I am very pleased to present the Department of Anesthesiology’s fourth Annual Report, which highlights the exceptional care we provide to patients at HSS.
In 2013 we introduced the concept of the Perioperative Orthopedic Surgical Home (POSH) model of care, a coordinated approach that is patient-centered, physician-led, and multidisciplinary. The Department of Anesthesiology, through many preoperative, intraoperative and postoperative initiatives, plays a vital role in this multidisciplinary model, with the primary goal of providing the highest quality surgical experience.
Some of the Department’s perioperative initiatives, including the role of regional anesthetics, acute pain management and orthopedic critical care, are discussed at length in this report.
We have also highlighted some of the less traditional but equally important initiatives that have taken place in our Department over the past year. The roles of focused transthoracic echocardiography, bioethics in anesthesia and
perioperative medicine, and our global outreach initiatives set this Department apart from many others that simply provide basic clinical care. This differentiation is vital in maintaining HSS’s position as the global standard in orthopedics.
In addition to clinical accomplishments, our physicians continue to publish ground-breaking research, lecture locally, nationally and internationally, and educate the next generation of regional and orthopedic anesthesiologists. I hope you enjoy reading about these and other achievements of the Department of Anesthesiology in our 2013 Annual Report.

ANESTHESIOLOGY AT HSS: SPECIALIZING IN REGIONAL TECHNIQUES
The Department of Anesthesiology at Hospital for Special Surgery (HSS) prides itself on providing exceptional specialty care for an exceptional specialty hospital. Recognized as the premier physicians in the world for the practice of regional anesthesia and pain medicine for orthopedics, 85 percent of the 41,441 anesthetics performed by HSS anesthesiologists in 2013 involved the application of regional anesthesia (RA) techniques.
Ideal for joint replacement procedures, RA is also a key contributor to the Hospital’s low rates of infection, complications and mortality. Its use significantly shortens surgical time, and patients benefit from decreased postoperative pain, less nausea and fewer side effects. Rates of deep vein thrombosis and pulmonary embolism are lower than with general anesthesia, and
use of RA results in a markedly reduced risk of surgical infection. The Hospital’s low infection rates were acknowledged in New York State’s Hospital Acquired Infection 2012 Report, which was published in December 2013. The report confirmed that HSS’ risk-adjusted surgical site infection rate is less than half of the state average and the Hospital’s hip infection rate is less than half of what would be expected for our particular mix of patients.
The Department continually supports research into how RA affects orthopedic outcomes. In 2013, Stavros Memtsoudis, MD, PhD, and a team of investigators studied the impact of anesthesia technique in patients undergoing total knee and hip replacement surgeries by analyzing national data from approximately 400,000
patients undergoing surgery at 400 hospitals. The goal of the study was to prove Dr. Memtsoudis’ theory that regional anesthesia favorably influences perioperative outcomes. Postoperative complications, 30-day mortality, length of stay, and patient cost were analyzed and compared based on anesthesia technique: regional, general, and a combination of the two.
Dr. Memtsoudis published a landmark article after successfully proving that the utilization of regional anesthesia for primary joint arthroplasty is associated with superior perioperative outcomes. The study confirmed that patients who received regional anesthesia had statistically significant decreases in 30-day mortality, length of hospital stay and in-hospital complications, including fewer infections.

AN INTEGRATED APPROACH TO PATIENT CARE
Anesthesiologists at HSS do much more than provide anesthesia in operating rooms; they are also responsible for many other patient care services integral to the entire perioperative experience.
The Perioperative Surgical Home (PSH) model of care, a patient-centered, physician-led multidisciplinary system of coordinated care that guides a patient throughout the entire surgical experience, is currently being developed by the American Society of Anesthesiologists. The purpose of the integrated care provided via the PSH model is to create a better, more cohesive patient experience while making surgical care safer and more efficient. Similarly, HSS provides a Perioperative Orthopedic Surgical Home (POSH) model through multidisciplinary coordination between the Departments of
Anesthesiology, Medicine, Surgery, Nursing and other ancillary services.
The Hospital’s pre-surgical screening process is tailored specifically to a patient’s history and complexity. Patients may receive pre-surgical consultation by a physician in the Department of Medicine, and depending on complexity of the surgery, medical history and past experience with anesthesia, the patient may also receive an anesthesiology consultation. During the anesthesia consultation the anesthesiologist evaluates past medical history and assesses the patient’s anesthetic options. Furthermore, patients with comorbidities or a complex medical history may be referred to the Complex Case Committee for review prior to surgery. The committee, chaired by Michael Urban, MD, PhD, consists of a multidisciplinary group

