INTRODUCTIONSECTION 1

AHP contracts with LIBERTY Dental Plan of California Inc. (LDP) to provide your dental and administrative services through its special plan Alignment Preferred Dental Plan.

Alignment Preferred Dental Plan is affordable and easy to use! There are virtually no claim forms to complete, and this full service dental plan offers up to 50% discounted fees on basic services. Specialty referral benefits have no annual or lifetime maximum.

You automatically become a member of Alignment Preferred Dental Plan when you become a member of AHP. Your enrollment is effective the first day of the month, following the date you signed up for services. For example, if you sign up on September 15th, your effective date will be October 1st. If you sign up during an Initial Enrollment Period or during the Annual Election Period, your effective date may vary.

You are not required to select a specific dentist at the time of enrollment. However, you must use LDP’s contracted providers to utilize dental benefits. These contracted providers are listed in this directory. The most current provider information may also be found on the Alignment Health Plan’s website at alignmenthealthplan.com or through LDP’s website www.libertydentalplan.com. Always check with your dental office before receiving services to make sure they are a LIBERTY Dental Plan contracted provider.

You may contact LDP via telephone toll free at 1-888-273-3183 Monday through Friday between the hours of 8:00 am and 5:00 pm (excluding holidays). LIBERTY Dental Plan has bilingual representatives that can assist in the language most convenient for you. The chart listed in this section shows all your covered dental procedures and co-payments. Not all benefits may be suitable for you.

Your dentist will determine the appropriate care that you may need. The co-payments listed in the chart apply for services only when prescribed by a contracted LDP provider as a necessary, adequate and appropriate procedure for your dental condition.

Alignment Health Plan (AHP) proudly offers Alignment Preferred Dental Plan. This comprehensive dental plan has no monthly premium, no deductibles and low cost co-payments for more than 250 procedures that include checkups, cleanings, gum care, and restorative work. What is the service area for Alignment Health Plan?

The service area for this plan includes: Los Angeles, Orange, Riverside, San Bernardino, San Joaquin, Stanislaus, Santa Clara and Sand Diego Counties, CA. You must live in one of these areas to join the plan.

OPTIONAL DENTAL TREATMENT

Optional dental treatment is any procedure that is:

• A dental laboratory upgrade of a standard covered material(s) (members may be charged a surcharge based on the additional laboratory costs) or

• A more extensive covered service that is an alternative to an adequate, but more conservative, covered dental service. If you choose a more extensive treatment than what is recommended by your dentist or that is an alternative to an adequate, but more conservative covered dental service, you must pay the difference between your selected dental office’s usual and customary fee for the more extensive treatment, and the usual and customary fee for the covered benefit. You must also pay the co-payment for the covered benefits that are listed in the dental procedures chart.

Sometimes, there are several clinical options that are professionally recognized and could be considered for you by your dentist. To ensure that you receive acceptable dental benefits in a consistent manner, LDP publishes its Governing Administrative Policies (GAPs), and other clinical criteria and guidelines, and distributes them to contracted dentists.

This document is revised periodically to incorporate guidelines under which some treatments should be considered covered or optional. Your dentist may refer to these guidelines to determine the treatment plan to be utilized.

EMERGENCY DENTAL CARE

Emergency dental care is any dental service (provided inside or outside the Alignment Health Plan service area) required for treatment of severe pain, swelling, bleeding, or the diagnosis and treatment of unforeseen dental conditions which, if not treated immediately, may lead to disability, dysfunction, or permanent damage to your health.

IN-AREA EMERGENCY DENTAL CARE

If you feel you require emergency dental care and you are within your Alignment Health Plan service area, call your selected general dentist immediately. The dental office personnel will advise you what to do. Your dental office is available in an emergency 24 hours a day, 7 days a week.

