The Special Needs Accommodation Process (SNAP) has changed its name to the Multidisciplinary Inclusion Action Team (MIAT). The MIAT team is established to determine what accommodations need to be made for children with special needs to be successful in the CYS Services programs.

The members of the MIAT team include the parents and/or guardian, ACS Exceptional Family Member Program (EFMP) representative, Army Public Health Nurse (APHN), and Child Youth and School Services designee. All other personnel are augmented as needed. By providing the MIAT team with a brief history of your child’s current condition, concerns, and actions taken, you are a vital part of the MIAT team and your participation is essential to your children’s overall success in CYS Services programs. You may also be asked to provide medical documentation, statements of illnesses, allergies, and copies of the Individualized Education Plan (IEP).

The Multidisciplinary Inclusion Action Team (MIAT)

How does the MIAT process work? You will complete a CYS Service DA 7625-1 Health Screening, and submit it to CYS Parent Central Services, who will forward the completed forms to the APHN for review and recommendations. In some instances, a Medical Action Plan (MAP) and/or a Special Diet Statement to be completed by your child’s physician will be provided to you. If the APHN determines a full MIAT is necessary, then a MIAT meeting must be scheduled. ACS EFMP will contact you and the appropriate MIAT team members to schedule a MIAT meeting.

At the meeting your child’s needs are discussed, along with recommendations of a developmentally appropriate environment; we will consider any health, developmental, physical, social, emotional, learning, and behavioral issues that affect your child. Also addressed at the MIAT meeting will be the expectations of the service to be provided by the CYS Service staff. The goal is that your child has the very best experience at CYS Services and that staff are trained to provide the care that your child needs.

Some things to keep in mind regarding a MIAT meeting are:

• Please arrive ten minutes early

• Please be ready to discuss your child’s medical history, as this will assist the team in placing your child within CYS Services

• Please bring appropriate documentation such as copies of your child’s Individualized Education Plan (IEP), Behavioral Support Plan (BSP) and/ or Individual Family Support Plan (IFSP)

• Expect 3 MIAT members to be present

• MIAT meetings generally last 30-45 minutes

• Please remember, it is the responsibility of each parent or guardian to follow up and provide requested information to the MIAT team

ARMY EMERGENCY RELIEF CAMPAIGN KICK OFFFriday, March 4, 2016 from 4:15 p.m. - 5:00 p.m. at Schofield Barracks’ Nehelani Banquet and Conference Center.

The United States Army Hawaii (USARHAW) Community is cordially invited to attend the Army Emergency Relief (AER) Annual Fundraising Campaign Kick-Off hosted by United States Army Garrison Hawaii (USAG-HI).

AER was established in 1942 by Soldiers for Soldiers in response to the financial demands of a World War; that’s why it’s referred to as “Soldiers Helping Soldiers”. Today, financial demands continue to press our Soldiers, mostly through no fault of their own. AER assists with such things as emergency travel costs

when the unit can not, vehicle repairs when the repair exceeds the Soldiers savings as well as, rent, initial deposit and basic living necessities.

In 2015, the office collected $301,000.00 in donations but distributed over $2.1 million dollars in loans and grants to over 1200 Soldiers and Family members.

The secondary mission of AER is Scholarships for Spouses and children of active duty and retirees. The scholarship is “needs-based” and last year the amount ranged from $500 - $3,300. The Application period started January 4 and runs to May 1 (when all supporting documentation is due to AERHQ). The entire

process is online – no snail mail required! Last year AER awarded 4,245 scholarships, totaling more than $9 million to Spouses and children of Soldiers.

Outdoor Recreation offers a wide variety of great outdoor adventures that you are sure to love, with more and more being added every month!

Adventure Biking Find the great beach locations along the scenic North Shore bike path, or cruise through Pearl City down to the historical Pearl Harbor. This is a mild biking activity for all ages! We will provide you with all the transportation (up to 12 people), guide and equipment, all you need to bring is some water, snacks, and a great attitude. This is a beginner program but participants must be able to ride a bike. Mar. 5 • 8:30 a.m. – 12:30 p.m. $30 per person

Stand Up Paddle-boarding and Kayak Adventure Come enjoy a SUP or Kayak paddle with Outdoor Recreation as we cruise the Anahulu River on the North Shore of Oahu and other various beaches around the island! These locations are great for the whole Family and usually have many opportunities for turtle spotting. Don’t know how to SUP or Kayak, we can teach you! Our outdoor facility contains 196 covered dog kennels, 19 cat kennels, grassy lawns, large exercise and play areas.
