PA Board of Barber Examiners Cumberland County You'topia Beauty Spa, license no. BO434507, of Camp Hill, Cumberland County, was levied a civil

penalty of one thousand dollars, because they employed Julie Masters, a person unlicensed to practice

massage therapy, as a massage therapist; aided and abetted Julie Masters, a person unlicensed to practice

as a massage therapist, in the practice of the profession of massage therapy by providing employment and

a location at which to practice the profession; used rooms for massage therapy, a purpose other than

barbering and a purpose that has not been approved by the Board. (12/17/18)

PA Board of Certified Real Estate Appraisers Mifflin County Robert L. Baer, certificate nos. GA001070L and RL000014L, of Lewistown, Mifflin County,

permanently and voluntarily surrendered his certificates to practice as a certified general real estate

appraiser and certified residential real estate appraiser in the Commonwealth of Pennsylvania, because he

violated the law by failing to complete an appraisal assignment within the requirements of the USPAP; by

failing or refusing to exercise reasonable diligence in developing an appraisal, preparing an appraisal

report, or communicating an appraisal; and by negligently or incompetently developing an appraisal,

preparing an appraisal report, or communicating an appraisal. (12/13/18)

Washington County Ryan R Neve, certificate no. RL139611, of McDonald, Washington County, was ordered to pay a civil

penalty of $750 and a public reprimand was placed on his permanent record with the Board because he

failed to comply with continuing education requirements to renew his certificate. (12/13/18)

PA Board of Chiropractic

Lackawanna County John Williams Stevens, D.C., license nos. DC005225L and AJ005225L, of Chinchilla, Lackawanna

County, had his license to practice as a Chiropractor and license to practice Adjunctive procedures

temporarily suspended, pending a hearing, on the grounds that his continued practice within the

Commonwealth may be an immediate and clear danger to the public health and safety. (12/03/18)

licenses to practice Chiropractic temporarily suspended, pending a hearing, on the grounds that his

continued practice within the Commonwealth may be an immediate and clear danger to the public health

PA Board of Crane Operators Allegheny County Franco Associates, of Pittsburgh, Allegheny County, was levied a civil penalty of $1,000 because they

employed an unlicensed individual to operate a crane. (11/28/18)

PA Board of Dentistry Bucks County Gregory John Keiser, D.M.D., license no. DS026629L, of Churchville, Bucks County, had his license to

practice as a dentist temporarily suspended, pending a hearing, on the grounds that his continued practice

within the Commonwealth may be an immediate and clear danger to the public health and safety.

PA Board of Medicine Allegheny County Madhu Aggarwal, license no. MD027043E, of Coraopolis, Allegheny County, was ordered to voluntarily

surrender his license, because he was convicted of a felony or a misdemeanor relating to a health

profession or received probation without verdict, disposition in lieu of trial or an Accelerated

Rehabilitative Disposition in the disposition of felony charges, in the courts of this Commonwealth, a

Berks County Jillian Rae Palm, license no. BH002456, of Reading, Berks County, was indefinitely suspended, because

she was convicted of a felony or a misdemeanor relating to a health profession or received probation

without verdict, disposition in lieu of trial or an Accelerated Rehabilitative Disposition in the disposition

of felony charges, in the courts of this Commonwealth, a Federal court or a court of any other state,

Crawford County Cynthia Ann Allen, license no. YM002858L, of Saegertown, Crawford County, permanently and

voluntarily surrendered her license to practice as a respiratory therapist, because she is unable to practice

the profession with reasonable skill and safety to patients by reason of addiction to alcohol. (12/11/18)

Monroe County Bhargav Chowdary Paleti, license no. MD455210, of Tobyhanna, Monroe County, was ordered to pay a

civil penalty of $5,000 and a public reprimand was placed on his permanent disciplinary record with the

Board, because he engaged in immoral conduct by committing an act involving moral turpitude which

directly affects the health, welfare or safety of a citizen of this Commonwealth. (12/11/18)

Northampton County Kenneth Paul Sun, license no. MD056132L, of Easton, Northampton County, was ordered to voluntarily

surrender his license because he had a license or other authorization to practice the profession disciplined

by the proper licensing authority of another state. (12/11/18)

Philadelphia County Denise Gena Avender, license no. RT003078, of Philadelphia, Philadelphia County, was assessed a $750

civil penalty and a public reprimand was placed on her permanent disciplinary record with the Board

because she continued to practice while her license or certificate was expired, not registered, suspended or

Mark A Rupert, license no. MA003224L, of Philadelphia, Philadelphia County, was suspended for no less

than three (3) years, with such suspension immediately stayed in favor of no less than three (3) years of

probation, because he cannot practice as a medical physician assistant with reasonable skill and safety to

Westmoreland County Mamdouh F El-Attrache, license no. MD027815E, of Mount Pleasant, Westmoreland County, was

ordered to permanently voluntarily surrender his license, because he had a license or other authorization

to practice the profession suspended, revoked, refused or otherwise disciplined by the proper licensing

authority of another state, territory, possession or country, or a branch of the Federal Government is guilty

of unprofessional conduct by departing from and/or failing to conform to the quality standard of the

profession, and violated the minimum standards for prescribing, administering and dispensing. (12/11/18)

Nabil Jabbour, license no. MD028428E, of Greensburg, Westmoreland County, was indefinitely

suspended with such suspension immediately stayed in favor of a period of indefinite probation, because

he is guilty of unprofessional conduct by failing to conform to the quality standard of the profession.

