California Healthy Kids Survey Procedure
The CHKS is administered to all 5th, 7th, 9th, and 11th grade students
Prior to 2005/06, “positive parental permission” was required for participation; some districts now use “passive consent” for secondary students.
Only districts receiving Title IV or TUPE funding are required to participate in the CHKS
