The major focus of The University of Melbourne Department of Surgery, Austin Health is the development and delivery of high quality surgical education and training. The Department provides a comprehensive surgical education program to both undergraduate and postgraduate students in parallel with the clinical surgical training delivered by the members of the Division of Surgery. In addition the department provides opportunities for scientific research in a wide variety of fields in a warm, friendly and collegial atmosphere. Four separate research groups with international repute provide excellent oppportunities for advancement in both basic science and clinical research. Researchers and students have easy access to supervisors and support and close supervision at all times. Modern facilities and easy access to transport ensure a pleasant working environment.

Welcome Message

We wish to welcome all Surgical Trainees together with all members of the Division of Surgery to the General Surgery Education Program for the year 2015. This program is aimed at providing continuing surgical education and training delivered through a series of tutorials, seminars, journal clubs and workshops. It also provides participation to all members of the Division of Surgery through the Surgical Forums and the Annual Surgical Unit Audits. The program has continued to evolve over the past 12 years and has reached its current level of excellence predominantly due to the commitment and dedication by the members of the Division of Surgery who have volunteered their valuable time for the benefit of our trainees.

The establishment of an Austin General Surgical Trainees Education Committee two years ago has allowed the development of leadership in education amongst trainees with their active participation driving changes to the program. Changes have continued to be incorporated in to the program to reduce the demand on trainees and surgeons while concentrating the teaching in to manageable segments. We have endeavoured to limit surgeon involvement to a maximum of one Saturday and one Tuesday for the majority of surgeons.

The Saturday Surgical Seminars held once a month on Saturday afternoons continue to be our flagship education module. Particular thanks to our colleaguse from The Northern Hospital whose involvement in the Saturday Seminars has complemented the sessions enourmously. The introduction of trainee debates took off to a slow start but with the changed strcture introduced mid year, appers to have become a worthwhile component.

A paradigm shift in our educational activities took place in 2014 with the delivery of two advanced surgical workshop with wet lab specimens. The first was on bowel anastomosis and the second on peptic ulcer disease. The success of these workshops were predominantly due to the involvement of our senior trainees and inductry support. In 2015 this has expanded to five such workshops at the Austin complemented by another four to be held at the RACS Skills Lab under the auspices of the Vic/Tas Regional Training Committee

We wish to emphasize that the program is open to anyone who is interested from the Austin and other centres, including all Surgical Trainees, Unaccredited Registrars, Surgical HMOs, Interns and International Medical Graduates. The success of this program is dependent on active participation from all Surgical Trainees and staff members.

Thank you to all the members of the Division, Surgeons from The Northern Hospital, Specialty Fellows and Surgical Trainees who participate in the surgical education program and we look forward to another successful year.

Surgical Education Program

INTRODUCTION: All Surgical Trainees are expected to participate in the Surgical Education Program. This program provides a continuous series of educational activities for trainees at all levels. Since its inception in 2003, the program has continued to evolve and at present encompasses a wide range of topics that is intended to provide a broad knowledge base.

SURGICAL PROGRAM CO-ORDINATORS: Two trainee representatives based at the Austin Hospital are chosen (usually SET4 trainees) to co-ordinate the Surgical Education Program. It will be the trainee representative’s responsibility to communicate with the relevant tutors in advance to confirm the sessions or make alternative arrangements if necessary.

ATTENDANCE: Attendance is MANDATORY for all SET & NSET trainees based at the Austin Hospital. All trainees including those on regional rotations are expected to attend the Saturday Surgical Seminars. Surgical HMOs, Interns and medical students are also strongly encouraged to participate.

ABSENCE: Trainees who are unable to attend a particular session are expected to inform the Surgical Trainee representative so that apologies may be noted. If trainees are unable to attend a session where they are expected to present, they must make alternate arrangements.

Information Sources

The Department of Surgery publishes a number of regular publications relating to surgical education, research and training. These are aimed at providing all relevant information to trainees and surgeons alike and encourage participation in the educational and training activities. They may be readily downloaded from the departmental web site or may be requested as hard copies from the departmental office.