OUT-OF-AREA EMERGENCY DENTAL CARE

If you are outside your Alignment Health Plan service area and are in need of emergency dental care, you may go to any licensed dentist. The services you receive from the out-of-area dentist are covered up to $75 (minus any applicable member co-payments) as long as a transfer to a network provider is a risk to your health. To obtain reimbursement, submit your request for reimbursement, payment receipt, and description of services rendered in writing to:

There are time limits for filing claims for reimbursement. Generally, bills for reimbursement must be submitted to LDP within 6 months of the date of service, unless there is a reason for filing later.

DENIAL OF A REIMBURSEMENT CLAIM FOR OUT-OF-AREA EMERGENCY DENTAL CARE

If your claim for reimbursement of out-of-area emergency dental care is partially or fully denied, LDP will notify you in writing of the decision. The notice states the specific reason for the denial and informs you that you may request a reconsideration of the denial.

To request a review of the denial or partial denial, submit a written notice to Alignment Health Plan within sixty (60) days of receiving the denial notice.

GETTING A REFERRAL TO A DENTAL SPECIALIST

Your selected primary care dentist will assess whether you need a referral to a dental specialist. In cases involving dental emergencies that need the care of a specialist, your selected primary care dentist will contact LDP by telephone to request an emergency referral. For non-emergency referrals, the following procedures apply:

• Your selected primary care dentist completes a written referral form for you and mails it to LDP.

• LDP staff processes the referral request and has a dentist review the request. The referral, if approved by LDP, is made to a

specialist contracted with LDP who practices in your area. If there are no contracted specialists located in your area, LDP may approve treatment by a noncontracted specialist in your area.

For dental benefits to be covered by a LDP specialist, you must first obtain a referral from your LDP primary care dentist.

When approved, specialty services are provided, and your responsibility is limited to applicable co-payments:

• There is no annual maximum for covered specialty care services.

If LDP has not authorized your referral to a specialist, in writing and in advance, LDP is not obligated to pay for services performed.

OBTAINING A SECOND OPINION FOR DENTAL CARE

You may request a second opinion about your dental care from another dentist who contracts with LDP if:

• You are not satisfied with treatment you received from your selected dentist;

• You are uncertain about a proposed treatment plan;

• You do not agree with the recommendations of your selected dentist and/or the LDP Director; or

• You are dissatisfied with the quality of dental work being performed.

Price comparison regarding a proposed procedure or treatment plan is not sufficient grounds for obtaining a second opinion.

To request a second opinion, call Alignment Health Plan Member Services at 1-866-634-2247, TTY: 711, 8:00 a.m. to 8:00 p.m., 7 days a week (except Thanksgiving and Christmas) from October 1 through March 31, and Monday to Friday (except holidays) from April 1 through September 30. Alignment Health Plan’s Dental Consultant shall review your request.

SELECTING ANOTHER DENTIST

You may select any provider within LPD’s provider network, if you are not satisfied with the dentist you selected. Simply contact Alignment Health Plan’s Member Services Department at 1-866-634-2247, TTY: 711, 8:00 a.m. to 8:00 p.m., 7 days a week (except Thanksgiving and Christmas) from October 1 through March 31, and Monday to Friday (except holidays) from April 1 through September 30 and request another dentist.

Note: If you owe your dentist any money at the time you want to transfer to another dentist, you must first pay any monies owed to your current dentist. If you transfer dentists, you are responsible for a nominal fee for the duplication and transfer of X-rays or other records to your new dentist.

Contracted dentists may terminate a member from their practice when a breakdown in a workable doctor-patient relationship occurs or cannot be established. LDP cannot compel or require a contracted dentist to treat any member for any reason. LDP customer service representatives will assist members in selecting a new dentist; please contact Member Services at 1-888-273-3183. LDP will notify you at least thirty (30) days prior to the dentist’s effective termination date so that you may select another dentist.

If you are notified by LDP or by the dentist that

this has occurred, fees for the duplication and transfer of X-rays or other records are waived. You may contact Alignment Health Plan’s Member Services Department at 1-866-634- 2247, TTY: 711, between 8:00 a.m. to 8:00 p.m., 7 days a week (except Thanksgiving and Christmas) from October 1 through March 31, and Monday to Friday (except holidays) from April 1 through September 30, for assistance in transferring to another provider.

