An active applicant is one who submits a certified rank order list of programs. The NRMP classifies SMS applicants into 6 applicant types:
Graduates of U.S. allopathic medical schools: a graduate of a Liaison Committee on Medical Education (LCME) accredited U.S. allopathic school of medicine
Graduates of Canadian medical schools: a graduate of a Committee on Accreditation of Canadian Medical Schools (CACMS) accredited Canadian school of medicine
Graduates of osteopathic medical schools: a graduate of an American Osteopathic Association (AOA) accredited school of medicine.

The National Resident Matching Program (NRMP) is a private, not-for-profit corporation established in 1952 to optimize the rank ordered choices of applicants and program directors. The NRMP is not an application processing service; rather, it provides an impartial venue for matching applicants' and programs' preferences for each other consistently. In addition to the Main Residency Match for first and second year residency positions, the NRMP's Specialties Matching Service (SMS) conducts matches for advanced residency and fellowship positions. The NRMP conducted its first fellowship match in 1984 for Colon and Rectal Surgery, and in 2010 provided services for 38 sub-specialties within 18 separate matches. A full list of the specialties participating in the SMS for the 2010 appointment year can be found in Figure 1.
The specialty matches occur throughout the year, and each requires its own registration process with the NRMP.Fellowship positions generally begin in July. Because fellowship matches have appointments that start anywhere from 6 to 14 months after their match days, we have organized this report based on appointment year and notmatch year. IntroductionThis report summarizes all fellowship matches conducted by the NRMP's Specialties Matching Service (SMS). Specifically, data are provided for matches conducted in 2009 (and early 2010) for appointments beginning in July 2010. In addition, trend data over the past five years show the numbers of programs and positions as well as match rates by specialty. Lastly, the numbers of positions offered and filled for each participating program is presented at the end of this report.
The 2010 version of this report includes two new specialties (Pediatric Nephrology and Pediatric Pulmonology) and two new matches (Pediatric Specialties Fall Match and Pediatric Specialties Spring Match). In addition, the specialty-specific trend charts have been expanded to include additional measures and to allow better comparisons of measures with similar scale ranges.
The 2010 appointment year was the largest in the history of the SMS. The matches included:
2,608 programs within 38 specialties offering 6,002 fellowship positions
Of those positions, 5,348 (89.1%) were filled Of the 2,608 participating programs, 2,132 (81.7%)
filled all their positions 7,627 applicants participated in at least one of the
fellowship matches and 5,348 (70.1%) obtained a match

Table 1 provides a summary of the SMS fellowship matches for appointment year 2010.
Using Allergy and Immunology as an example:
The first two columns show that 122 U.S. allopathic medical school graduates and 173 total applicants ranked at least one Allergy and Immunology program.
The next two columns show that a total of 122 positions were offered in this specialty by 72 programs.
The next two columns show that 88 of the 122 positions offered in Allergy and Immunology were filled by U.S. allopathic graduates and 115 were filled by all applicants.
The percent filled (number filled/positions offered) can be found in the next two columns. Of the fellowship positions offered in Allergy and Immunology, 72.1 percent were filled by U.S. allopathic graduates and, overall, 94.3 percent were filled.
The ranked positions columns show that collectively U.S allopathic graduates ranked those Allergy and Immunology programs 659 times and the total number of ranks for all applicants was 998.
The last column indicates that 7 Allergy and Immunology programs had at least 1 unfilled position after the Match.
Of the 38 participating specialties, 22 filled more than 90 percent of the positions offered and 7 filled less than 75 percent of the positions offered. Five specialties filled 75 percent or more of the positions offered with graduates of U.S. allopathic medical schools and 14 specialties filled 50 percent or less of their positions offered with U.S. allopathic graduates.

specialties (or programs within a specialty) are not accredited by the ACGME. In these cases the fellowship programs are grouped under the RRC with the preponderance of programs (for multiple RRCs) or the RRC that would have responsibility for the programs if accredited.

Table 2 shows the number and percent of matches in each specialty by applicant type. Of the specialties offering at least 30 positions:
Pediatric Surgery had the highest proportion filled with U.S. allopathic graduates (91.2%) and Nephrology had the lowest (34.7%). The specialties of Pediatric Emergency Medicine, Gynecologic Oncology, Reproductive Endocrinology and Pediatric Surgery filled 80 percent or more of their positions with graduates of U.S. allopathic medical schools.
A larger than average percentage of osteopathic graduates matched to the specialties of Child and Adolescent
Psychiatry, Pediatric Gastroenterology, Pediatric Pulmonology and Primary Care Sports Medicine.
A larger than average percentage of Canadian graduates matched to the specialties of Pediatric Surgery and Female Pelvic Medicine and Reconstructive Surgery
A larger than average percentage of U.S. international medical school graduates matched to the specialties of
Pediatric Pulmonology, Surgical Critical Care, Clinical Oncology, Nephrology and Rheumatology.
A larger than average percentage of Non-U.S. international medical school graduates matched to the specialties of Clinical Oncology, Pediatric Nephrology, Abdominal Transplant Surgery and Nephrology.

