Dental plaque is a thin, invisible layer forming on the surface of teeth and gums every day, which may be removed by thorough and regular tooth brushing.

Lack of appropriate tooth brushing will result in tooth decay (caries) and the inflammation of the gum (gingivitis).

Through interaction with the minerals present in saliva, dental plaque calcifies and hardens, developing into a calculus (or tartar), which is a yellowish or brownish deposit.

Calculus can only be removed by a dentist or dental hygienist, and entails the danger of new layers of dental plaque containing bacteria developing on its surface. When enlarged, the calculus separates the gum from the dental neck, which causes the weakening and then a disease of the supporting structure of the teeth. The initial symptoms of the process include bleeding of the gums (ulemorrhagia), bad breath (halitosis) or pain and, in more severe cases, tooth movement, receding gums and pus formation in the pockets between teeth and gums.

Good oral hygiene includes tooth brushing, flossing and using mouth washes.

Proper dental health is achieved by removing the calculus with an ultrasonic scaler. If the surface of the tooth is smooth and clean, calculus is unable to form and thus by maintaining good oral hygiene we can prevent the reappearance of calculus.

The ultrasonic scaler does not damage tooth enamel. Its tip makes vibrating movements, thus removing the calculus from the surface of the tooth without damaging the enamel. As a result of sound pulse absorbed by the vibrating tip air bubbles develop, which break the calculus up.

Cleaning

Tooth polishingUltrasonic scaling is always followed by a special tooth polishing using Profiflex, an instrument similar to a sandblaster but emitting (instead of sand) an extremely fine bi-carbonate powder mixed with water at high pressure on the surface of teeth. The polishing powder removes debris, stains and plaque without harming the surface of the tooth. Polishing may be applied as a complementary treatment after calculus removal but is also useful for cleaning the teeth prior to bleaching or pit and fissure sealing.

Root canal treatment is often one the most challenging dentistry tasks. It requires patience, precision, sophisticated instruments and plenty of time. At our clinic the highly qualified dentists have all devices at their disposal, this makes it possible to carry out even the most complicated root canal treatment safely and successfully.

The exact position of the apex of the root is determined by an electric root canal measuring device and the latest nickel-titanium file systems make it possible to access and fill curved canals with the highest precision. In this way we avoid making several X-ray pictures, reduce the exposure of the patients to radiation and prepare precise root filling expanding along the full length of the root canal.

Today ozone is usually associated with environmental pollution and its beneficial effects are not too often mentioned

What is this material like? It is a light blue gas with a distinct smell, 48 times heavier than air.

Ozone is one of the strongest oxidising agents, which is why it is used in industry and healthcare. The strong oxidative effect damages the cell wall of bacteria and thus kills them. It also breaks the protein coat of viruses and modifies their DNA. It breaks down the acids harmful to teeth into water and carbon dioxide.

Ozone treatment does not require injection, drilling or filling. It is carried out using an Ozonitron device, which is an ozone generator based on electric discharge. It produces ozone from the oxygen present in the air in the gap between the glass electrode filled with a noble gas and the surface of the body. The shape of the electrode is selected to fit the purpose. The ozone is produced and acts right at the tip of the electrodes, i.e. it is generated at the surface to be treated. It kills germs efficiently in a few seconds.

This treatment is an excellent accompaniment to e.g. filling without drilling, since it sterilises the cavity before filling. But it is also highly effective in other areas of dentistry: treating mouth mucosa disorders, thrush, coldsores, sterilising root canals, anastalsis, reducing sensitivity at the dental neck, gum disease, pit and fissure sealing etc.

Pit and fissure sealing serves the purpose of providing a protective coating for dental surfaces prone to decay and in this way helps to avoid the development or spread of caries. The biting surfaces of the back teeth have pits and tiny grooves, in which food and bacteria may build up and cause cavities. In the course of pit and fissure sealing a liquid sealant is applied to the biting surface of the tooth, allowed to fill the grooves and set hard.

The back teeth of children (both primary and permanent teeth) may be sealed. In cases of children prone to tooth decay it is also recommended to protect all permanent back teeth in this way. If the biting surface of the back teeth in the mouth of your child is brown, black or of a different colour to the other teeth but not yet decayed, it is advisable to consider this treatment since it is fast, completely painless, does not require drilling and preparation of the tooth and is cheaper than a filling.

In the course of home whitening, the bleaching agent is applied with the help of well-fitted application trays, these are worn at nights for a duration recommended by the dentist. The carbamide peroxide contains easy-to-use Opalescence bleaching gel and we recommend is delivered in a syringe and is available in several flavours. It provides results after a few days of usage. A veneer is a thin layer of porcelain made to fit over the outer surface of front teeth in order to improve their appearance with minimum loss of the tooth’s own material.

Only the surface layer of the enamel is removed on the outer, visible surface of the tooth and the veneer is bonded in place with a special adhesive.

They may be made of filling materials (direct veneers) or of full porcelain (Empress, Procera). They are excellent for correcting discoloration, enamel development disorders, chipping or abrasion. They are suitable for modifying the length of a tooth or reducing or closing gaps between teeth.

As opposed to applying a crown, veneers require removing only a tiny amount of the enamel and very little preparation of the tooth is needed.

The most advanced technology for restoring the aesthetics and function of teeth at present is CAD/CAM. We take an impression (mould) of the teeth, digitise it with the help of a 3D laser scanning device and send the information to Sweden via the Internet, where the zirconia or aluminium oxide framework (Procera, Nobel Biocare) of the inlays is prepared by computer controlled machines without the touch of human hands and excluding the possibility of errors.

