RATIONALAlthough over last 20 years there was a significant extension of the therapeutic expertise in regards to chronic lymphocytic leukemia (CLL), and not only an increase in therapeutic responses has been achieved but also a better quality of them, CLL is still considered a not eradicable disease, with many patients presenting recurrent relapses. In many years, the chemotherapy represented the unique approach to the CLL treatment. However, introducing the monoclonal antibodies like the anti CD20, has allowed the achievement of better results with the introduction of the chemo-immunotherapy association in the clinical trials.During these last years, the new biological knowledge of the mechanism of the disease has allowed the development of agents able to inhibit the intracellular signaling of the B cells receptor. The approval and commercialization of two kinase’s inhibitors (idelalisib and ibrutinib) have changed the quality of treatment for patients with a relapsing disease.Furthermore, the most recent clinical trials showed how the introduction of kinase’s inhibitors can produce surprising benefits for patients who have never been treated before, as well as for subjects with a bad prognosis. This is leading to a thorough review of the treatment principles of the disease. However, this story seems to have just begun, as proved by the recent clinical studies of BH-3 mimetics: a new class of anti-cancer agents mimicking the activity of phisiological antagonists of BCL2 in order to trigger apoptosis. Venetoclax, a strong inhibitor selective of BCL2, has proved to have an important anti-cancer activity in patients with CLL relapse, including those with bad prognosis. In this new scenario, plenty of news as far as monoclonal antibodies CD20 are concerned- especially with the recent obinutuzumab and ofatumumab approval - it appears increasingly important to understand the kind of patients who can benift from the chemo-immunotherapy rather than those who could better benefit from the new approach of the new molecules.This represents a new challenge for the hematologists. Therefore, a new correct training of the specialists appears essential in order to achieve a correct use of the new and old resources and to well manage them.
