    Dear Applicant:

    Thank you for your interest in applying for a grant under the Child Care Access MeansParents in School (CCAMPIS) Program. This letter highlights a few items in theapplication package that will be important to you in applying for a grant. You shouldreview the entire application package carefully before preparing and submitting yourapplication.

    We are requiring that applications for Fiscal Year (FY) 2005 grants under the CCAMPISProgram be submitted electronically using e-Application available through theDepartments e-Grants system. The e-Grants system is accessible through its portal page

    at: http://e-grants.ed.gov.

    An applicant who is unable to submit an application through the e-Grants system mustsubmit a written request for a waiver of the electronic submission requirement at least twoweeks before the deadline date. Additional information about e-Application submissionrequirements is in the Notice Inviting Applications (Notice), published in the FederalRegister, and in the transmittal instructions, which are included in this package.

    In addressing the Need for the Project selection criterion, please provide only data thatrelates to students who may be assisted by your project: only eligible students -- thosestudents eligible for Federal Pell Grants -- enrolled at your institution. We will notconsider demographic data on other students for the purpose of determining the extent towhich you demonstrate need for the grant.

    All applicants must provide a summary budget (ED 524) and a detailed Budget Narrativefor the first twelve-month budget period. It is not necessary to provide a budget summaryfor the entire grant period. The maximum funding level for the first year of the grant isstipulated in the Notice, and the Department will determine the funding levels for eachadditional year of the grant award. We will use the required grantees interim 18-monthand 36-month performance reports to determine progress for purposes of continuationfunding.

    Applicants successful under this competition will be required to submit 18-month and 36-month performance reports. The requirements for these reports are detailed in theauthorizing statute, Section 419N(e) of the Higher Education Act of 1965, as amended, acopy of which is included in this application package.

    The Secretary will not award grants that supplant funds for current child care services.CCAMPIS Program funds may be used only to supplement child care services or establisha new child care program. CCAMPIS Program funds may be used either to provide child

    care services or contract for the provision of child care services. However, the institutionmust have oversight authority for all child care services, whether provided directly by theapplicant or by a contractor.

    Please note that for the FY 2005 competition we have established a competitive preference

    priority. Applicants may receive up to 10 additional points based on the extent to whichprojects meet this priority. Additional information about this competitive priority isincluded in the Notice and in the application package.

    All applicants are required to adhere to the page limit of 45 pages for the combinedBudget Narrative and Program Narrative. This page limit will be strictly enforced.Applications that exceed the page limit will not be reviewed. The Notice contains specificinformation governing page limits.

    An eligible applicant may submit only one application. All currently funded grantees areeligible to apply under the FY 2005 competition. Because we do not anticipate conducting

    a FY 2006 competition, we urge all grantees, including those whose grants end in 2006, toapply under this competition. The Notice contains detailed information. We encourageapplications from Student Support Services (SSS) Program grantees. The SSS Program, aFederal TRIO program, is designed to assist economically disadvantaged andnontraditional students, who also may qualify for CCAMPIS Program services.

    The application must be submitted on or before the deadline date. Applications submittedlate will not be accepted. We suggest that you submit your application at least one daybefore the deadline date. The Department is required to enforce the established deadline toensure fairness to all applicants. No changes or additions to an application will beaccepted after the deadline date.

    Authorizing Statute

    Title IV, Part A, Subpart 7, Section 419N of the Higher Education Act of 1965,as amended (HEA)

    Subpart 7--Child Care Access Means Parents in School

    SEC. 419N. CHILD CARE ACCESS MEANS PARENTS IN SCHOOL.`(a) PURPOSE- The purpose of this section is to support the participation of low-income parents in postsecondary education through the provision of campus-based child care services.`(b) PROGRAM AUTHORIZED-

    `(1) AUTHORITY- The Secretary may award grants to institutions of highereducation to assist the institutions in providing campus-based child careservices to low-income students.`(2) AMOUNT OF GRANTS-

    `(A) IN GENERAL- The amount of a grant awarded to an institutionof higher education under this section for a fiscal year shall notexceed 1 percent of the total amount of all Federal Pell Grant fundsawarded to students enrolled at the institution of higher education forthe preceding fiscal year.`(B) MINIMUM- A grant under this section shall be awarded in anamount that is not less than $10,000.

