CERTIFICATION REGARDING CASH DISBURSEMENTS AND COMPLIANCE AND PAYMENT OF POSTPETITION TAXES AND
INSURANCE I, David F. Smith, III, Chief Financial Officer of Trident Holding Company, LLC et al. (the “Debtors”)(1), hereby certify as follows:
1. Included in MOR-1a is a schedule of the Debtors’ bank accounts, by account number, along with the closing balances. Account statements for these accounts have been provided to the United States Trustee.
2. To the best of my knowledge and belief, all disbursements made during the reporting
period in satisfaction of prepetition liabilities were made under authority of an order entered by the Bankruptcy Court.
3. To the best of my knowledge and belief, the Debtors have filed all necessary tax returns and are current on all postpetition taxes, or are in the process of remediating any immaterial late filings or payments.
4. To the best of my knowledge and belief, the Debtors are current on all payments of

GLOBAL NOTES AND STATEMENT OF LIMITATIONS, METHODOLOGY, AND DISCLAIMERS REGARDING THE DEBTORS’ MONTHLY OPERATING
REPORT
On February 10, 2019 (the “Petition Date”), the Debtors each commenced voluntary cases under chapter 11 of title 11 of the United States Code (the “Bankruptcy Code”) in the United States Bankruptcy Court for the Southern District of New York (the “Bankruptcy Court”).
The Debtors continue to operate their businesses and manage their properties as Debtors and Debtors-in-Possession pursuant to sections 1107(a) and 1108 of the Bankruptcy Code. The Debtors’ cases have been consolidated for procedural purposes only and are being jointly administered under case number 19-10384 (SHL).
General Methodology. The Debtors are filing their monthly operating report (“MOR”) solely for purposes of complying with the monthly operating requirements applicable in the Debtors’ chapter 11 cases. The financial information contained herein is unaudited. The MOR should not be relied upon by any persons for information relating to current or future financial conditions, events, or performance of any of the Debtors, as the results of operations contained herein are not necessarily indicative of results which may be expected from any other period or for the full year, and may not necessarily reflect the results of operations, financial position, and schedule of receipts and disbursements in the future. There can be no assurance that the information contained herein is complete, and the MOR may be subject to revision. The following notes, statements, and limitations should be referred to, and referenced in connection with, any review of the MOR.
Basis of Presentation. In preparing the MOR, the Debtors relied on financial data derived from their books and records available at the time of preparation. Nevertheless, in preparing the MOR, the Debtors made reasonable efforts to supplement the information set forth in their books and records with additional information concerning transactions that may not have been identified therein. Subsequent information or discovery may result in material changes to the MOR and errors or omissions may exist. Notwithstanding any such discovery, new information, or errors or omissions, the Debtors do not undertake any obligation or commitment to update the MOR.
The MOR has not been prepared in accordance with accounting principles generally accepted in the United States of America (“U.S. GAAP”) and does not include all of the information and footnotes required by U.S. GAAP. Although the MOR may, at times, incorporate information prepared in accordance with U.S. GAAP, the MOR neither purports to represent nor reconcile to financial statements otherwise prepared and/or distributed by the Debtors in accordance with U.S. GAAP or otherwise. Certain revenues and expenses resulting from the reorganization may be included in the Debtors’ operating activity, versus reported separately as Reorganization Items. Therefore, there can be no assurance that the financial information presented herein is complete, and readers are strongly cautioned not to place undue reliance on the MOR.