of physicians and other caregivers which reviews and evaluates high acuity patients, ensuring the highest level of specialized care.
On the day of surgery, an anesthesiologist meets each patient in the holding area, where he or she explains the anesthetic procedure and answers any questions the patient may have. In the operating room, the anesthesiologist is responsible for ensuring the patient’s safety and comfort during surgery.
In the Hospital’s Post-Anesthesia Care Unit, Step-Down Unit and Orthopedic Special Care Unit, board-certified critical care anesthesiologists provide 24-hour, seven-day-a-week coverage and leadership. These double board-certified physicians provide a level of care unprecedented in orthopedic centers around the country.
The intensivists care for patients as they emerge from anesthesia and supervise a multidisciplinary team to ensure safe recovery and proper pain control. Patients are then discharged or transferred to an inpatient room for further recuperation and rehabilitation.
Anesthesiologists and nurses on the Acute Pain Service round on all inpatient units to examine and treat pain issues. This service helps to ensure that pain does not interfere with a patient’s physical therapy so that their path to recovery is smooth. In addition, an anesthesiologist is always available on call to respond to urgent medical matters, address critical concerns and care for patients
who experience non-routine pain or other discomfort
postoperatively. At HSS, patients benefit from the highest quality of specialized pain management care, as evidenced by the Hospital’s 99th percentile ranking in Press Ganey patient satisfaction survey inpatient pain control scores.
Management of non-routine post-surgical pain is extended through the innovative, attending-directed Recuperative Pain Service, which provides short-to-mid term (eight week) outpatient pain management treatment.
Additionally, the Department’s seven board-certified pain management
physicians treat and manage complex conditions, treat pain due to injury or illness, and provide longer-term care for those with chronic pain issues.
Through the POSH model at HSS, anesthesiologists provide leadership and participate in a coordinated team which manages patients from preoperative assessment through the post-discharge period. This makes surgical care safer and more aligned, while promoting better medical outcomes at a lower cost. Enhanced coordination in the management of surgical patients has been proven to increase quality, reduce complications and improve the patient’s perception of the surgical experience.

FOCUS ASSESSED TRANSTHORACIC ECHOCARDIOGRAPHY FIRST COURSE IN NORTH AMERICA
Transthoracic echocardiography (TTE) is a tool traditionally used by cardiologists to answer complex clinical questions about a patient’s cardiovascular health. However, few physicians without formal training in cardiology utilize a focused TTE exam for its other potential clinical benefits. Focused TTE is becoming an important tool for anesthesiologists and critical care physicians in the periop-erative setting when used to supple-ment clinical evaluation and optimize cardiopulmonary resuscitation. There is growing awareness of the benefits of focused TTE among anesthesiologists and intensivists, but there are few op-portunities available to adequately train physicians to incorporate this tool into their daily practice.
The Focus Assessed Transthoracic Echocardiography (FATE) exam was es-tablished in 1989 and has been taught via training courses since 1996. The FATE protocol training course teaches physicians how to perform a focused TTE exam and then utilize the informa-tion to improve patient management and care. Prior to the course, partici-pants complete an online e-learning module to prepare for hands-on training in the FATE exam. Although FATE originated in Denmark, the FATE train-ing courses have expanded dramati-
cally establishing a strong international presence throughout Europe, India, and South Africa.
In March 2013, the Anesthesiology Department partnered with USabcd.org to host the first official FATE course in North America. Stephen Haskins, MD, coordinated the program and taught the course under the supervision of FATE protocol originator, Professor Erik Sloth, MD, PhD. Twenty HSS attending anesthesiologists participated in the course, learning to perform a focused cardiac echo exam to supplement standard clinical evaluation and to guide cardiopulmonary optimization. Subsequent to the March course, at the invitation of Professor Sloth, Dr. Haskins traveled to Denmark to receive further training and was designated as the first clinical instructor of the USabcd.org FATE Course in the United States.
In July, Dr. Haskins, assisted by Jemiel Nejim, MD, Sean Garvin, MD, and Stephen Harris, MD, conducted a second FATE Course at HSS. In January 2014, HSS held a FATE protocol course open to non-HSS physicians to share this important skill with doctors outside the HSS community. This was the first open FATE course in North America.