    `(3) DURATION; RENEWAL; AND PAYMENTS-`(A) DURATION- The Secretary shall award a grant under thissection for a period of 4 years.`(B) PAYMENTS- Subject to subsection (e)(2), the Secretary shall

    make annual grant payments under this section.`(4) ELIGIBLE INSTITUTIONS- An institution of higher education shall beeligible to receive a grant under this section for a fiscal year if the totalamount of all Federal Pell Grant funds awarded to students enrolled at theinstitution of higher education for the preceding fiscal year equals orexceeds $350,000.`(5) USE OF FUNDS- Grant funds under this section shall be used by aninstitution of higher education to support or establish a campus-based childcare program primarily serving the needs of low-income students enrolledat the institution of higher education. Grant funds under this section may beused to provide before and after school services to the extent necessary to

    enable low-income students enrolled at the institution of higher education topursue postsecondary education.`(6) CONSTRUCTION- Nothing in this section shall be construed to prohibitan institution of higher education that receives grant funds under thissection from serving the child care needs of the community served by theinstitution.`(7) DEFINITION OF LOW-INCOME STUDENT- For the purpose of thissection, the term `low-income student' means a student who is eligible to

    receive a Federal Pell Grant for the fiscal year for which the determinationis made.

    `(c) APPLICATIONS- An institution of higher education desiring a grant under thissection shall submit an application to the Secretary at such time, in such manner,and accompanied by such information as the Secretary may require. Each

    application shall--`(1) demonstrate that the institution is an eligible institution described insubsection (b)(4);`(2) specify the amount of funds requested;`(3) demonstrate the need of low-income students at the institution forcampus-based child care services by including in the application--

    `(A) information regarding student demographics;`(B) an assessment of child care capacity on or near campus;`(C) information regarding the existence of waiting lists for existingchild care;`(D) information regarding additional needs created by

    concentrations of poverty or by geographic isolation; and`(E) other relevant data;`(4) contain a description of the activities to be assisted, including whetherthe grant funds will support an existing child care program or a new childcare program;`(5) identify the resources, including technical expertise and financialsupport, the institution will draw upon to support the child care program andthe participation of low-income students in the program, such as accessingsocial services funding, using student activity fees to help pay the costs ofchild care, using resources obtained by meeting the needs of parents whoare not low-income students, and accessing foundation, corporate or otherinstitutional support, and demonstrate that the use of the resources will notresult in increases in student tuition;`(6) contain an assurance that the institution will meet the child care needsof low-income students through the provision of services, or through acontract for the provision of services;`(7) describe the extent to which the child care program will coordinate withthe institution's early childhood education curriculum, to the extent thecurriculum is available, to meet the needs of the students in the earlychildhood education program at the institution, and the needs of the parentsand children participating in the child care program assisted under thissection;`(8) in the case of an institution seeking assistance for a new child careprogram--

    `(A) provide a timeline, covering the period from receipt of the grantthrough the provision of the child care services, delineating thespecific steps the institution will take to achieve the goal of providinglow-income students with child care services;

    `(B) specify any measures the institution will take to assist low-income students with child care during the period before theinstitution provides child care services; and`(C) include a plan for identifying resources needed for the child careservices, including space in which to provide child care services, and

    technical assistance if necessary;`(9) contain an assurance that any child care facility assisted under thissection will meet the applicable State or local government licensing,certification, approval, or registration requirements; and`(10) contain a plan for any child care facility assisted under this section tobecome accredited within 3 years of the date the institution first receivesassistance under this section.

    `(d) PRIORITY- The Secretary shall give priority in awarding grants under thissection to institutions of higher education that submit applications describingprograms that--

    `(1) leverage significant local or institutional resources, including in-kind

    contributions, to support the activities assisted under this section; and`(2) utilize a sliding fee scale for child care services provided under thissection in order to support a high number of low-income parents pursuingpostsecondary education at the institution.

