Complete by Tuesday, August 18th

Complete the Designation Form by identifying every student in your classroom.

Use the Identify/Designate Instructions on pgs. 9-10 to guide you.

Use the Electronic Signature Instructions on pgs. 11-14 to guide you.

Begin by saving a copy of this form to your desktop:

Click on Submit by Email at the top of the completed Designation Form.

Send by Friday, August 21st

1. Write due date on the top of the Recommended for Retention letter (copies available at your site).

2. Fill out the bottom section.

For every student designated as

AIP or ELD.

4. Staple letter and flyer together and send to every student you designated as AIP or ELD (copies of the flyer will be available at your site).

5. Remind students/parents to return the letter by Thursday, August 29th. The more you receive now, the less to receive and file later!

3. White and yellow copies to parent. Pink copy is for you.

For new students, email your designation of

that student to [email protected].

If a parent marks NO on the letter, immediately notify the Educational Options Department of the student’s name to [email protected].

You may re-designate students (i.e. from Proficient to AIP) by emailing your designation of that student to [email protected]. You may re-designate the student simply by “teacher judgment.” Remember to send the letter and flyer, receive the letter signed by the parent, and file it in the cum folder. In certain situations, it may require a face-to-face teacher/parent explanation for the change.

2nd Grade Identify/Designate Instructions

IDENTIFY

Step 1: Mark any from last year’s assessments (if applicable):

CE = CELDT (1 or 2)

DA = District Assessments (any one):

⇒ OARS Comprehension (Last unit – below 80% primary).

⇒ OARS Writing (below 2.5 average)

⇒ Spring Math Benchmark assessment (below 65%)

BG = below grade level (teacher judgment).

Step 2: Mark only one (if previous boxes not applicable):

PR = Proficient in each criteria above.

MO = Moved out of your class.

DESIGNATE

Step 3: Mark only one box:

AIP if any red box is checked from the IDENTIFY section.

ELD if the yellow box is checked from the IDENTIFY section

PRO if the green box is checked in the IDENTIFY section

MO if the white box is checked from the IDENTIFY section.

Only 1 check is needed in the entire Identify

Section.

Actual numbers are not needed.

Just place an “X” in a box.

If a student is both AIP and ELD

then… Check ELD only!

MA = Math CST (below 325)

DA = District Assessments (any one):

70% intermediate).

BG = below grade level (teacher judgment).

The Reason

Please read, sign, and return the notification letter. YES Answer: You are accepting After-School AIP and Summer School services. NO Answer: You are refusing After-School AIP and Summer School services. (If your child is in After School Program ASES during academic hours, your child will be sent to an AIP class).

First Day: Starts Tuesday, September 1st. Days: Every Tues, Wed, and Thurs. Times: See schedule. Focus: Language Arts and Math Instruction (ELD instruction also available).

How? Your child’s teacher has made this early recommendation based on last year’s test scores. Why? To give your child extra and specialized instruction to increase his/her chances to be promoted to the next grade. Your child earns promotion points!
