It’s natural that your circumstances may change or that you may decide you want to change the way in which the service you receive is delivered. If this is the case then please tell me/us and we will happily work with you to change those things we can. If we/I can’t make the appropriate changes we/I will look at ways that you can achieve the changes you want and how I/we can support you to live the life you want.

What If I change my mind?

I/We are happy to review the support and care we/I are providing to you at any time and make the changes we can. In most cases, it is likely that there will be no change to any contract arrangements we have made, but it is possible the changes requested may mean I/we have to look at what you are being charged for the service provided.In these cases, you will be fully involved and made aware of any such change before we officially Concerns about the care you receiveI/we believe that you have the right to guide the care you receive and how this is provided to you. As such, if you feel that something is not right, or that you want to change something about the service we provide, please talk to us and we will do our best to address any concerns you may have.

ComplaintsIf you feel that we haven't dealt with your comments fully, or you have more serious concerns, then please raise the matter with your local Social Services department. I/We have included their contact details on the rear of this leaflet.

How will this affect the care and support I receive?

Printing To print your leaflet, make sure you have selected ‘Print on Both Sides’ with ‘Flip pages on short edge’, otherwise the inside page will print upside down!

You can get pictures through Clip Art in Word, or through online companies like Bigstock or 123rf who sell stock photos.

You may well want to change the photographs in the leaflet. To do this in Word 2007 onwards simply right click on a photograph, choose change picture and then pick what you want. You will need to delete and reinsert pictures in earlier versions of Word, or download the Compatibility Pack free here

Changing the Photographs

Word 2007 onward, in earlier versions of Word click format menu then pick

Change the whole design Microsoft word gives you the chance to change the colours and feel of the leaflet simply by going to Design and changing the Theme.

Have a play with the themes and see which feels right for your leaflet.

Once you’ve completed the changes you’ll want to delete this page so that you’re just left with your leaflet. Click on the outside of the big box so that little squares appear at the corners and then press delete. Then click on the page again and click the backspace. The whole page should disappear and you’ll be left with just the two page document.