    `(e) REPORTING REQUIREMENTS; CONTINUING ELIGIBILITY-`(1) REPORTING REQUIREMENTS-

    `(A) REPORTS- Each institution of higher education receiving agrant under this section shall report to the Secretary 18 months, and36 months, after receiving the first grant payment under this section.`(B) CONTENTS- The report shall include--

    `(i) data on the population served under this section;`(ii) information on campus and community resources andfunding used to help low-income students access child careservices;`(iii) information on progress made toward accreditation of anychild care facility; and`(iv) information on the impact of the grant on the quality,availability, and affordability of campus-based child careservices.

    `(2) CONTINUING ELIGIBILITY- The Secretary shall make the third annualgrant payment under this section to an institution of higher education only ifthe Secretary determines, on the basis of the 18-month report submittedunder paragraph (1), that the institution is making a good faith effort toensure that low-income students at the institution have access toaffordable, quality child care services.

    `(f) CONSTRUCTION- No funds provided under this section shall be used forconstruction, except for minor renovation or repair to meet applicable State or localhealth or safety requirements.

    `(g) AUTHORIZATION OF APPROPRIATIONS- There are authorized to beappropriated to carry out this section $45,000,000 for fiscal year 1999 and suchsums as may be necessary for each of the 4 succeeding fiscal years.'.

    competition. Applicants for FY 2005 CCAMPIS Program grants must submit theirapplications electronically, unless they receive a waiver. The Notice containsspecific requirements and waiver instructions.

    You must submit your grant application through the Internet using the softwareprovided on the e-Grants Web site (http://e-grants.ed.gov) by 4:30 p.m.(Washington, D.C. time) on the application deadline date. The regular hours ofoperation of the e-Grants Web site are 6:00 a.m. Monday until 7:00 p.m.Wednesday; and 6:00 a.m. Thursday until midnight Saturday (Washington, D.C.

    time). Please note that the system is unavailable on Sundays, and after 7:00 p.m.on Wednesday for maintenance (Washington, D.C. time). Any modifications tothese hours are posted on the e-Grants Web site.

    If you submit your application through the Internet via the e-Grants Web site, youwill receive an automatic acknowledgment when we receive your application.

    If you are unable to submit your application electronically, you must request awaiver of the electronic transmission requirement pursuant to the specificinstructions contained in the Notice. Note: All requests for a waiver must besubmitted at least two weeks before the deadline date. Address and mail or fax

    Intergovernmental Review of Federal Programs

    This applies to each program that is subject to the requirements of

    Executive Order 12372 (Intergovernmental Review of Federal Programs) and the

    regulations in 34 CFR part 79.

    The objective of the Executive order is to foster an intergovernmental

    partnership and to strengthen federalism by relying on State and local processes

    for State and local government coordination and review of proposed Federal

    financial assistance.

    Applicants must contact the appropriate State Single Point of Contact to

    find out about, and to comply with, the State's process under Executive Order

    12372. Applicants proposing to perform activities in more than one State should

    immediately contact the Single Point of Contact for each of those States and follow

    the procedure established in each of those States under the Executive order. A

    It is estimated that in 2004 the Federal Government will outlay $400 billion in grants toState and local governments. Executive Order 12372, "Intergovernmental Review of FederalPrograms," was issued with the desire to foster the intergovernmental partnership and strengthenfederalism by relying on State and local processes for the coordination and review of proposed

    Child Care Access Means Parents in School(CCAMPIS) Program

    SELECTION CRITERIAThe Secretary uses the following selection criteria to evaluate applications for newgrants under the CCAMPIS Program. The maximum score for the total of thesecriteria (A-E) is 100 points. The maximum score for each criterion is indicated inparentheses. For applicants who have an existingchild care program, themaximum score for each criterion is in italics: e.g., (10). The maximum score foreach criterion for applicants who will establish a new child care program areunderlined: e.g., (10)

    Note: The authorizing statute, Section 419N(b)(7) of the HEA, defines low-

    income student as a student who is eligible to receive a Federal Pell Grant for thefiscal year for which the determination is made.