Pathways to Discovery: This is the flagship publication for the research arm of the depart-ment. Austin Surgery Education Program: This is printed as an A5 sized 44 page full colour booklet for ease of storage. It contains all relevant details about the surgical education program, surgical rosters and details of departmental activities.

All trainees, surgical HMOs and surgeons are provided a hard copy of the booklet. The electronic version available on the departmental web site is continuously updated.

Austin Research Prize Book:This booklet is the official publication for the Annual Austin Surgery Research Prize. It is published with all the abstracts being submit-ted in that year along with the abstracts of the winners from pre-vious years. This booklet provides a good idea of the quality and breatth of research being presented by general and sub specialty trainees.

INVITED LECTURES (February – July)

This will occur in the first half of the year from the 19th of February to the 16th of July. External speakers have been invited by the department to deliver clinical updates on current topics.

SURGICAL UNIT ANNUAL AUDITS (July – November)

This will be conducted in the second half of the year from the 23rd July to the 26th November. Each unit within the Division of Surgery is allocated a single day for a presentation of their Annual Surgical Audit.

The Austin Surgery Annual Surgical Audits provide an opportunity for all surgical units to present their annual activity, trends, outcomes and benchmarking. The unit members are encouraged to present the figures for the unit activity and the general and specific morbidity and mortality rates for the previous 12 month period. The morbidity should preferably relate to both the operation and the indication if possible (criteria auditing). All deaths should be listed. Detailed discussion of complications and deaths should be avoided in general as they would have been dicussed at the weekly surgical audits unless events led to a change in practice.

Units are encouraged to deliver a 15 minute update on any subject within their subspecialty relating to advances in treatment, changes in the unit activity, new technologies or future directions.

It would be ideal if the heads of units would encourage other participating staff to attend (apart from the unit members) to their unit audits. This may include ward, theatre and anaesthetic nursing staff, as well as staff from other departments such as physiotherapy, nutrition, etc. In addition, members of similar units from other Melbourne hospitals may be invited and encouraged to attend. All units are kindly requested to supply an electronic version of the audit to the Department of Surgery archives. (See Kim McAughtry)

Content: The weekly audit meeting is attended by all units within the Division of Surgery.

Audit Content: The audits should display the weekly activity of the relevant unit, including operative, endoscopic and non operative cases. Morbidity and mortality should be listed with all UR numbers and relevant cases discussed in detail highlighting specific issues and any suggested corrective measures.

Audit Format: The surgical audit is presented electronically using the lectern computer and projector.

Audit Submission: Electronic format of the audit should be sent to Kim McAughtry at the Department of Surgery (T: 9496 5468; E: kimem@unimelb.edu.au) to be received latest by 1400hrs on the Wednesday preceding the Audit. These will be uploaded to the computer in the Lecture Theatre ready for presentation. Late submissions will NOT be uploaded.

Late comers: We ask those who are unable to email their audits to Kim, to kindly arrive 15 minutes early and upload your presentation on to the lectern comouter. Attempting to upload it during the audit presentations not only delays the session but is extremely inconsiderate towards the audience.

Multi Unit Involvement: We would suggest that if specific complications or management issues involve more than one unit then the presenting unit contact the other units prior to the audit to allow time for them to discover information. This would allow a more productive audit meeting as the issues will then likely not need to be revisited repeatedly.

Clinical Cases, Video Debriefing & Journal Clubs

Case Discussions (Fortnightly): This is an open format discussion with Andrew Bui and Prof Christophi. Trainees are welcome to present a medium case as practice or simply discuss topics of their choice.

Journal Club (Monthly): Journal clubs will be convened by the Journal Club Co-ordinator who will liaise with the Subspecialty Fellow or Surgeon to select one relevant journal article which will be assigned to the trainee denoted below. The trainee will present the journal article and the co-ordinator will present an article on the methodology of the main paper.