ORTHOPEDIC SPECIAL CARE UNIT
In October 2013 the Orthopedic Special Care Unit (OSCU), a one-of-a-kind, dedicated intensive care unit, opened on the fifth floor of the Hospital. This four-bed unit was designed to specifically care for high acuity patients with complex medi-cal needs. Under the leadership of Stavros Memtsoudis, MD, PhD, and Michael Nurok, MBChB, PhD, a specially trained group of critical care anesthesiologists and a multidis-ciplinary team of nurses, nutritionists, physical and respiratory therapists,
pharmacists, and patient advocates work closely together to provide intensive patient care.
Most patients are admitted to the OSCU because of the complexity of their operation and the need for mechanical ventilation. Occasion-ally, the OSCU is used for patients with surgical complications or for advanced monitor-ing of patients with complex medical
conditions that existed prior to their operation, such as pulmonary hyper-tension, obstructive sleep apnea, or coronary artery disease.
The OSCU, in conjunction with NewYork-Presbyterian Hospital’s spe-cialty ICUs, differentiates HSS from other hospitals performing orthopedic surgery. The unit allows us to safely undertake increasingly complex and
challenging cases, and helps the Hospital maintain a leader-
ship role in advancing the care of the complex orthopedic patient into the 21st century.

ANESTHESIOLOGY AND BIOETHICS
Advances in medicine are often accompanied by social and ethical challenges. Although anesthesiology and ethics are not always readily as-sociated, as facilitators of perioperative surgical services, anesthesiologists are confronted with difficult patient care decisions that go beyond periopera-tive evaluation, selection of anesthetic technique and postoperative care. The Department of Anesthesiology addresses these ethical challenges through multiple clinical, educational and research initiatives.
In 2013, Douglas Green, MD, Michael Nurok, MB ChB, PhD, and Gregory Liguori, MD, were part of a team that revised the Hospital’s Do Not Resuscitate (DNR) policies, ensur-ing alignment with New York State Law and national society guidelines on patient options for DNR in the operating room. In addition, they are currently developing a policy for situa-tions where requested care may not be
clinically appropriate. HSS anesthesiol-ogists serve on a variety of committees to promote ethics in medicine:
Mary Chisholm, MD, serves as a member of the HSS Institutional Review Board. The Board oversees the treatment of human subjects and brings a focus on ethics to the com-mittee’s activities. Dr. Nurok serves as an alternate member.
Dr. Chisholm and Dr. Nurok are mem-bers of the HSS Ethics Committee, and Dr. Green serves as Co-Chair. Committee members foster an inter-disciplinary approach to clinical ethics and convene regularly to review current research and approaches. The committee supports clinicians, patients and family members who are facing ethical challenges, and mem-bers provide consultations to assist in the resolution of ethical issues.

American Society of Anesthesi-ologists Committee on Ethics and the Society of Critical Care Medi-cine’s Quality and Safety Commit-tee. In 2013, he gave a number of invited lectures around the country on ethics-related topics.
To prepare medical students for challenging ethical questions they may face in their practice, Weill Cornell Medical College offers an annual course which introduces second year medical students to issues and methods in clinical ethics. Dr. Chisholm is a member of the diverse faculty, which includes members from Medicine, Surgery, Anesthesiology, Nursing, Public Health, Geriatrics, Pediatrics, Obstetrics and Gynecology, Pastoral Care, Philosophy and Health Law – all with expertise in medical ethics. Lecture topics in the eight-week course include the rise
of medical ethics in the 20th Century, informed consent, decision-making at the end of life, reproductive medicine, and ethics in pediatrics and surgery.
Following each lecture, Dr. Chisholm facilitates small group discussions where students, prepared through assigned readings and a case-based
written application of the issue, review the topic of the week.
She is also a member of the selection committee for the student prize the-sis in ethics. According to Dr. Chisholm, “inter-acting with the students,
sharing difficult situations that do not have clear-cut
solutions and helping them think clearly about the issues make this a rewarding teaching experience.”
The Department’s principal investiga-tors, in conjunction with members of the Research Division, have published several manuscripts in 2013 as a result of our physician’s work in the field of ethics.