    A. Need for the Project. (Maximum 35 Points)

    The applicant must demonstrate the need for campus-based child servicesfor low-income students enrolled at the institution by including the following:

    1. Information regarding student demographics; (10) (10)

    B. Plan of Operation. (Maximum 25 Points)

    The applicant must describe the activities to be assisted, and must specifywhether the grant funds will support an existingchild care program or a new childcare program.

    Note: Only projects for new child care programs are required to addresssubcriterion 2, below.

    1. All applicants must address the following:

    (a) The plan to identify and select students with the greatest need of childcare services; (7) (4)

    (b) The type of child care services to be provided and the likely impact ofthe services on the intended recipients of those services; (6) (3)

    (c) (1) If the institution has an early childhood education curriculum, theextent to which the child care program will coordinate with thatcurriculum to meet the needs of the students in the early childhoodeducation program at the institution, and the needs of the parents andchildren participating in the child care program; (6) (3)

    (2) If the institution does not have an early childhood educationcurriculum, the extent to which the project will coordinate with theinstitutions other programs for economically disadvantaged students tomeet the needs of the parents and children participating in the child careprogram. (6) (3);

    (a) Provide a timeline, covering the period from receipt of the grant throughthe provision of the child care services, delineating the specific steps theinstitution will take to achieve the goal of providing low-income studentswith child care services; (4)

    (b) Specify any measures the institution will take to assist low-incomestudents with child care during the period before the institution provideschild care services; (4)

    C. Management of Project. (Maximum 20 Points)

    All applicants must describe the management plan for the proposed project, andinclude the following:

    1. The resources, including technical expertise and financial support, theinstitution will draw upon to support the child care project and theparticipation of low-income students in the project, such as accessing socialservices funding, using student activity fees to help pay the costs of childcare, using resources obtained by meeting the needs of parents who arenot low-income students, and accessing foundation, corporate or otherinstitutional support; and must demonstrate that the use of the resourceswill not result in increases in student tuition; (5) (5)

    2. The qualifications, including relevant training, experience, and timecommitment of key project personnel; (5) (5)

    3. The organizational placement of the project; (5) (5)

    AND

    4. Specific plans for financial management, student records management,and personnel management. (5) (5)

    D. Evaluation. (Maximum 15 Points)

    All applicants must describe the evaluation plan for the project and must:

    1. Demonstrate that the evaluation plan is:

    (a) appropriate to the project; (3) (3)

    Competitive Preference Priority

    In accordance with 34 CFR 75.105(b)(2)(iv), this priority is from section419N(d)(1)(2) of the HEA. An applicant can receive up to 10 points, depending onthe extent to which the application meets this priority:

    Priority is given to institutions of higher education that submit applicationsdescribing child care programs that:

    1. Leverage significant local or institutional resources, including in-kindcontributions, to support the activities;

    AND

    2. Utilize a sliding fee scale for child care services provided under this sectionin order to support a high number of low-income parents pursuing

    postsecondary education at the institution.

    Supplemental Information

    INTRODUCTION: The following information supplements the informationprovided in the Dear Applicant letter and the Notice Inviting Applications for NewAwards (Notice).

    A. Criteria for Funding

    All applications for grants under the CCAMPIS Program will be evaluatedusing the selection criteria provided in this application package. Pleasenote that for FY 2005, we have established a competitive priority, which isincluded in the Notice and in this application package.

    B. Length of New Awards

    Applications for new awards may apply for a maximum of four years offunding.

    C. Evaluation of Applications for Awards

    Each application will be reviewed by a panel of experts. Each reviewer willprepare a written evaluation of the application and assign points for eachselection criterion.

    D. Selection of Grantees

    The Secretary will select an application for funding in rank order, based onthe applications total score for the selection criteria plus any competitivepriority points received.