Members may not usually transfer dentists while in the middle of a multi-visit procedure where a final impression for a fabrication of a dental appliance has occurred, unless exceptional cause can be shown. This includes crowns, inlays and onlays (advanced restorative procedures), removable partial dentures and complete dentures (removable prosthodontics) and components of bridges (fixed prosthodontics).

In cases where LDP allows for transfer in midprocedure, you may be charged for laboratory charges incurred by the dentist in fabricating the dental appliance. The charges may not exceed the listed co-payments for the covered procedures. In addition, you may be charged for laboratory charges for optional features ordered for you by your dentist.

IF YOUR DENTIST NO LONGER CONTRACTS WITH LIBERTY DENTAL PLAN OF CALIFORNIA, INC.

If the dentist is unable to continue to contract with LDP or if LDP cancels the contract, LDP will notify you at least thirty (30) days prior to the dentist’s effective termination date. Your fees for the duplication and transfer of X-rays or other records will be waived.

EXCLUSIONS & LIMITATIONSSECTION 2

DENTAL BENEFIT EXCLUSIONS AND LIMITATIONS

There are some dental services not covered by Alignment Health Plan under any circumstances (called exclusions). There are also some dental services that are only covered under specific conditions (called limitations).

Once you have signed your agreement to accept and pay for optional treatment, excluded or non-covered services, and those services are initiated by the dentist, you are responsible for payment for those services. For more information, contact Alignment Health Plan Member Services at 1-866-634-2247, TTY: 711, 8:00 a.m. to 8:00 p.m., 7 days a week (except Thanksgiving and Christmas) from October 1 through March 31, and Monday to Friday (except holidays) from April 1 through September 30.

LIMITATIONS

1. Prophylaxis are covered once every six consecutive months.

2. Full Mouth X-rays are limited to once every 36 consecutive months.

3. Fluoride Treatments are covered once every 6 consecutive months.

4. Sealants are covered only on the first and second permanent molars and up to the 14th birth date.

5. Crowns, Jackets, Inlays and Onlays are benefits on the same tooth only once every five years, and consistent with professionally recognized standards of dental practice.

6. Replacement of existing Full and Partial Dentures are covered once per arch every 5 years, except when they cannot be made functional through reline or repairs.

7. Denture Relines are covered twice per year, and only when consistent with professionally recognized standards of dental practice.

8. Any routine dental services performed by a Primary Care Dentist or Specialist in an inpatient/outpatient hospital setting, under certain circumstances, will be considered for coverage.

EXCLUSIONS

1. Any procedure not specifically listed as a Covered Benefit.

2. Replacement of lost or stolen prosthetics or appliances including crowns, bridges, partial dentures, full dentures, and orthodontic appliances.

3. Any treatment requested, or appliances made, which are either not necessary for maintaining or improving dental health, or are for cosmetic purposes unless otherwise covered as a benefit.

4. Procedures considered experimental, treatment involving implants or pharmacological regimens other than listed as Covered Benefit (See “Independent Medical Review” in the Group Evidence of Coverage and Disclosure Form).

5. Oral surgery requiring the setting of bone fractures or bone dislocations.

9. Durable Medical Equipment.

10. Mental Health services.

11. Chemical Dependency services.

12. Home Health services.

13. General anesthesia, analgesia, intravenous/intramuscular sedation or the services of an anesthesiologist other than listed as Covered Benefit.

14. Treatment started before the member was eligible, or after the member was no longer eligible.

15. Procedures, appliances, or restorations to correct congenital, developmental or medically induced dental disorder, including but not limited to: myofunctional (e.g. speech therapy), myoskeletal, or temporomandibular joint dysfunctions (e.g. adjustments/ corrections to the facial bones) unless otherwise covered as an orthodontic benefit.

16. Procedures which are determined not to be dentally necessary consistent with professionally recognized standards of dental practice.

