CORRIGENDUM AND ADDENDUM TO DIVISION MEMORANDUM NO.34, S.2018 ENTITLEDANNUAL CHECKING OF FORMS IN PUBLIC ELEMENTARY AND SECONDARY LEVELS

To: OIC - Assistant Schools Division SuperintendentsCID and SGOD ChiefsEducation Program SupervisorsPublic Schools District SupervisorsConcerned SGOD PersonnelPublic/Private Elementary and Secondary School Heads/OICsVocationallTrade School Administrators

1. In compliance with the DepEd Order No.11, s.2018 entitled Guidelines on the Preparation andChecking of School Forms, this is to announce that the scheduled checking of forms and thecomposition of the Division Checking Committee stipulated in Enclosure No.1 to DivisionMemorandum No.34, s.2018 are hereby changed.

2. Enclosure No.1 contains the Final Schedule for Checking of Forms and the New Set of DivisionChecking Committee in the elementary and secondary levels respectively.

3. It is required that the School Checking Committee conducts pre-checking activity and ensures theaccuracy/correctness and completeness of all the school forms. The school head shall affix his/herinitial below the name of the Chairman of Division Checking Committee.

4. Both elementary and secondary schools that were not selected in the sampling need not to reportin the checking venue. Instead, the PSDSes and Secondary School Heads should coordinate withthe Division School Forms Coordinator, Dr. Jerry D. Cruz (0975-0511-184/0923-3546-470).

5. Immediate and wide dissemination of this Memorandum is desired.
