ROM or medium on which the software is recorded or fixed. 3. WARRANTY 3.1. We warrant that the medium on which we supplied the software to you will be free from defects under normal use for a period of thirty (30) days from supply. If a defect within the warranty period occurs you will be entitled to either a replacement or a full refund. 3.2. The software is provided to you without technical support or warranty of any kind from us including, without limitation, a warranty of satisfactory quality, fitness for a particular purpose and non-infringement of third party intellectual property rights. 4. LIABILITY 4.1. Neither we nor any of our directors, officers, employees, subcontractors or agents shall be liable to you (including without limitation, incontract, negligence and tort liability) for any loss of profit, loss of revenue, loss of opportunity or goodwill or for any consequential or indirect loss or damage in connection with this agreement. You acknowledge that the software has been priced on the basis of this restriction of liability. If you require us to accept a higher level of liability you may contact us and we will provide you with a revised quotation for the software. 4.2. Notwithstanding the foregoing, our liability for (1) death and personal injury or (2) fraudulent misrepresentation is unlimited. 5. TERMINATION 5.1. This agreement will terminate automatically on expiry of the term indicated on-screen during the installation process or otherwise set out in the associated documentation, whereupon you must cease using the software and either take appropriate steps to renew your agreement (either by contacting us or one of our authorised agents) or delete and destroy your copies of the software. 5.2. This agreement will terminate automatically if you fail to comply with any of these provisions. 6. GENERAL 6.1. This agreement contains the entire agreement of the parties and supersedes all prior or contemporaneous agreement or understandings, whether written or oral. 6.2. Any person who is not a party to this agreement shall have no right to enforce any term against us under the Contract (Rights of Third Parties) Act 1999. 6.3. Ten Simple Steps to Activation
Month 1:
Step 1: All practices will be sent the eclipse live implementation pack.
Step 2: Two Partners and your Practice Manager need to fill out and sign pages 15 & 16 of the agreement form.
Step3: Post your completed copy to the PCT/CCG
Step4: Apollo will contact you and arrange for your initial data extraction. (This will contain no personally identifiable data and not extract patients who have opted out of the NHS Spine.)
Step 5: An overnight weekly extract will then be scheduled to keep your eclipse live interface up to date.
Step 6: You Practice Administrator will be able to activate your eclipse live usernames and passwords and will be guided through this simple process by our help desk.
Month 2:
Step 7: Once you have logged in you will be automatically activated to receive your eclipse live workbooks and be able to book an online one to one training session.
Step 8: Weekly Training Webinars allowing up to 16 users will be carried out on Wednesday and Thursday lunchtimes.
Step 9: Your surgery Alerts will be received once a week on the night following your weekly extract.
Step 10: There will be regular general training sessions for your CCG with the ability to feedback any concerns or highlight development opportunities.

The Future
Thanks to this project, lives are already being saved, with more than a million patients already being protected by secondary automated screening and risk alerts.
Integrated care Specialists are now able to track all their patients within the community, GPs are able to discuss cases directly with specialists and community staff are now fully informed.
Patients have access to their health summary, can generate self-management plans and interact with their health care team.