17. Treatment of malignancies, cysts, or neoplasms.

18. Orthodontic treatment started prior to member’s effective date of coverage.

19. Appliances needed to increase vertical dimension or restore occlusion.

20. Any services performed outside of your assigned dental office, unless expressly

authorized by Liberty Dental Plan, or unless as outlined and covered in “Emergency Dental Care” section.

¡Alignment Preferred Dental Plan es un plan económico y fácil de usar! Prácticamente no hay formularios de reclamos que completar y este plan de servicio dental le ofrece hasta 50% de descuento en tarifas de servicios básicos. Las remisiones a especialistas no tienen un máximo anual o de por vida.

TRATAMIENTO DENTAL OPCIONAL

El tratamiento dental opcional es cualquier procedimiento que es:

• Una mejora de laboratorio dental a materiales estándares cubiertos (se puede cobrar a los miembros un recargo basado en los costos adicionales de laboratorio) o

• Es un servicio más extenso que sea una alternativa a un servicio dental cubierto adecuado, pero más conservador. Si elige un tratamiento más extenso que lo recomendado por el odontólogo, o a lo que sea una alternativa a un servicio dental cubierto adecuado, pero más conservador, usted debe pagar la diferencia entre la tarifa usual y acostumbrada del consultorio dental que eligió, el costo del tratamiento más extenso, y el costo usual del beneficio cubierto. RECHAZO DE UN RECLAMO DE REEMBOLSO POR CUIDADO DENTAL DE EMERGENCIA FUERA DEL ÁREA

Si su reclamo de reembolso por cuidado dental de emergencia fuera del área es rechazado total o parcialmente, LDP le informará por escrito su decisión. El aviso le explicará el motivo específico del rechazo y le informará que puede solicitar una reconsideración del rechazo.

Para solicitar una revisión del rechazo total o parcial, envíe un aviso por escrito a Alignment Health Plan dentro de los sesenta (60) días de haber recibido el aviso de rechazo.

OBTENER UNA REFERENCIA A UN ESPECIALISTA DENTAL

Su odontólogo de cuidado primario seleccionado evaluará si usted necesita una referencia a un especialista dental. Su odontólogo primario se contactará por teléfono con LPD para solicitar una referencia de emergencia en casos que involúcran emergencias dentales que requieren el

cuidado de un especialista. Para referencias que no son de emergencia, se aplicarán los siguientes procedimientos:

• Su odontólogo primario seleccionado completará un formulario de referencia por escrito y lo envíará por correo a LPD.

• El personal de LPD procesará la solicitud de remisión y hará que la revise un odontólogo. Si LPD aprueba la remisión, ésta será realizada por un especialista contratado por LPD que ejerza en su área. Si no hay especialistas contratados en su área, LPD puede aprobar que el tratamiento sea proveído por un especialista no contratado en su área.

Para que los beneficios dentales sean cubiertos por un especialista de LPD, primero deberá obtener una referencia de su odontólogo de cuidado primario de LPD.

Una vez aprobados, los servicios especializados serán proporcionados y su responsabilidad se limitará a los copagos aplicables:

• No hay un máximo anual por servicios cubiertos de cuidado especializado.

Si LPD no autoriza su referencia a un especialista por escrito y con anticipación, LPD no está obligado a pagar los servicios proporcionados.

SOLICITUD DE UNA SEGUNDA OPINIÓN PARA CUIDADO DENTAL

Usted puede solicitar una segunda opinión acerca de su cuidado dental de otro odontólogo que tenga un contrato con LDP si:

• Usted no está satisfecho con el tratamiento

que recibe de su odontólogo seleccionado;

• Usted no está seguro acerca de un plan de tratamiento propuesto;

• Usted no esta de acuerdo con las recomendaciones de su odontólogo seleccionado y/o el Director de LDP; o

• Usted no está satisfecho con la calidad del trabajo dental obtenido.

La comparación de precios relacionados a un procedimiento o plan de tratamiento propuesto no constituye un motivo suficiente para obtener una segunda opinión.