    E. Applicant funding

    The Department is often unable to award the full amount of fundsrequested. The amount of funds awarded will be based on the availability

    of funds and the need for the services described in the application. Pleasenote: There is a maximum funding amount for FY 2005, which is specifiedin the Notice.

    To Whom Does This Provision Apply?

    (If this program is a State-formula grant program, aState needs to provide this description only for

    projects or activities that it carries out with fundsreserved for State-level uses. In addition, localschool districts or other eligible applicants that applyto the State for funding need to provide thisdescription in their applications to the State forfunding. The State would be responsible for ensuringthat the school district or other local entity hassubmitted a sufficient section 427 statement asdescribed below.)

    What Does This Provision Require?

    Section 427 requires each applicant for funds (other

    than an individual person) to include in itsapplication a description of the steps the applicantproposes to take to ensure equitable access to, and participation in, its Federally-assisted program forstudents, teachers, and other program beneficiarieswith special needs. This provision allows applicantsdiscretion in developing the required description.The statute highlights six types of barriers that canimpede equitable access or participation: gender,race, national origin, color, disability, or age. Basedon local circumstances, you should determinewhether these or other barriers may prevent yourstudents, teachers, etc. from such access or participation in, the Federally-funded project oractivity. The description in your application of stepsto be taken to overcome these barriers need not belengthy; you may provide a clear and succinct

    description of how you plan to address those barriersthat are applicable to your circumstances. Inaddition, the information may be provided in a singlenarrative, or, if appropriate, may be discussed inconnection with related topics in the application.

    Section 427 is not intended to duplicate the

    requirements of civil rights statutes, but rather toensure that, in designing their projects, applicants forFederal funds address equity concerns that may affectthe ability of certain potential beneficiaries to fully participate in the project and to achieve to highstandards. Consistent with program requirements andits approved application, an applicant may use theFederal funds awarded to it to eliminate barriers itidentifies.

    What are Examples of How an Applicant Might

    Satisfy the Requirement of This Provision?

    The following examples may help illustrate how anapplicant may comply with Section 427.

    (1) An applicant that proposes to carry out anadult literacy project serving, among others,adults with limited English proficiency, mightdescribe in its application how it intends todistribute a brochure about the proposed projectto such potential participants in their nativelanguage.

    (2) An applicant that proposes to developinstructional materials for classroom use might

    describe how it will make the materials availableon audio tape or in braille for students who areblind.

    (3) An applicant that proposes to carry out amodel science program for secondary studentsand is concerned that girls may be less likelythan boys to enroll in the course, might indicatehow it intends to conduct "outreach" efforts togirls, to encourage their enrollment.

    We recognize that many applicants may already beimplementing effective steps to ensure equity ofaccess and participation in their grant programs, andwe appreciate your cooperation in responding to therequirements of this provision.

    Information on theGovernment PerformanceAnd Results Act (GPRA)

    What is GPRA?

    The Government Performance and Results Act of 1993 is a straightforward statute thatrequires all federal agencies to manage their activities with attention to the consequencesof those activities. Each agency is to clearly state what it intends to accomplish, identifythe resources required, and periodically report their progress to the Congress. In doing so,it is expected that the GPRA will contribute to improvements in accountability for theexpenditures of public funds, improve Congressional decision-making through moreobjective information on the effectiveness of federal programs, and promote a newgovernment focus on results, service delivery, and customer satisfaction.

    How has the Department of Education Responded to the GPRA Requirements?

    As required by GPRA, the Department of Education has prepared a strategic plan for 2002-2007. This plan reflects the Departments priorities and integrates them with its missionand program authorities and describes how the Department will work to improve educationfor all children and adults in the United States. The Departments goals, as listed in theplan, are:

    Definitions for Form ED 424

    Novice Applicant (See 34 CFR 75.225). For discretionary grantprograms under which the Secretary gives special consideration tonovice applications, a novice applicant means any applicant for agrant from ED that

    Has never received a grant or subgrant under the program

    from which it seeks funding;

    Has never been a member of a group application, submittedin accordance with 34 CFR 75.127-75.129, that received agrant under the program from which it seeks funding; and

    Has not had an active discretionary grant from the Federal

    government in the five years before the deadline date forapplications under the program. For the purposes of thisrequirement, a grant is active until the end of the grantsproject or funding period, including any extensions of thoseperiods that extend the grantees authority to obligate funds.