Doce et Disce (Monthly): Teach while you learn and learn to teach.

Video Presentation (15 mins): Assigned trainee will present a video of a procedure or a powerpoint description of one and discuss pitfalls and learning points.

Microteach Session (15 Mins): The assigned trainee for the microteach session will have 12 minutes to present any topic in anatomy, physiology, pathology, radiology, surgery etc in a way that they can teach their colleagues. Examples are anatomy of the appendix, RLN injury, CEA, Gallbladder Polyp etc. Both these are intended to develop trainees ability to teach whie learning themselves.

Case Discussion (30 mins): This is an open format discussion with the supervisor of surgical training.

Austin General Surgery Non-SET Registrar Selection & Interviews

Selection Process: Due to the high demand for NSET positions at Austin the selection process has been modified for the interviews in 2015 for entry in 2016. It consists of the following:

- Strctured curriculum vitae (40%)- Interview (40%)- Referees Report (10%)- Independant MWU Reference (10%)Details will be forthcoming at the Austin Web Portal.

Selection Interview: Please note that selection interviews will be held on ONLY two specific dates so denoted below: Dates: Wednesday 15 July 2015 & Tuesday 21 July 2015 Time: 1300 - 1730 Venue: Education precinct

Format: On each of the two specified days there will be three panels for interviews Each panel will consist of one surgeon, one SET trainee and one member of the MWU. The interview will consist of the following:- one general introductory question (10 points)- one situational management question (20 points)- one clinical management question (20 points)- rebuttal of CV (10 points)- global communications score (10 points)This results ina total of 80 points which resolves in to 40% of the total score.

Strctured curriculum vitae: Template for submitting the structured CV will be available from the Austin MWU web page along with scoring guidelines tobetter inform our trainees.

Independant MWU Reference: You will be scored on a number of non operative skills based on the nine core competencies described by the RACS. The score sheet will be filled out by a member of the relevant MWU of the last hopsital you worked in by direct or telephone interview.

Referees Reports: Standardized referees reports template will be available online.

Saturday Surgical Seminars

SATURDAY SURGICAL SEMINARS:

Each surgical seminar will be as described below:1300 - 1330 Lunch1330 – 1430: Operative Surgery Quiz by the panel1430 – 1500: Medical Updates for Surgeons 1500 – 1530: Case presentations & quiz by Surgical Fellows1530 - 1545: Tea Break1545 – 1615: Surgical Debate1615 – 1700: “How I do it!” by a selected member of the panel.

The aim is to provide to an interactive monthly session on a Saturday afternoon to enable trainees from metropolitan and regional as well as interstate rotations to participate.

Each Surgical Seminar will consist of the popular operative surgery quiz by the panel members. This will be followed by a consultant physician delivering a formal presentation on a very relevant non-surgical issue after which the surgical fellow relating to that subspecialty or one of the SET5 trainees would present a number of case studies related to the topic discussed.

A change from 2014 is the introduction of a debate as a learning tool. After a short break the debate will take place followed by a presentation (preferably video) by the chairperson.

Another change in 2014 is the inclusion of surgeons from The Northern Hospital who have kindly offered to participate in the panels. This has been an extremely successful venture.

All trainees except those posted interstate are expected to attend. Given the monthly frequency of the sessions, we expect trainees to be able to adjust their on call commitments in order to attend. Interstate trainees are strongly encouraged to attend where possible.

This session is open to all Victorian General Surgery Trainees, Surgical HMOs, Interns and Medical Students.

Saturday Surgical Seminars

DEBATES:

The topics for the debates are chosen from a list of topics supplied by the various heads of the General Surgery Units.

Captains: Two captains are identified on the program, one proponent and one opponent. It is the responsibiliyy of each captain to contact the chairman of the session and prepare the content for the debate.

Team members: In addition to the captains each team will consist of two other members who will be chosen at random on the day of the symposium during the lunch break. Obviously the team members will have no time to prepare. The captains will provide them with the necessary summary information for the debate.

