Outpatient testing now approved for use of Artificial Pancreas

The FDA recently approved a key phase in testing to see if the artificial pancreas will work in an outpatient setting. This

device, a hand-held standard smart phone, was developed by Patrick Keith-Hynes, PhD and Boris Koatchev, PhD. They

reconfigured the device to automatically monitor BG levels and provide as much insulin as needed based on the value.

This would mean people with diabetes will not need to check BG at such regular intervals, or inject with insulin based on

the BG levels.

Based on previous inpatient trials at UVA and in Europe, and an ongoing outpatient trial that started in Italy and France

last year which displayed promising results, the FDA has approved the first U.S. outpatient clinical trials. The first eight

type 1 diabetes European participants in the outpatient trial were able to maintain safe blood sugar levels while

spending a night outside of a hospital.

Kovatchev is eagerly anticipating this crucial step in the development of the device after years of research and testing,

saying: "Conducting the first U.S. tests of a portable artificial pancreas running on a cell phone in a real-world setting is

an important step toward evaluating its effectiveness and how it may impact treatment for type 1 diabetes patients in

the United States." He anticipates the U.S. outpatient trial to begin within about six weeks.

The Artificial Pancreas Project is a collaboration of researchers from UVA, the University of California Santa Barbara,

Montpellier University Hospital in France, and the Universities of Padova and Pavia in Italy.

JDRF Scholarships-

May 15th Deadline

The Diabetes Scholars Foundation Scholarship Program is available to incoming freshmen

seeking a higher education at an accredited four year university, college, technical or trade

school. This scholarship recognizes students who are actively involved in the diabetes

community, who have high academic performance, who participate in community and/or

extra-curricular activities and who have demonstrated that they are successfully managing

the challenges of living with diabetes.

Diabetes Scholars Foundation scholarships are not based on financial need. There are now 4

JDRF Scholarships included in the list.

• The JDRF Medical/Research Scholarship ($1,000) will be awarded to a student majoring in Healthcare

• The JDRF Advocacy Scholarship ($1,000) will be awarded to a student majoring in Political Science

• The JDRF Outreach Scholarship ($1,000) will be awarded to a student majoring in Mental Health

(Psychology/Social Work)

• The JDRF Rising Star Scholarship ($1,000) will be awarded to an incoming freshmen with Type 1 diabetes

The deadline to apply is May 15th

Who doesn’t like a contest…especially one with prizes!!

DiabetesMine is asking YOU, the empowered PWDs (peoples with diabeteses) to tell them

about your most pressing design needs. Your mission, should you accept, is to create a 2-3

minute video testimonial about the biggest challenges you face in your diabetes care, and

how you would present those concerns to experts.

The DiabetesMine team will select 10 of the top entrants to win an “e-Patient Scholarship”

to attend the 2012 DiabetesMine Innovation Summit at Stanford University in Palo Alto,

CA, on Nov. 16, 2012.

The winners will act as “delegates” for the patient community, expressing needs and

desires to the Powers That Be: Pharma R&D and marketing executives, human interaction

designers, entrepreneurs, physicians, regulatory experts, investors, health gaming

developers, technology gurus and experts in mobile health platforms.

Each of the 10 winners will also receive an iBGStar Blood Glucose Monitoring System, the first meter to directly

connect to an iPhone* or iPod touch*! This exciting new product has just been introduced to the market as of May 1,

2012.

T1D Exchange Clinic Registry

The T1D Exchange is a three-year, $26 million project funded by the Leona M. and Harry B. Helmsley Charitable Trust’s

Type 1 Diabetes Program. The T1D Exchange combines critical components of traditional clinical research with the

expanding fields of health information technology and social networking. In addition to this registry, the program will

also have a type 1 diabetes portal where eligible members can connect and collaborate in a social media setting.

Further, a biobanking service is being developed to work with T1D Exchange to allow researchers to easily access

samples from a pre-identified number of consenting individuals.