    In the case of a group application submitted in accordance with 34CFR 75.127-75.129, a group includes only parties that meet the

    Type of Submission.     PROTECTION OF HUMAN SUBJECTS IN RESEARCH

    I. Definitions and Exemptions

    A. Definitions.

    A research activity involves human subjects if the activity isresearch, as defined in the Departments regulations, and the

    research activity will involve use of human subjects, as definethe regulations.

    Research

    The ED Regulations for the Protection of Human Subjects, TitleCode of Federal Regulations, Part 97, define research assystematic investigation, including research development, tesand evaluation, designed to develop or contribute to generalizaknowledge.     (3) Research involving the use of educational tests (cognitive,diagnostic, aptitude, achievement), survey procedures, interviewprocedures or observation of public behavior that is not exemptunder section (2) above, if the human subjects are elected orappointed public officials or candidates for public office; or federalstatute(s) require(s) without exception that the confidentiality of thepersonally identifiable information will be maintained throughoutthe research and thereafter.

    (4) Research involving the collection or study of existing data,documents, records, pathological specimens, or diagnosticspecimens, if these sources are publicly available or if theinformation is recorded by the investigator in a manner that subjects

    cannot be identified, directly or through identifiers linked to thesubjects.

    (5) Research and demonstration projects which are conducted by orsubject to the approval of department or agency heads, and which aredesigned to study, evaluate, or otherwise examine: (a) public benefitor service programs; (b) procedures for obtaining benefits or servicesunder those programs; (c) possible changes in or alternatives to thoseprograms or procedures; or (d) possible changes in methods or levelsof payment for benefits or services under those programs.

    (6) Taste and food quality evaluation and consumer acceptancestudies, (a) if wholesome foods without additives are consumed or

    (b) if a food is consumed that contains a food ingredient at or belowthe level and for a use found to be safe, or agricultural chemical orenvironmental contaminant at or below the level found to be safe, bythe Food and Drug Administration or approved by theEnvironmental Protection Agency or the Food Safety and InspectionService of the U.S. Department of Agriculture.

    II. Instructions for Exempt and Nonexempt Human Subjects

    Research Narratives

    If the applicant marked Yes for Item 13 on the ED 424, theapplicant must provide a human subjects exempt research ornonexempt research narrative and insert it immediately followingthe ED 424 face page.

    A. Exempt Research Narrative.

    If you marked Yes for item 13 a. and designated exemptionnumbers(s), provide the exempt research narrative. The narrativemust contain sufficient information about the involvement of humansubjects in the proposed research to allow a determination by EDthat the designated exemption(s) are appropriate. The narrative mustbe succinct.

    B. Nonexempt Research Narrative.

    If you marked No for item 13 a. you must provide the nonexeresearch narrative. The narrative must address the following sepoints. Although no specific page limitation applies to this secof the application, be succinct.

    (3) Recruitment and Informed Consent: Describe plans forrecruitment of subjects and the consent procedures to be followInclude the circumstances under which consent will be sought

    obtained, who will seek it, the nature of the information toprovided to prospective subjects, and the method of documenconsent. State if the Institutional Review Board (IRB) authorized a modification or waiver of the elements of consent orrequirement for documentation of consent.

    (4) Potential Risks: Describe potential risks (physpsychological, social, legal, or other) and assess their likelihoodseriousness. Where appropriate, describe alternative treatmentsprocedures that might be advantageous to the subjects.

    (5) Protection Against Risk: Describe the procedures for protecagainst or minimizing potential risks, including risks

    confidentiality, and assess their likely effectiveness. Wappropriate, discuss provisions for ensuring necessary medica professional intervention in the event of adverse effects tosubjects. Also, where appropriate, describe the provisions monitoring the data collected to ensure the safety of the subjects.