GLOBAL INTERNATIONAL OUTREACH
Anesthesiologists at HSS, as leaders in the field of orthopedic anesthesia, are committed to reducing disparities in healthcare and narrowing the burden of surgical and anesthetic disease in low and middle-income countries. Most of these countries have a critical shortage of qualified surgical and anesthesia providers, causing a healthcare workforce crisis.
To help alleviate this, HSS anesthesiolo-gists take an active role volunteering in surgical missions around the world, including Dominican Republic, Ecuador, Ghana, India and Haiti. They are mak-ing strides within global international outreach through their participation in these missions, which provide surgery to those who would otherwise be un-able to afford them. Over the course of
one week, teams are able to perform an average of 50 operations, and many of these surgeries prove to be life-changing for patients.
While short-term surgical missions are impactful, the effect is often finite. As a result, the Depart-ment is fostering a new focus on long-term collaborations that provide a greater impact by teaching anesthesia providers modern and safe practices, with a particular focus on regional anesthesia for orthopedic sur-gery. In 2013, the Department began a collaboration with the Hospital for Traumatology and Orthopedics (HTO) in Ho Chi Min City, Vietnam. This 440 bed
orthopedic hospital, serves as a major teaching institution and referral center for orthopedics. As a government hos-pital, HTO treats patients who cannot afford private healthcare, a majority of whom are orthopedic trauma patients who require surgery.
Through this global collabora-tion, HSS anesthesiologists
visit HTO twice each year to teach regional an-esthesia techniques to their anesthesia provid-ers, helping to improve
outcomes, safety and postoperative pain analge-
sia while decreasing the need for expensive equipment. Ulti-
mately, the goal of the collaboration is to strengthen the anesthesia workforce in Vietnam and provide an improved standard of anesthetic care. Sustain-able development through teaching is the key to empowering local communi-ties and providing lasting change.

1–5: In June 2013, the Department again proudly hosted HSS participation in the JPMorgan Corporate Challenge, a 3.5 mile race benefiting the Central Park Conservancy. More than 400 HSS staff members participated in a night filled with fun, camaraderie and team spirit.
7: At the Department’s bi-annual retreat, 50 staff members and phy-sicians participated in an afternoon of teambuilding, and gained insight
about themselves and their co-workers by completing the Meyers-
Briggs personality assessment.
8–9: Many of the Department’s research and administrative staff members attended the American Society of Anesthesiologists’ An-
nual Meeting, held in San Francis-co in October 2013. Six physicians
and staff members participated in a pre-conference 5K race, and
Research Assistant Marcus DiLallo came in second place with a time
of 17.17. Marcus also finished the 2013 New York City Marathon
with a time of 2:55:37.

STEPHEN C. HASKINS, MD Assistant Attending Anesthesiologist
Dr. Haskins is a graduate of New
York University School of Medicine. He completed residency training at NewYork Presbyterian-Weill Cornell Medical Col-lege, where he served as Chief Resident and received the Ronald A. Leff Award for Excellence in Clinical Teaching by a Resident. He is a graduate of HSS’ regional anesthesia and acute pain medi-cine fellowship, where he received the Attending Anesthesiologists’ Award for Clinical Excellence. Dr. Haskins is board-certified in anesthesiology with addi-tional certifications in Basic Perioperative Transesophageal Echocardiography and Focus Assessed Transthoracic Echocar-diography (FATE).

A graduate of Weill-Cornell/Rockefeller/Sloan-Kettering’s Tri-Institutional MD-PhD Program, Dr. Kirksey completed an anesthesiology
residency at NewYork-Presbyterian Hospital-Weill Cornell Medical College and a critical care anesthesiology fellow-ship at NYPH-Columbia University. She received the Resident Research Award from Weill Cornell Medical College and the Award for Clinical Excellence during her HSS residency rotation. Dr. Kirksey is double board certified in anesthesiology and critical care.

Prior to joining the HSS medical staff,
Dr. Kumar served as Director of Regional Anesthesia at Johns Hopkins Bayview Medical Center. She received her medical degree at Albert Einstein College of Medi-cine and completed residency training in anesthesiology and critical care medi-cine at Johns Hopkins University. Board certified in anesthesiology, Dr. Kumar is a graduate of HSS’ Fellowship in Regional Anesthesia and Pain Medicine.

Cephas Swamidoss, MD, was chosen as the 2013 HSS Teacher of the Year by the anesthesiology residents of Weill Cornell Medical College and was also awarded Teacher of the Year by the Anesthesiology Department’s graduating fellows. This is the first time a physician has won both awards in the same year.

About Hospital for Special SurgeryFounded in 1863, Hospital for Special Surgery (HSS) is a world leader in orthopaedics, rheumatology and rehabilitation. HSS is nationally ranked No. 1 in orthopaedics, No. 4 in rheumatology, and No. 5 in geriatrics by U.S.News & World Report (2013-14), and is the first hospital in New York State to receive Magnet Recognition for Excellence in Nursing Service from the American Nurses Credentialing Center three consecutive times. Located in New York City, HSS also serves patients in the regional area with physician offices in Greenwich, Long Island, and Queens, and serves Florida patients with an outpatient rehabilitation office in South Florida.