“The T1D Exchange platform is intended to bring together individuals with type 1 diabetes, clinicians and researchers to

advance the understanding of type 1 diabetes, identify challenges in treatment and control and allow easier access to

meaningful disease-specific data,” said Janak Joshi, Executive Director of the T1D Exchange. “It is very exciting to see the

dramatic pace at which this registry has grown in a very short period of time. This is just the beginning. At a time when

the focus of healthcare in the United States is being directed at patient outcomes, the T1D Exchange is unprecedented in

its potential to change the lives of those living with type 1 diabetes. We look forward to working on many studies and to

launching the biorepository and the social media site, Glu, which is a new approach to social networking for people with

type 1 diabetes.

Clinic Registry Objectives:

Collect clinical and laboratory data on a large number of patients with type 1 diabetes to:

• Address pertinent clinical issues

• Conduct exploratory/hypothesis-generating analyses

• Identify patients interested in being contacted about participating in research studies

• Categorize patients for future clinical studies and biobank analyses

Initial Findings from data include:

� Increase in frequency of hypoglycemia* as age increases >50 years. (*defined as

needing assistance)

� Increase in frequency of severe hypoglycemia* as age increases >50 years (*defined as

seizure or coma or loss of consciousness)

� Decrease in frequency of DKA as age increases >50 years.

� Frequency of DKA most prevalent in population age 13 – 17 with 18-24 age range close

behind.

� Frequency of DKA also climbed from 2% of the time in those with A1C of 6% to 26% for

those with A1C values >11%.

Jenny’s Fun Facts

On the way to work every day I have seen an increasing number of cyclists. This is likely due to

the increase in sunny days. I can’t help but think of all the time many of you will soon be

spending outdoors doing any number of activities. I thought it would be a good idea to go

over the appropriate treatment of hypoglycemia, or low BG. Since I myself do not live too far

from my office I take the opportunity to walk every day and I always carry something in my

bag to treat a low …just in case.

What is appropriate and how much??

Simple sugar should always be used first. This would include: glucose tablets, skittles,

SweetTarts, jelly beans (1 Jelly Belly has 1 gram of carb – so you get a lot of these) juice or hard

candy.

Treatment amount will vary depending on your body size (weight).

Some general rules to follow:

<60 lbs = 1 gram will raise BG 6-10mg/dl (.33-.55 mmol/l)

60-100 lbs = 1 gram will raise BG 5mg/dl (.28 mmol/l)

101-160 lbs = 1 gram will raise BG 4mg/dl (.22 mmol/l)

161-220 lbs = 1 gram will raise BG 3mg/dl (.17 mmol/l)

When treating a low that occurs during or just after exercise, you will likely need more than the usual treatment since

your BG will be dropping faster than usual. As a general rule, check your blood sugar and take enough carb to raise your

BG to 140-150 (8 mmol/l).

Why should you only use simple sugar to raise BG?

You may think it’s a great time to indulge in your favorite caramel cashew dark chocolate clusters, or some other

favorite food that contains more than just sugar - many times a lot of fat and sometimes protein. These nutrients

(especially fat) do not digest quickly and will not bring the BG up fast enough. This can leave you feeling the low BG a lot

longer and likely lead to overtreatment.

A good rule of thumb is to treat the low BG and then retest in 15-20 minutes to ensure the BG is rising. Consider when

your next meal is planned. If it is >2 hours away, you may need a small snack AFTER the treatment has brought your BG

back up to keep the BG stable until the next meal.

This may also be a time to evaluate how you are planning exercise in your day. An increase in cardio or aerobic exercise

will always bring BG down – this is good!! However, it’s also important to have a plan to avoid or at least be prepared to

treat the lows when they do arise.

An exciting NEW event for young adults with Type 1 diabetes is being organized for the Summer of 2012. June 1-3 in

Tampa, FL will be the Students With Diabetes National Conference. This event will feature 2 1/2 days of diabetes

education, social events and connection opportunities for young adults ages 18-30.

Diabetes celebrities will be speaking, health professionals will be talking about diabetes reality for young adults, and

participants will be engaging in team-building activities like ropes courses and theme park fun.