    (6) Importance of the Knowledge to be Gained: Discussimportance of the knowledge gained or to be gained as a result ofproposed research. Discuss why the risks to subjects are reasonin relation to the anticipated benefits to subjects and in relation toimportance of the knowledge that may reasonably be expecteresult.

    Budget Narrative Guidance

    Please use the Budget Summary Form (ED 524, Section A) to categorize requestedfunds.

    The Budget Narrative must include a comprehensive budget including a detailedbreakdown of costs within each budget category, and an explanation of the basis fordetermining the amounts needed for personnel, staff travel, workshop costs, materialspreparation, evaluation, and any other costs appropriate for the project.

    Please Note: The application Budget Narrative and Program Narrative, combined,cannot exceed 45 pages. The Notice contains specific instructions governing pagelimits.

    For this competition, applicants may request funding for four years (48 months). TheDepartment is requesting that you complete the Budget Summary Form (ED 524) forthe 2005-2006 year and provide a detailed Budget Narrative for that one year (12months) budget period only. It is not necessary to provide a budget summary for thetotal grant period requested, up to 4 years. The funding level for the first year isstipulated in the Notice, and the Department will determine the funding levels for eachadditional year of the grant award.

    The funding request may include all costs that are allowable, reasonable, andassociated with carrying out the objectives of the CCAMPIS program. Among the costs

    that may be supported with grant funds are:

    1. Personnel: On line 1 (ED 524), enter the project personnel salaries and wages.(Fees and expenses for consultants should be included on line 8.) The BudgetNarrative should note the total commitment of time and the total salary to becharged to the project for each key member of the project staff. You shouldprovide a breakdown of project personnel that includes the position titles, thepercent of time and number of months committed to the project for each key staffmember, the salary for each key staff member, and total salary costs to becharged to the grant.

    2. Fringe Benefits: On line 2 (ED 524), enter the amount of fringe benefits. Theinstitutions normal fringe benefit contribution may be charged to the program.Leave this blank if fringe benefits applicable to direct salaries and wages aretreated as part of the indirect costs.     3. Travel: On line 3 (ED 524), provide the costs for project personnel. Pleaseinclude the cost to travel to Washington, DC (for approximately 2 days) for aproject directors meeting. In the Budget Narrative, you should detail theproposed travel costs: for each trip explain the purpose and objectives of thetravel and provide the number of persons traveling. Transportation costs should

    not exceed tourist class airfare. For automobile mileage, the establishedinstitution rate should be used. Reimbursement is allowed for taxicab, bus, train,or limousine transportation. Per diem at the established institution rate ispermitted when an individual is away from home over night on official projectbusiness (see OMB Circular A-21, J.53 Travel costs). No foreign travel will beauthorized under the grant.

    4. Equipment: On line 4 (ED 524), indicate the cost of equipment - -non-expendable personal property, which has a usefulness of greater than oneyear and an acquisition cost of $5,000 or more per unit. (Consistent with anapplicants policy, a lower dollar amount may be used to define equipment.) In

    the Budget Narrative, explain why the requested equipment is necessary to carryout project activities, and include a list of all equipment in the following format:item, quantity, cost per unit, and total cost.

    5. Supplies: On line 5 (ED 524), include the costs of all tangible personal propertythat was not included as equipment on line 4. In the Budget Narrative, providean itemized list of the supplies.

    6. Contractual: On line 6 (ED 524), include the costs if you are contracting out forday care/child care services. In the Budget Narrative explain why it is notfeasible to provide these services on campus. Provide the name of the centerthat will be providing the services.

    7. Construction: On line 7 (ED 524), include the costs for minor renovation orrepairs. Note: No funds shall be used for construction, except for minorrenovation or repair to meet applicable State or local health or safetyrequirements.

    8. Other: On line 8 (ED 524), indicate all direct costs not covered on lines 1 through7. The costs/fees for consultants and consultants travel should be includedhere.