The framework of inlays prepared by robots has a precision of 5 microns, while an average good traditional metal-ceramic crown has a gap of 50-80 µm at the edge. Its material is entirely tissue-friendly and is highly resistant. Due to its white colour it lends itself for beautiful, natural-looking crowns/bridges. Its precision is unparalleled because of the computer-controlled manufacturing, which prevents secondary decay at the edges of the crown.

The other type of metal-free crowns belongs to pressed ceramics (IPS Empress). They provide an entirely natural looking and extremely superb aesthetics solution for the cosmetic correction of front teeth, be it a crown, veneer or inlay.

An inlay is a filling made by dental technicians after taking an impression. It is applied in cases when the part of the tooth missing is too large for using a filling. Inlays are more attractive and more durable than fillings and are able to restore the biting surface of the teeth as well as the adjoining surfaces of teeth next to each other (which cannot be properly restored using large fillings and therefore some food may jam between the teeth), which is also important for the temporo-mandibular joints.

They may be made of Gradia (plastic), traditional ceramic and pressed ceramics (IPS Empress). Ceramic inlays are extremely nice looking. Because of the use of a special dental adhesive, the inlay forms an organic whole with the tooth and protects and supports the remaining tooth tissue.

After a thorough cleaning of the surface to be treated, we identify the extent of the decay using a laser caries detector developed for this purpose (DIAGNOdent).

If the results are above a certain threshold, compressed air and fine aluminium oxide powder are blown by a tiny air abrasion device (RONDOflex) on the surface to be treated, which only wears off decayed, loose tooth tissues and does not damage healthy tooth tissues. The treatment is completely painless.

Tooth jewels are tiny shiny precious and semi-precious stones or crystals, (Swarovski crystals www.strass.com) the combination of gold, silver, white gold or another precious metal and a gem either in a figurative or non-figurative form.

Recently it has become fashionable again – especially among young people – to wear various tooth jewels to embellish their teeth. They can only be fixed by a dentist, using a special gluing technique.

The jewels can be attached to both real and replacement teeth. The materials used for gluing are not harmful to the teeth and do not damage the enamel. Although its durability varies person by person, it usually stays in place for years.

Implants are the latest procedure available for replacing lost teeth, using organic ingredients – they are safe, precise and anti-allergenic.

By using implants it is possible to replace any number of teeth with fixed replacement (crowns, bridges) or removable dentures.

The jaws will be functional again, the face will be plumped up and regain its youthful and healthy shape.

Implants are placed under local anaesthetic.

The replacement (crown, bridge or denture) is fitted following a healing period after the operation.

Prior to surgery, X-ray images and a 3D CT scan will be obtained in order to identify the implantation technique or the number of implants needed as well as the need for bone grafting.

There are cases when there is not enough bone and therefore implants can only be placed after augmenting the bone.

For placing an implant, at least a bone of 6 mm width is needed: in case of a thinner bone, bone augmentation is necessary. Bone augmentation is carried out by using bone substitutes, which are sterile, safe and tissue-friendly, or the patient’s own bone.

Another requirement is “bone height”, this needs to be at least 8-10 mm. In case of back teeth, the closeness of the oral cavity (sinus maxillaris) makes it more difficult to place implants, and in case of an unfavourable anatomy, a sinus lift operation is required. In this case the bone is augmented by lifting the mucosa of the oral cavity. It is a well-tested solution for increasing the height of the bone.

The bone augmentation may be carried out at the same time as placing the implant, or in a very bad case the augmentation is a first step followed by a healing period of 6-8 months, after which the implant is placed.

All parents would like it if their children have not only nice looking but also properly fitting teeth, a proportionate and harmonic face and a beautiful smile. It is important to have your children checked by an orthodontist at the age of 6-8 because a lot of occlusion problems can already be detected at this age, which may be excellently corrected using removable braces.

Misalignment of the teeth may cause functional, aesthetic and psychological problems. Only an even and complete dentition makes it possible to clean your teeth easily and have uncomplicated oral hygiene. In case of crowded teeth, it is easier for dental plaque to build up causing decay and gum disease. An ill-fitting set of teeth puts extra pressure on certain teeth and may cause jaw joint problems.

Orthodontics do not have an upper age limit, although your age will also have an influence on the procedure of the treatment. Due to new methods (fixed braces bonded to the outer or inner surface of the teeth) it is possible to have your teeth aligned and appearance improved even at the age of 50, 60 or 70.

Removable appliance• At the age of 4: if there is an occlusion (biting)

problem detected in the baby teeth, it may be corrected after the age of 4 in order to prevent speech disorders.

• Until the age of 6-12 the conditions for orthodontics are favourable as there is fast growth going on. It is easy to treat nearly all disorders at this age. It is enough to take your child to the first check-up at this age (aged 7) unless you see a conspicuous misalignment of the teeth (open bite, underbite). Removable appliances are usually worn by children. Children easily get used to the appliance, which may be customised in a playful way by using colourful patterns and glitter powder. It is absolutely recommended to wear them during the night but also in peaceful periods like writing homework, watching TV or reading. The more often they are worn the more effective they are. Fixed appliances are often also needed after wearing removable appliances. It is because removable appliances are not suitable for finetuning the position of teeth, thus for a perfect result, we also recommend a fixed appliance.

• Teenage is mainly suitable for wearing fixed braces. At this age it also possible to influence the growth and position of the jaws to some extent.

• In adulthood fixed appliances can be highly effective, since the teeth may be moved in the jaws. The appliance actually works on the bones through the teeth. The bone is dissolved under pressure and grows under tension, and so the tooth moves in the direction of the pressure. This mechanism works whether the patient is 20 or 50 years old.

Damon 3As opposed to traditional appliances, the correction of misalignment requires less time (14-18 months) and it is achieved with gentle forces. Damon 3 is more comfortable to wear, causes very little inconvenience and reduces the number of visits to the dentist needed: it is enough to adjust the appliance every 8-10 weeks.