The results of operations contained herein are not necessarily indicative of results that may be expected from any other period or for the full year and may not necessarily reflect the consolidated results of operations and financial position of the Debtors in the future. The Debtors record and report results of operations and financial transactions at the Accounting Entity (“Accounting Entity”) level, which does not always correspond to, or reflect activity at, an individual Debtor. In certain instances, the Accounting Entity where transactions are recorded represents the financial activities of multiple Debtors. Thus, certain MOR Schedules are presented by Accounting Entity versus by individual Debtors. For example, the books and records maintained at the Accounting Entity “Diagnostic Laboratories” or “DL” (which is the accounting entity for the Debtor Kan-Di-Ki LLC) represents the activity of four separate Debtors: Community Mobile Diagnostics, LLC; Community Mobile Ultrasound, LLC; MDX-MDL Holdings, LLC; and Kan-Di-Ki LLC. The activity for these four Debtors is reflected together under the “DL” Accounting Entity field. Likewise, the financial information reported at the Accounting Entity “Schryver” or “SM2” (which is the accounting entity for the Debtor Schryver Medical Sales and Marketing, LLC) represents the activity of three Debtors: Main Street Clinical Laboratory, Inc.; MetroStat Clinical Laboratory – Austin, Inc.; and Schryver Medical Sales and Marketing, LLC. Seven of the Debtors (American Diagnostics Services, Inc., JLMD Manager, LLC, MX USA, LLC, Symphony Diagnostic Services No. 1, LLC, TridentUSA Mobile Clinical Services, LLC, TridentUSA Mobile Infusion Services, LLC, and U.S. Lab & Radiology, Inc.) maintain separate books and records. The remaining nine Debtors (FC Pioneer Holding Company, LLC, Trident Holding Company, LLC, New Trident Holdcorp, Inc., MX Holdings, LLC, Diagnostic Labs Holdings, LLC, Rely Radiology Holdings, LLC, Trident Clinical Services Holdings, Inc., Trident Clinical Services Holdings, LLC, and TridentUSA Foot Care Services LLC) have no business activity and therefore do not maintain books and records. Combined Entity Accounts Payable and Disbursement Systems. Cash is received and disbursed by the Debtors as described in the FINAL ORDER SIGNED ON 3/8/2019, (I) AUTHORIZING CONTINUED USE OF EXISTING CASH MANAGEMENT SYSTEM, BANK ACCOUNTS, AND BUSINESS FORMS AND PAYMENT OF RELATED PREPETITION OBLIGATIONS; (II) MODIFYING CERTAIN DEPOSIT REQUIREMENTS; AND (III) AUTHORIZING CONTINUANCE OF INTERCOMPANY TRANSACTIONS AND HONORING CERTAIN RELATED PREPETITION OBLIGATIONS (Docket # 164) and is consistent with the Debtors’ historical cash management practices. Debtor-in-Possession Financing. On March 8, 2019, the Debtors received final authorization from the Bankruptcy Court to access $50 million of Debtor-in-Possession financing pursuant to the FINAL ORDER SIGNED ON 3/8/2019, (I) AUTHORIZING THE DEBTORS (A) TO OBTAIN POST-PETITION FINANCING, AND (B) TO UTILIZE CASH COLLATERAL; AND (II) GRANTING ADEQUATE PROTECTION TO PRE-PETITION SECURED PARTIES (Docket #166) (the “DIP Order”). Please see the DIP Order for additional detail.

Payment of Prepetition Claims Pursuant to First Day Orders. The Bankruptcy Court entered orders (the “First Day Orders”) authorizing, but not directing, the Debtors to, among other things, pay certain prepetition (a) insurance obligations, (b) employee wages, salaries, and related items, (c) obligations related to the provision of utility services, and (d) taxes and assessments. If any payments were made on account of such claims following the commencement of these chapter 11 cases pursuant to the authority granted to the Debtors by the Bankruptcy Court under the First Day Orders, such payments have been included in the MOR unless otherwise noted. Liabilities Subject to Compromise. The Debtors’ balance sheet distinguishes the presentation of liabilities between liabilities subject to compromise and liabilities not subject to compromise. Liabilities subject to compromise exclude certain contingent and unliquidated liabilities which are either not probable or not able to be reasonably estimated as of the reporting period, or both. The Debtors’ balance sheet reflects estimated values which are subject to further revision, reconciliation, and ultimate agreement with the final claims register. A plan of reorganization could materially change the amounts reported in the enclosed financial statements. Reservation of Rights. The Debtors reserve all rights to amend or supplement the MOR in all respects, as may be necessary or appropriate, but shall be under no obligation to do so. Nothing contained in the MOR shall constitute a waiver of any of the Debtors’ rights or an admission with respect to their chapter 11 cases. Reorganization Items. Expenses and income directly associated with the Debtors’ chapter 11 cases are reported separately in the income statement as reorganization items. Reorganization items primarily include expenses related to legal advisory, professional consulting and advisory services and United States Trustee fees.

“The requirements of Bankruptcy Code section 345(b) are waived with respect to Cathay Bank. The balance of Kan-Di-Ki, LLC’s operations account at Cathay Bank (account number
ending in 6532) shall not exceed $250,000 plus the amount of all outstanding checks issued under such account plus the amount of checks anticipated to be issued under such account
within the following two business days”. As of 5/31/2019, the account balance was $1,636,814 which is less than the “shall not exceed” amount of $1,843,198 ($250,000; plus
outstanding checks, $1,578,983 plus the estimated amount of checks issued in the following 2 days, $14,215).
