PURPOSE OF THE POSITION This position is responsible
(i) To provide specialist clinical nursing expertise to Urological patients under the care of Surgery, Anaesthesia and Perioperative Services (SAP) at Counties Manukau Health
(ii) To promote excellence and innovation in nurse led clinical practice, ensuring the provision of high quality care that is validated by evidence based best practice.
(iii) To work closely with all members of the multidisciplinary team to ensure that Urological patients receive seamless, patient/family centred care across hospital and ambulatory services whilst strengthening integration initiatives already underway regionally and in localities of Counties Manukau.
(iv) Provide nursing leadership within the service in order to ensure patient treatment and care planning is comprehensively co-ordinated and informed by specialist knowledge utilising care models that best meets the speciality whilst in partnership with the patient and family.

NATURE AND SCOPE OF RESPONSIBILITIES

Demonstrate clinical expertise in the planning and delivery of patient care within the Urology service.
Be familiar with and ensure safe progression to competency to undertake a range of urological procedures to include catheterisation, cystoscopy, bladder instillations, prostate biopsy and urodynamic studies.
Undertake nurse led clinical activities for appropriately triaged Urology patients
Utilise advanced clinical assessment skills to identify potential or actual problems and collaborate with the multidisciplinary team to proactively address these.
Participate in the review, development and implementation of Urology referral guidelines and care pathways to better support patient care.
Work collaboratively with all members of the health care team to ensure staff receive high quality education input and support.
Develop and maintain a collegial relationship/partnership with Auckland Regional Urology services and Auckland Regional Spinal Unit and participate in regional nursing initiatives.
Initiate and/or contribute to forums that critique current practice and debate topical issues
Monitor national and international evidenced based research and trends and apply to practice as appropriate.
Promote innovative and creative clinical nursing practice and ensure it is validated with best practice and research activities.
Promote a patient/family/ whaanau focussed approach by addressing practical aspects of care in response to patients’ needs thus ensuring great patient satisfaction.
Report monthly to Service Manager Urology - keep CND advised on professionals issues
Leadership
Act as an effective role model by demonstrating high levels of clinical competence and professionalism.
Oversee the assessment, planning, implementation and evaluation of nursing care and practice within the speciality.
Provide coaching and mentorship to staff in the speciality.
Advocate on behalf of patient / family/ whaanau, and colleagues as appropriate and in a culturally safe manner.
Assist the multidisciplinary team with decision making related to medico-legal and ethical issues.
Participate in business, nursing and education planning processes at a strategic and service level.

PROBLEM COMPLEXITY The Clinical Nurse Specialist Urology is expected to manage a diverse variety of complex and unpredictable problems that are associated with the assessment, treatment and management of Urological patients. Coordination of care and appropriate services need to be customised to provide focused solutions that meet the patient circumstances. There will be demands to meet deadlines and requirements to prioritise issues, to negotiate timeframes and maintain accuracy and quality of information while providing a quality customer service. The Clinical Nurse Specialist Urology will provide clinical expertise for Urological patients seen in a variety of inpatient, outpatient settings and associated phone consultations.
Will refer to senior medical and senior nursing colleagues for advice or second opinion on clinical issues.
SCOPE FOR ACTION The Clinical Nurse Specialist Urology will be required to develop innovative approaches to problem solving and utilise the skills of other team members to co-ordinate and resolve problems. They will need to negotiate with and co-ordinate a variety of staff at all levels across the organisation and will frequently need to develop individual solutions for each issue and problem. The Clinical Nurse Specialist Urology will be required to respond to referrals from regional and CMH emergency departments, wards, theatres, nursing, allied and medical staff. The Clinical Nurse Specialist Urology will be required to provide nursing leadership in the speciality and contribute at an organisational and regional level including making recommendations for improvement in clinical practice within services related to quality and resource issues. This includes effective liaison/consultation with staff, appropriate external parties including patients and their families/ whaanau and contributing to effective training and development. Discretion is required to be exercised in releasing confidential information to the appropriate parties.
DIMENSION OF THE POSITION The Clinical Nurse Specialist Urology will be responsible to the Service Manager for achievement of objectives and goals for their service and the Clinical Nurse Director for professional advice and direction whilst operating within the confines of Counties Manukau Health. The Clinical Nurse Specialist Urology will be required to provide an expert contribution at the organisational and regional level. They will make recommendations for improvement in clinical practice within the service related to quality, resource issues and performance development. This includes effective liaison with staff and appropriate external parties and groups, co-ordination of available resources, optimisation of staff skills and effective training and development.
