What is a One Page Profile?

The One Page Profile was developed by Helen Sanderson. She developed a One Page Profile to provide her daughter’s teacher a brief introduction to her daughter so she would know what was important to her and how to best support her in the educational setting. Since then, thousands of One Page Profiles have been developed for a variety of reasons. The specific purpose of this One Page Profile version is address the needs of people afflicted by COVID-19 and are in need of medical care. One Page Profiles always have 3 components: A positive introduction of the person What matters to that person, or, what is Important TO that person, And, how to best support the person, given the purpose of the One Page Profile.

Person Centered Description

My mom is a quiet woman. She tries to keep people from noticing

her. She is polite and is very intelligent, resourceful, frugal, and creative. She writes poems and has

had several published.

Mom loves her sons and they are very important to her. She lost two other sons in tragic accidents. Mom

does not express love with a hug but through a phone call and

immediate hang-up when answered. It is expressed with her tolerance when her sons try to take

care of her – she wants to be independent and left alone.

Mom is a simple person that carries her “Nuclear Football” (important papers) where ever she goes. She

drinks coffee from an old percolator and politely declines a good Keurig

brewed cup.

Mom often gets scared at the voices she hears, the people that try to get in her business, the cameras planted in the holes around the house, and the

memories of abuse.

Mom is good at managing her funds and making ends meet. She often

gives the little she has to help someone else. This often frustrated me because she was often worse off than the person she helped. But this taught me the importance of other people and that self-sacrifice is a

noble characteristic.

Mom wanted to die on her own. She did not want me to sit with her. She wanted me to help her get dressed

and leave the hospital room. I think she had lost so much that in this

moment of silence…all alone…she could leave assured that my pain

would not be as great as hers.

If you are developing a One Page Profile for COVID-19, or for any other reason, you need to ask yourself three questions: Where is the One Page Profile going to be used and in what context? For this purpose, it is to assist the health care professionals in knowing the person receiving care and how to support the person when diagnosed with COVID-19. But, we also want to ask ourselves: What do we want the health care professional to learn from reading the One Page Profile, and, What do you want the professionals to do with the learning? It is also important to know that even if we write something brilliant, but no one reads or uses it, we have wasted our time.

In looking at these examples, we hope one or more resonates and helps with the people that you are supporting. As you start to ask the questions in the following slides, take notes on the answers, but don’t worry about filling out the template yet. Use the questions to guide your work, but first you need to consider who can best answer the questions. Who is the best person (or persons) to ask. Who knows the answers? We always start with the person. Can they answer all of the questions? If others, who? Family? Close friends? Care Givers? Who can tell you what others like, admire, or appreciate about the person?

In the COVID-19 Health Care Setting, people will be isolated. We all know how impactful being completely alone, for several days, can be on a person. So, when finding out what is important to the person, you first want to ask who you do you want to be in contact with? And, how often? Who are the people that need to know how you are doing? Would they want to be contacted every day? What is their contact information? In the hospital, the person we want the person to feel safe, so we want to ask, “What helps you feel better when you are sick? What can we do to help you when you are upset? What allows you to fall asleep and sleep well? Do you have any spiritual practices that we should be aware of? Such as with Joanne, having her rosary and prayer book within reach is important to her. And finally, What what helps you feel valued and respected? Do you want detailed information about your health or just a quick summary? How involved in your healthcare decisions makes you feel respected? What about your need for privacy?

What Does Good Support Look Like?

• Describe how staff can help you feel less anxious or uncomfortable, or calm or safe

• Describe the best way for staff to communicate with you• Outline how staff can help prepare you for procedures, tests, and

changes in care• Explain how you might express pain• Include how you prefer to take medications• Include any physical or environmental accommodations you might

need• Describe any tips that might help staff help you

You also want to include what good support looks like: What do you want the staff to do to make you feel less anxious and comfortable? Are there things that make you feel more calm or safe? What is the best way for the nurses and other staff to communicate with you? Do you need details or just a brief summary of what is going on? Do you want step-by-step details of how testing will be performed, or, do you just want to get it done and over? You may ask the person how they prefer to take medication. Do they want it with food? Pill or liquid form? In the healthcare profession, people are often asked to provide a number on the 1-10 scale for pain. Ask the person if this works for them or if there is some other way we would know they are in pain and how acute the pain is. Is there anything you might need to make you more comfortable; such as, an extra pillow? The call button on your bed or tray table? Or with Josie, she would like to have something wrapped around her pillow to avoid sweating. What other tips might you share to provide you with the best care?

3When it is done, read it out loud -it should take no more than a minute

Look for the 4, 5, or 6 top things for each section. Some people will want to list more than this and that is ok. But try to keep the information as short as possible and do not feel you must use complete sentences. Use plain, simple language that everyone can understand. It is important to read the final One Page Profile out loud, if it takes more than one minute to read, it will not get read. Healthcare professionals will not have time to read the Profile. If it does take longer, go back to the person and explain this point and ask what are the 4, 5, or 6 top things that are most important to note in each section.

Share the first draft with the person (and representative where appropriate), If the person says “not quite” edit it until you get it right Review the “final” copy with the person. If it is too long, remind the person of the intent and negotiate to get the critical information but no more than that. It will be more powerful if there is a photo, so use a photo if at all possible. Remember that hospital staff will want the basic healthcare information. It is likely the person may get transferred to other departments, or even hospitals, so it is important to be sure the One Page Profile is available to the various healthcare professionals that will need the information. If providing a printed copy of the Profile, be sure the One Page Profile is on the front page and the Basic Healthcare Information is on the back.

part of a “go bag”• Consider laminating or putting in plastic sleeves• In the hospital –