Table 3 The ratios of fellowship positions to active applicants from 2006 through 2010 are provided in Table 3 and shown graphically in Figure 3. Active applicants are defined as those who submitted certified rank order lists. The red line in Figure 3 indicates the point at which there are equal numbers of positions and applicants.
The ratio of fellowship positions per active U.S. allopathic graduate remained fairly constant between 2006 and 2008 before increasing to 1.54 positions per applicant in 2009, and 1.55 positions in 2010. Across all five years, there were more fellowship positions offered than there were U.S. allopathic graduate applicants.
The ratio of fellowship positions to total active applicants decreased slightly between 2006 and 2008, from 0.84 positions per applicant in 2006 to 0.75 positions per applicant in 2008, but increased slightly to 0.78 in 2009, and 0.79 in 2010.
Taken together, these data suggest that the increase in the number of positions offered in the SMS matches between 2006 and 2010 is consistent with the increase in the number of applicants when compared to the previous four years.

Table 4 provides a summary of the participants in the SMS fellowship matches between 2006 and 2010 by applicant type. The total number of registrants for the 2010 appointment year (8,311) increased 2.8% percent from the 2009 figure of 8,086, and the number of active applicants grew by 283 (a 3.9% increase). An active applicant is one who submits a certified rank order list of programs. Those increases can be attributed to both the addition of new specialties (Endocrinology and Nephrology) and the increase in the number of applicants for some of the specialties participating over the entire period.
Applicants are grouped based on citizenship and medical school of graduation. For example, U.S. allopathic graduates are prior graduates of U.S. allopathic medical schools. U.S. international graduates are defined as U.S. citizens who are graduates of international medical schools, and non-U.S. international graduates are non-U.S. citizen graduates of international medical schools.
The increase between 2006 and 2010 in active applicants has not been uniform across applicant types. For example, active applicants who are non-U.S. international graduates rose by 101.3 percent while graduates of U.S. allopathic medical schools increased by only 40 percent. Active applicant groups that increased faster than the overall rate of 59.1 percent are identified in Table 4 with green arrows.
The trends in the numbers of active applicants and their match rates can be seen graphically in Figures 4.

Table 5Table 5 displays fellowship match results by specialty and applicant choice. Applicant choice is determined by the placement of programs on applicants' rank order lists. For single specialty matches (top half of table), the data presentation and interpretation are straightforward. For multiple specialty matches (bottom half of table) they are slightly more complicated. Single Specialty Matches Using Allergy and Immunology as an example, there were at total of 173 applicants, of whom 115 matched. Of the 173 applicants, 37.6 percent matched to the program that was listed first on their rank order list, 14.5 percent matched to their second choice, 8.1 percent to their 3rd choice, and 6.4 percent to a choice further down on their list. Fifty eightapplicants in the Allergy and Immunology Fellowship Match did not match, representing 33.5 percent of the total number of applicants. Combined Specialties Matches In order to count applicants only once for the combined specialties matches, each applicant is included in the counts for the specialty ranked first on the rank order lists (preferred specialty). Because applicants can match to other specialties within the same match, "Matched in Another Specialty" data also are provided. For example, Cardiovascular Disease participates with ten other specialties in the Medical Specialties Matching Program (MSMP). Of the 1,184 applicants who ranked Cardiovascular Disease, 1,174 ranked it first. Overall, 713 applicants matched to cardiovascular disease, and 712 of the applicants who preferred the specialty matched. Of the 1,184 applicants who ranked Cardiovascular Disease, 35.6 percent matched to the program that was listed first on the rank order list, 10.0 percent matched to the second choice, 5.5 percent to the 3rd choice, 9.1 percent to a choice further down on their list, and 0.8 percent matched to another within the MSMP. Four hundred and sixty two applicants who ranked Cardiovascular Disease programs failed to match, representing 39.0 percent of the total number of applicants. *Specialties are grouped by Residency Review Committees (RRCs). Some specialties are under the auspices of multiple RRCs and some
