penalty of $1,000, because it failed to timely complete a peer review for the renewal period January 1,
2012, to December 31, 2013, that was required as a condition of renewal for the January 1, 2014 to

Philadelphia County Jarett T. Devaney, license no. CA057089, of Philadelphia, Philadelphia County, was suspended for failing

PA Architects Licensure Board Allegheny County Dunn & Associates Inc. a/k/a Alan Dunn Architect, Inc., license no. AX004095, of Pittsburgh, Allegheny
County, paid a civil penalty of $5000 and costs of investigation of $475.55, because they engaged in or
offered to engage in the practice of architecture and implied it was competent to engage in the practice of
architecture through the use of titles, signs, cards and/or devices while its registration was lapsed, expired

PA Board of Auctioneer Examiners Philadelphia County Associated Auctioneers, license no. AH000162L, of Philadelphia, Philadelphia County, paid a civil
penalty of two hundred and fifty dollars ($250.00), for failing to make records open at all reasonable
times for inspection by the Board or by any person who is authorized by the Board to make such

Schuylkill County Karen E. Arner-Moyer, license number AU005659, of New Ringgold, Schuylkill County, was suspended
for a period of 6 months and paid a civil penalty of $10,000 and costs of investigation, for failing to:
obtain a written contract for consignment of auction items, maintain a record of sales related to items
consigned, open records for inspection, provide records to investigator as requested, provide a written
receipt of all items sold or unaccounted for related to consigned items, immediately deposit monies into
escrow account, pay individual or account for all items consigned at auction, pay over monies received
from the sale of all items consigned within a reasonable time and meet the standards of honesty and

period of one (1) year; ordered to pay a civil penalty in the amount of $1,000 and assessment fee of

record with the Board, because she violated the Board regulation by practicing cosmetology without a

ordered to pay a civil penalty of $1,000.00 and costs of investigation of $741.00 because he employed an

Jefferson County Eric S. Manners, license no. LCO003953, of Big Run, Jefferson County, was indefinitely suspended for
at least three years because he is unable to practice with reasonable skill and safety due to impairment,
and was stayed in favor of probation while he participates in a structured monitoring and treatment

unprofessional conduct by failing to conform to the standards of acceptable and prevailing dental practice.

order of corrective action specified in the Consent Agreement for no less than 36 months, in accordance to
the Board’s terms and conditions, because he engaged in unprofessional conduct by assigning certain
duties related to infection control in violation of the Dental Law (relating to assignment of duties).

the Board’s terms and conditions, because he engaged in unprofessional conduct by assigning certain
duties related to infection control in violation of the Dental Law (relating to assignment of duties).

Lycoming County Robert Scott Van Ess, license no. DS025515L, of Hughesville, Lycoming County, had his license placed
on probation for 18 months and was ordered to pay a civil penalty of $4000 and costs of $2,749, because
in June 2016, his practice did not pass an inspection for infection control under the Infection Control
Guidelines for Dental Settings published by the Centers for Disease Control. (11/17/17)
Out of State Michael Allen Moore, PA license no. DS023247L, of Detroit, Michigan, was reprimanded and issued a civil penalty of $1,000 based on disciplinary action taken by another state. (04/11/17)
Runjan Seth, PA license no. DS036386 and permit no. DN002158, of Pennington, New Jersey, had a
public reprimand placed on his permanent disciplinary Board record and was ordered to pay a civil
penalty of $1,000, because he had a license to practice dentistry or dental hygiene or certificate for
expanded function dental assisting revoked, suspended or had other disciplinary action imposed or
consented to by the proper licensing authority of another state, territory or country or had an application

Chester County Thomas Drew, license and registration no. PG003169G, of West Chester, Chester County, paid a civil
penalty of $1,300 and costs of investigation in the amount of $656.80 because he practiced the profession

Lehigh County Matthew Paul McTish, license and registration no. PE035423E, of Orefield, Lehigh County, was
indefinitely and actively suspended for a period of at least 3 years, retroactive to April 28, 2016, and paid
a civil penalty of $10,000, because he was convicted, as defined by the Act and the Board's regulations, of
conspiracy to commit bribery offenses, a felony, which involved conduct related to the practice of