Martin D Fried, PA license no. MD434168, of Asbury Park, New Jersey, was indefinitely suspended until

Secretary of the Commonwealth – Notaries Public

Lehigh County Nancy G. Moreno, notary commission no. 1004768, of Allentown, Lehigh County, agreed to the

suspension of her notary commission for three months, such suspension immediately stayed in favor of

probation, to successfully complete a six-hour notary practice and procedure course, and to pay a civil

penalty in the amount of $500 and probation assessment fees in the amount of $150 because she

submitted an application that contained a material misstatement or omission of fact by answering “no” to

ever having been convicted or entered a plea of guilty or nolo contendere to a felony or lesser offense;

failed to maintain an accurate and chronological notary register; failed to be familiar with the duties and

responsibilities of a notary public; failed to be of good character, integrity and ability. (12/04/18)

Philadelphia County Tobias L. Biddle, notary commission no. 1011576, of Philadelphia, Philadelphia County, agreed to the

suspension of his notary commission for six months, such suspension immediately stayed in favor of

probation, to successfully complete a six-hour notary practice and procedure course, and to pay a civil

penalty in the amount of $250 and probation assessment fees in the amount of $300 because he failed to

be familiar with the duties and responsibilities of a notary public by failing to include the essential

notarial language on a document, indicating the incorrect date of notarization on a document, and failing

to maintain a notary register; failed to maintain a notary register, and failed to be of good character,

integrity and ability. (12/04/18)

PA Board of Nursing Allegheny County Mary Grace Reynolds, license no. RN563113, of Pittsburgh, Allegheny County, was suspended for no

less than three (3) years, with such suspension to be stayed in favor of no less than three (3) years of

probation, and her license will be publicly reprimanded, because she is unable to practice professional

nursing with reasonable skill and safety to patients by reason of mental or physical illness or condition or

physiological or psychological dependence upon alcohol, hallucinogenic or narcotic drugs or other drugs

which tend to impair judgment or coordination, so long as such dependence shall continue. (12/06/18)

Berks County Bruce L. Zellman, license no. PN104837L, of Womelsdorf, Berks County, was ordered to voluntarily

surrender his license because he was convicted of a crime of moral turpitude in the courts of this

Bucks County Blessing Emenyonu, license no. PN301707, of Bensalem, Bucks County, was suspended for no less than

one (1) year, with said suspension stayed in favor of no less than one (1) year of probation, ordered to take

6 hours of continuing education on proper charting and 6 hours of continuing education on nursing ethics,

Carbon County Gregory J Sabo, license no. RN574340, of Lansford, Carbon County, was indefinitely suspended

retroactive to September 12, 2018, and after one (1) year of active suspension, he may seek a stay of the

active suspension in favor of probationary status, because he is unable to practice professional nursing

with reasonable skill and safety by reason of mental or physical illness or condition or dependence upon

alcohol, hallucinogenic or narcotic drugs or other drugs which tend to impair judgment or coordination

and has violated a lawful disciplinary order of the Board. (12/06/18)

Chester County Qui Tang Frey, license no. PN092303L, of Exton, Chester County, was ordered to voluntarily surrender

her license, because she is addicted to alcohol or is addicted to hallucinogenic or narcotic drugs or other

drugs which tend to impair judgment or coordination, so long as such dependence shall continue, or has

become mentally incompetent. (12/06/18)

Linda Kohn, license nos. TP002032G and RN169229L, of Devault, Chester County, was ordered to

voluntarily surrender her license, because she is unable to practice professional nursing with reasonable

skill and safety by reason of mental or physical illness or condition or dependence upon alcohol,

hallucinogenic or narcotic drugs or other drugs which tend to impair judgment or coordination. (12/06/18)

Dauphin County Cheryl Lynn Bretz, license no. PN265235, of Harrisburg, Dauphin County, was suspended for no less

than two (2) years with such suspension to be immediately stayed in favor of no less than two (2) years of

probation, ordered to pay a civil penalty of $250, and her license will be publicly reprimanded, because

she was convicted or pleaded guilty or entered a plea of nolo contendere or was found guilty by a judge or

jury of a felony or a crime of moral turpitude in the courts of this Commonwealth, the United States, or

any other state, territory or country, or received probation without verdict, disposition in lieu of trial or an

Accelerated Rehabilitative Disposition in the disposition of felony charges, or has been dishonorably

discharged or has been discharged under circumstances amounting to dishonorable discharge from the

military forces of the United States or of any other country. (12/06/18)