Para solicitar una segunda opinión, llame al Departamento de Membresía de Alignment Health Plan al 1-877-399-2247, TTY: 711 de 8:00 a.m. a 8:00 p.m., los 7 días de la semana (excepto el Día de Acción de Gracias y Navidad) desde el 1° de Octubre hasta el 31 de Marzo, y de lunes a viernes (excepto los feriados) desde el 1° de Abril hasta el 30 de Septiembre. Un Consultor Dental de Alignment Health Plan revisará su solicitud.

ELECCIÓN DE OTRO ODONTÓLOGO

Puede elegir cualquier otro proveedor dentro de la red de proveedores de LPD, si no está satisfecho con el odontólogo que eligió. Simplemente llame al Departamento de Membresía de Alignment Health Plan al 1-877-399-2247, TTY: 711 de 8:00 a.m. a 8:00 p.m., los 7 días de la semana (excepto el Día de Acción de Gracias y Navidad) desde el 1° de Octubre hasta el 31 de Marzo, y de lunes a viernes (excepto los feriados) desde el 1° de Abril hasta el 30 de Septiembre y solicite otro odontólogo..

Nota: Si usted le debe dinero a su odontólogo

en el momento que quiere cambiarse a otro, primero debe pagar el balance que le deba a su odontólogo actual. Si cambia de odontólogo, usted es responsable por una tarifa nominal por la duplicación y traspaso de radiografías y otros expedientes a su nuevo odontólogo.

Los odontólogos contratados pueden dejar de atender a un miembro si se interrumpe la relación de médico-paciente o cuando ésta no puede establecerse. LPD no puede obligar o persuadir a un odontólogo contratado que atienda a un miembro por cualquier razón.

Un representante de servicio de LDP le puede asistir para seleccionar otro odontólogo; por favor llame al servicio de membresía al 1-888-273-3183. LPD le notificará con al menos treinta (30) días de anticipación a la fecha de terminación del odontólogo, para que pueda elegir otro odontólogo.

Si LDP o el odontólogo le notifican que ésto ha ocurrido, la tarifa nominal por la duplicación y traspaso de radiografías y otros expedientes queda cancelada. Llame al Departamento de Membresía de Alignment Health Plan al 1-877-399-2247, TTY: 711, de 8:00 a.m. a 8:00 p.m., los 7 días de la semana (excepto el Día de Acción de Gracias y Navidad) desde el 1° de Octubre hasta el 31 de Marzo, y de lunes a viernes (excepto los feriados) desde el 1° de Abril hasta el 30 de Septiembre para obtener ayuda y cambiar de proveedor.

Usualmente, los miembros no se pueden cambiarse a otros odontólogos en medio de un procedimiento que requiere múltiples visitas, cuando se ha tomado un molde final para fabricar un dispositivo dental, a no ser que pueda demostrarse un motivo excepcional. Esto incluye coronas, incrustaciones y facetas (procedimientos de restauración avanzados),

dentaduras removibles parciales y completas (prostodoncias removibles) y mecanismos de puentes (prostodoncias fijas).

En casos cuando LDP permita el traspaso en medio de un procedimiento, es posible que le cobren por los cargos de laboratorio que se le deben al odontólogo que estaba fabricando el dispositivo dental. Los cargos no deben exceder la cantidad del copago establecido por los servicios cubiertos. Adicionalmente, se le pueden cobrar los cargos de laboratorio por requerimientos adicionales que haya pedido su odontólogo.

SI SU ODONTÓLOGO YA NO TIENE UN CONTRATO CON LIBERTY DENTAL PLAN OF CALIFORNIA, INC.

Si su odontólogo no puede continuar su contrato con LDP o si LDP cancela el contrato, LDP le notificará por lo menos treinta (30) días antes de la fecha de terminación del contrato del odontólogo. Las tarifas por la duplicación y traspaso de radiografías y otros expedientes serán canceladas.