Concept: The idea of the debate is for trainees to be able to speak under pressure with little preparation.

Format: Each speaker (six including the two captains) will speak for 3 minutes alternating between the proponent and opponent. The first speaker introduces the team and their debate topic along with some facts. The second speaker speaks on the body of factual information. The final speaker summarises the arguments. Each speaker also will counter the previous speakers’ argument.

Results: The panel will retire from the room to decide on the winner. The winner is based on the quality of the debate, the strength of the arguments and the style of debating rather than the factual details.

While the panel decides on the winner both teams will be asked to undertake a Q&A from the audience.

The aim is to provide interactive simulationa and training in selected areas. These will be repeated annually depending on the success and participation by trainees.

In 2014 two separate workshops were conducted in the Simulation Centre on level 7 HSB, Austin Health. The Bowel Anastomosis workshop and the Peptic Ulcer Workshop were a great success. Building on that success we now present five separate simulation workshops. In 2015 a room with swipe access is likely to become available to host four laparoscopic trainer boxes kindly donated by Applied Medical. Once established this will enable Austin tsurgical trainees to access and practice advanced laparoscopic skills at their own convenience in addition to formal workshops.

These workshops are the beginning of an expanded training portfolio initiated by the Vic/Tas Regional Training Committee (RTC) to improve training in a number of complex procedures. Depending on feedback and outcomes these are likely to expand in topics and numbers. Places will be limited but are available at no cost to the trainee. The Simulations Subcommittee chaired by Dr Kate Martin has commenced this effort with four specific simulation workshops supported by industry partners and the RACS Skills Centre.

Austin Surgery Research Prize

Eligibility Criteria: Trainees belonging to a training scheme within the Division of Surgery in ALL surgical specialties ( all SET and Non-SET registrars, Surgical HMOs (PGY2 & 3) as well as Advanced Trainees in Anaesthesia, Radiology, Emergency, Intensive Care & Pathology at Austin Health are invited to submit abstracts relating to original scientific research.

Exclusion Criteria: Anyone not on a surgical pathway, namely interns and medical students.

Presentation Format: Oral presentation judged by a panel of adjudicators.

Presentation Timings: Each candidate will be allocated a maximum of 15 minutes for their presentation. Candidates should aim to limit their talk to eight minutes after which there will be seven minutes of question time. A warning bell will be rung at eight and fourteen minutes in to their presentation and a final bell at fifteen minutes to mark the end.

Submission of abstracts: Submission will be through an interactive PDF document which will be made available in September 2015 by email and on the departmental web site.

Prizes:

The trainee with the best presentation as judged by the adjudicators will be awarded a gold medallion by the Johnson & Johnson representative at the end of the proceedings. The winners name will also be added to the Honour Board in the department of surgery. All presenters will be awarded certificates of attendance.

“Gratias Cena” - Austin General Surgery Trainees Dinner

This is an event hosted by the Austin surgical trainees in appreciation of the education and training given them by the surgeons at the Austin Hospital.

The initial event was held in 2006 and after a hiatus of two years recommenced in 2009. The Austin Trainees conducted a very successful dinner event in December 2010 at Collin’s Kitchen, Grand Hyatt which for the first time included surgeons from many of the attached training hubs. The event moved to “The Residence”, Grand Hyatt, Melbourne in 2011 and 2012 on a Saturday night which resulted on more than 70 attendees. In 2013, the trainees hosted the event at the courtyard and the Mezzanine level of the award winning restaurant “The Italian” and in 2014 at “The Boat House” in Moonee Ponds.

The event provides an informal setting for trainees and surgeons to meet. In particular trainees were able to meet in person some of their consultants for the following year. This event has progressively expanded both in scope and quality and is expected be continued perpetually by the Austin surgical trainees. It also serves as an important time for surgical valedictorians, particularly those who have successfully completed their training. Supervisors and surgeons from Austin and Northern hospitals as well as all other metropolitan, regional and interstate hospitals where our trainees rotate to are invited to attend.
