Dear Friends and Supporters,
In the past year, Touchstone Mental Health has continued to meet the changing needs of people in our community. Lyric Lane, our new residential treatment program in Fridley, served 236 people, of which, 168 received crisis residential services. Residential crisis services offers an alternative to hospitalization and can bridge the gap for people who are struggling to make it through each day. The Fridley Lions Club made a generous donation that allowed Touchstone to transform Lyric Lane’s backyard into a pleasant patio with shrubs and flowers, bringing nature to people in the program.
Our highly successful Community Housing Programs served 142 people in the past year, of which, 93% maintained their housing for six months, and 81% retained their housing for at least twelve months. We received funding from the State of MN to expand Housing Innovations and received continued support for Project Community Success.
We rely on supporter and foundation support to bring complementary approaches to whole person health for people we serve. We were able to meet basic needs for 169 people with generous support from our donors, the Richard Schulze, Fredrikson & Byron and Allina Foundations. Support from Aroha Philanthropies brought creative expression through music, movement and art for 160 people in our community and residential programs. These opportunities wouldn’t be possible without our supporters, donors, funders and partners.

Behavioral Health Home and Care CoordinationBehavioral Health Home and Care Coordination are community programs which help individuals find community resources and coordinate physical and mental health care. Our staff work with individuals to create a plan that meets individual needs and preferences.
Case ManagementCase Management connects people to medical, mental health, housing, financial, vocational, social and other necessary resources to meet their mental health needs and support their independence.
Housing Innovation ProgramHousing Innovation assists people in finding and maintaining their independence by providing intensive in-home supports that includes independent living skills, service coordination, mental health, nursing and 24-hour emergency services.
Intensive Residential Treatment ServicesShort-term residential treatment designed to improve mental health stability, personal and emotional adjustment, self-sufficiency and community independence. Services are offered in Bloomington, Minneapolis and Lyric Lane, in Fridley. Lyric Lane also offers Crisis Residential Services that serves individuals in crisis with immediate treatment needs.
Intentional CommunitiesThe Intentional Communities Program provides in-home services to build household management, social and community skills. Program members meet weekly as a community where peers support each other in their mental health recovery.
Project Community SuccessProject Community Success assists people to locate and move into an apartment through transition planning and services. Staff provide housing support which includes tenant education and resources which help people be successful in their housing.
Rising Cedar and New Hope ApartmentsRising Cedar and New Hope Apartments provide permanent housing with assisted living services that include counseling, nursing, housekeeping, service coordination, community services and 24-hour support. Apartments are located in Minneapolis and New Hope.
Touchstone Fitness CenterThe Fitness Center offers a variety of machines, weights, personalized fitness training, individual and group member-ships. The Fitness Center is open for anyone in the community and is located in the Minneapolis Seward Community.