EXCLUSIONES Y LIMITACIONES DEL BENEFICIO DENTAL

Existen algunos servicios que no están cubiertos por Alignment Health Plan bajo ninguna circunstancia (exclusiones). También hay algunos servicios dentales que están cubiertos únicamente bajo ciertas condiciones específicas (limitaciones).

Una vez usted haya firmado su contrato y acepte pagar por un tratamiento opcional, o por servicios excluidos o no-cubiertos, y tales servicios son iniciados por un odontólogo, usted es responsable por el pago de esos servicios. LIMITACIONES

1. La profilaxis se cubre una vez cada seis meses consecutivos.

2. Las radiografías bucales completas se limitan a una vez cada 36 meses consecutivos.

3. Los tratamientos de flúor se cubren una vez cada 6 meses consecutivos.

4. Los selladores se cubren sólo en los primeros y segundos molares permanentes y hasta el decimocuarto cumpleaños.

5. Las coronas, fundas, incrustaciones y facetas son beneficios del mismo diente sólo una vez cada cinco años, y cumplen con los estándares de práctica dental profesionalmente reconocidos.

6. El reemplazo de prótesis completas y parciales se cubre una vez por arco cada 5 años, excepto que no puedan hacerse funcionales mediante reparación o recubrimiento.

7. Los recubrimientos de dentaduras se cubren dos veces por año, y solo si cumplen con los estándares de práctica dental profesionalmente reconocidos.

8. Bajo ciertas circunstancias, se considerará cubrir todo servicio dental de rutina proveído por un odontólogo de cuidado primario o especialista en un entorno hospitalario ambulatorio/de internación.

EXCLUSIONES

1. Cualquier procedimiento no mencionado específicamente como un beneficio cubierto.

2. Reemplazo de prótesis o aparatos perdidos o robados, incluyendo coronas, puentes, dentaduras parciales, dentaduras completas y aparatos de ortodoncia.

3. Cualquier tratamiento requerido, o aparato fabricado, no necesarios para el mantenimiento o mejora de la salud dental, o que se realizan con fines cosméticos, a no ser que se cubran de otro modo como un beneficio.

4. Procedimientos considerados experimentales, tratamiento con implantes o regímenes farmacológicos otros que los enumerados como beneficios cubiertos (consulte “Revisión Médica Independiente” en la Evidencia de Cobertura y el formulario de divulgación).

5. Cirugía oral que requiere el arreglo de fracturas o dislocaciones óseas.

6. Hospitalización

7. Servicios ambulatorios.

8. Servicios de ambulancia.

9. Equipo médico duradero/durable.

10. Servicios de salud mental.

11. Servicios de dependencia química.

12. Servicios de cuidado médico a domicilio.

13. Anestesia general, analgesia, sedación intravenosa/intramuscular o los servicios de un anestesiólogo fuera de los enumerados como beneficios cubiertos.

14. Tratamiento iniciado antes de que el miembro fuera elegible, o después de que ya no fuera elegible.

15. Procedimientos, aparatos o restauraciones para corregir trastornos dentales congénitos, del desarrollo o médicamente inducidos, que incluyen pero no están limitados a: trastornos miofuncionales (por ejemplo terapia del habla), mioesqueléticos, o de la articulación temporomandibular (por ejemplo ajustes/correcciones de huesos faciales), a no ser que estén cubiertos de otro modo como beneficios de ortodoncia.

16. Procedimientos que no se consideran odontológicamente necesarios según los estándares profesionalmente reconocidos de la práctica odontológica.

17. Tratamiento de tumores malignos, quistes o neoplasmas.

18. Tratamiento ortodóntico que se haya iniciado antes de la fecha de entrada en vigor de la cobertura.

19. Aparatos necesarios para incrementar la dimensión vertical y restaurar la oclusión.

20. Cualquier servicio realizado fuera del consultorio dental asignado, a no ser que haya sido específicamente autorizado por Liberty Dental Plan, o a no ser que esté enumerado y cubierto en la sección “Cuidado Dental de Emergencia”.