Out of State Charles Michael Settlecowski, PA license, and registration no. SU075159, of Clarksburg, New Jersey,
was paid a civil penalty of $2500 because he engaged in the practice of land surveying and/or practiced as
a land surveyor while his license was lapsed, expired and/or not current with the Board. (11/08/17)
David J. Klimas, PA license, and registration no. PE076340, of Palos Hills, Illinois, had a public
reprimand placed on his permanent disciplinary record with the Board and paid a civil penalty of $1,000
because he was guilty of misconduct as defined by the Board, in that he had a license to practice
engineering suspended or revoked or had other disciplinary action taken for conduct relating to the
practice of engineering by the proper licensing authority of another state. (11/08/17)
PA Board of Funeral Directors Allegheny County Nicholas R. Bellich, license no. FD138738, of Wilmerding, Allegheny County, paid a civil penalty of two
hundred dollars ($200.00) and ordered to attend and successfully complete the missing hour of continuing

the minimum six (6) total hours of continuing education required for the renewal of his license, and he

Clarion County J. Robert Varner, license no. FD013007L, of Sligo, Clarion County, was ordered to pay a civil penalty of
six hundred dollars ($600); and shall attend and successfully complete the missing hours of continuing
education necessary to meet the requirements for the February 2, 2016 - February 1, 2018, biennial
reporting period, because he was three (3) hours short of the minimum six (6) total hours of continuing
education required for the renewal of his license; and obtained or attempted to obtain a license by the

Delaware County Rigby Funeral Home, license no. FO014901, of Media, Delaware County, was ordered to pay a civil
penalty of two thousand dollars ($2,000.00), and an assessment for the costs of investigation in the
amount of five hundred thirty dollars and thirty-seven cents ($530.37), because it practiced on a lapsed

Fayette County Enoch James Eley, Jr, license no. FD010898L, of Fayette City, Fayette County, was ordered to pay a civil
penalty of six hundred dollars ($600); and shall attend and successfully complete the missing hours of
continuing education to meet the requirements for the February 2, 2016 - February 1, 2018 biennial
reporting period, because he failed to comply with the continuing education requirements to renew his
funeral director license in that he was three (3) hours short of the minimum six (6) total hours of
continuing education required for the renewal of his license; and obtained or attempted to obtain a license

Philadelphia County Oglatha Jacks, license no. FD012648L, of Philadelphia, Philadelphia County, was ordered to pay a civil
penalty of two hundred dollars ($200) and shall attend and successfully complete the missing hours of
continuing education necessary to meet the requirements for the February 2, 2016 - February 1, 2018
biennial reporting period, because she failed to comply with the continuing education requirements to
renew her funeral director license in that she was one (1) hour short of the minimum six (6) total hours of
continuing education required for the renewal of her license and she obtained or attempted to obtain a

PA Board of Medicine Allegheny County David H. Newman, license no. MD071742L, of Pittsburgh, Allegheny County, was revoked based on a

Armstrong County Arnold Berkowitz, license no. MD024466E, of Parker, Armstrong County, was automatically suspended

suspended for a period of at least two (2) years, such suspension to be immediately stayed in favor of

license or other authorization to practice the profession disciplined by the proper licensing authority of

practice as a medical physician and surgeon temporarily suspended, pending a hearing, on the grounds
that his continued practice within the Commonwealth may be an immediate and clear danger to the public

Lackawanna County Joseph Emmanuel Lena, license no. MD066386L, of Dickson City, Lackawanna County, was authorized
by the Board to inactivate his license and prior to any reactivation of said license, he must obtain an
evaluation that he is fit to practice medicine with reasonable skill and safety to patients. In addition, upon
reactivation, said license shall be indefinitely suspended for no less than three years, such suspension to
be immediately stayed in favor of no less than three years of probation, because he cannot practice as a
medical physician and surgeon with reasonable skill and safety to patients by reason of illness, addiction
to hallucinogenic, narcotic or other drugs or alcohol, or mental incompetence. (10/31/17)
Philadelphia County Heidi Vidal, license no. MD061470L, of Philadelphia, Philadelphia County, was indefinitely suspended
for no less than three years with the suspension immediately stayed in favor of indefinite probation for no
less than three years based upon being convicted of several misdemeanors relating to the profession and

Martin Douglas Weaver, M.D., license no. MD042968L, of the City of Philadelphia, Philadelphia County
had his license to practice medicine temporarily suspended, pending a hearing, on the grounds that his
continued practice within the Commonwealth may be an immediate and clear danger to the public health

Alexis Patricia Patterson, license no. YM012073, of Philadelphia, Philadelphia County, paid a five-
hundred-dollar ($500) civil penalty and had a public reprimand placed on her permanent disciplinary
record with the Board because she engaged in the practice of respiratory therapy while the license to

Schuylkill County Virginia Maria Vesay, license no. MA002510L, of Minersville, Schuylkill County, permanently and
voluntarily surrendered her license, because she is guilty of immoral or unprofessional conduct in that she
was convicted of a felony offense in a court of this Commonwealth in violation of the Board’s regulations