Lehigh County Laurice Stevens Dunning, license no. RN301131L, of Allentown, Lehigh County, was indefinitely

suspended with such suspension immediately stayed in favor of no less than two (2) years of probation,

because she is unable to practice professional nursing with reasonable skill and safety by reason of mental

or physical illness or condition or dependence upon alcohol, hallucinogenic or narcotic drugs or other

drugs which tend to impair judgment or coordination. (12/06/18)

Luzerne County Noelle Renee Dupont, license no. PN287709, of Wilkes Barre, Luzerne County, was ordered to pay a

civil penalty of $500 and her license was publicly reprimanded, because she was convicted or pleaded

guilty or entered a plea of nolo contendere or was found guilty by a judge or jury of a felony or a crime of

moral turpitude in the courts of this Commonwealth, the United States, or any other state, territory or

country, or received probation without verdict, disposition in lieu of trial or an Accelerated Rehabilitative

Disposition in the disposition of felony charges, or has been dishonorably discharged or has been

discharged under circumstances amounting to dishonorable discharge from the military forces of the

Montgomery County Kristina Marie Crosley, license no. PN285926, of Oreland, Montgomery County, was ordered to pay costs

of investigation of $500, to take 14 hours of remedial education on the topics of medication

administration and nursing law and ethics; and had a public reprimand placed on her permanent

disciplinary record with the board, because she engaged in unprofessional conduct. (12/06/18)

Julieann O'lone, license no. RN599054, of Pottstown, Montgomery County, was indefinitely and actively

suspended for no less than three (3) years, retroactive to July 25, 2018, because she is unable to practice

nursing with reasonable skill and safety by reason of physical or mental illness or condition or

physiological or psychological dependence upon drugs or alcohol and has violated a lawful disciplinary

Northampton County Katherine Morgan Butler, license no. PN303262, of Hellertown, Northampton County, was indefinitely

suspended for no less than three (3) years, with such suspension stayed in favor of no less than three (3)

years of probation, because she is addicted to alcohol or hallucinogenic or narcotic drugs or other drugs

which tend to impair judgment or coordination or has become mentally incompetent. (12/06/18)

Philadelphia County Tawanda Harris, license no. PN300722, of Philadelphia, Philadelphia County, was will be suspended for

no less than three (3) years retroactive to September 12, 2018 with such suspension stayed in favor of no

less than three (3) years of probation retroactive to September 12, 2018, because she is addicted to alcohol

or hallucinogenic or narcotic drugs or other drugs which tend to impair judgment or coordination or has

become mentally incompetent. (12/06/18)

Brady Earle Daniels, license no. RN670640, of Philadelphia, Philadelphia County, was indefinitely

suspended for no less than one (1) year retroactive to September 12, 2018 with such suspension stayed in

favor of no less than one (1) year of probation retroactive to September 12, 2018, because he is unable to

practice professional nursing with reasonable skill and safety by reason of mental or physical illness or

condition or dependence upon alcohol, hallucinogenic or narcotic drugs or other drugs which tend to

impair judgment and coordination. (12/06/18)

Vincent K. Agbemehia, license nos. PN284435 and RN675169, of Philadelphia, Philadelphia County,

was suspended for no less than one (1) year, with such suspension stayed in favor of no less than one (1)

year of probation, ordered to complete six hours of continuing education on proper charting and six hours

of continuing education on nursing ethics, because he has been guilty of unprofessional conduct and has

falsified or knowingly made incorrect entries into a patient's records and pay a civil penalty of $500.

for no less than one (1) year, with such suspension stayed in favor of no less than one (1) year of

probation, ordered to take 6 credits of continuing education and 6 credit hours of nursing ethics, and

Somerset County Joseph W. Visinsky, license no. RN333469L, of Central City, Somerset County, voluntarily surrendered

his license to practice as a registered nurse in the Commonwealth of Pennsylvania, because he is unable to

practice professional nursing with reasonable skill and safety because of mental or physical illness or

condition or dependence upon alcohol, hallucinogenic or narcotic drugs or other drugs which tend to

impair judgment or coordination. (12/06/18)

Washington County Janet Marie Barkley, license no. PN293292, of Burgettstown, Washington County, was indefinitely and

actively suspended for no less than 1 1/2 years or 18 months, must successfully complete remedial

education regarding professional boundaries during suspension, and upon reapplying for reinstatement

will be on probation for eighteen months, because she engaged in conduct defined as a sexual violation or

sexual impropriety in the course of a professional relationship, specifically, by kissing a patient, in that

she engaged in unprofessional conduct by kissing a patient. (12/06/18)

Out of State Dakila Ricarte Romano, PA license no. RN277285L, of Cedar Grove, New Jersey, was ordered to

voluntarily surrender her license because she had a license suspended or revoked or has received other

disciplinary action by the proper licensing authority in another state, territory, possession or country.

practice nursing, because she had a license suspended or revoked or has received other disciplinary action

offered for sale, purchase, or exchange by the dealership; did not have a single business line telephone,

located within the permanent enclosed building, that was used for the dealership and failed to have the

number listed under the dealership’s licensed name; because Respondent did not exhibit a sign, either

permanently affixed to the building or erected in the outdoor display area, that showed the licensed name

of the dealership and which was visible to the public; and did not post its regular business hours in a
