• A problem called varicocele (VAIR-ih-koh-seel). This happens when the veins on a man's testicle(s) are too large. This heats the testicles. The heat can affect the number or shape of the sperm.

• Other factors that cause a man to make too few sperm or none at all.

• Movement of the sperm. This may be caused by the shape of the sperm. Sometimes injuries or other damage to the reproductive system block the sperm.

• Testicular extraction of sperm (TESE) If a man's semen completely lacks sperm in the ejaculate (azoospermia), sperm can

sometimes be directly removed from the testes. This is done in a minor surgery or by using a needle to aspirate semen under local anesthesia. If sperm can be found and extracted from the testis, the sperm will be used for ICSI and the fertilization rate of the oocyte is not very different from IVF. Thus, men with no sperm in the ejaculate can have a potential of fathering a child using these techniques.

• Microepididymal sperm aspiration (MESA) Microepididymal sperm aspiration (MESA) is a procedure performed for men who have vasal

or epididymal obstruction (typically from a vasectomy or absence of the vas deferens). It is either done as a scheduled procedure or is coordinated with their female partner’s egg retrieval. MESA is performed in the operating room with general anesthesia utilizing the operating microscope. Patients usually cryopreserve (freeze) sperm during this procedure for future in vitro fertilization procedures. MESA allows for an extensive collection of sperm as compared to aspiration techniques.
