1. Coverage & Effect

This rider (the “Rider”) is hereby deemed a part of the agreement referred to above (the “Agreement”), and is incorporated into the Agreement by reference therein. It is understood and agreed that the Agreement and the Rider may not be changed, modified or altered except by an instrument in writing, signed by both Producer and Promoter. It is expressly agreed that in the event of any conflict or inconsistency between the terms of the Agreement and the terms of this Rider, the terms of this Rider shall prevail and shall govern interpretation of the Agreement.

2. Payment

(a) Upon execution of the Agreement, Promoter shall remit, a deposit for fifty percent (50%) of the total compensation set forth in the Agreement (excluding percentages) in the form of a DIRECT BANK WIRE ONLY as follows:

(b) All remaining monies due or payable, shall be paid to the Artist (in her capacity as representative for DDB Productions, Inc.) in DIRECT BANK WIRE ONLY made payable to DDB Productions, Inc. 72 Hours prior to the commencement of Artist's performance (the “Pre-show Payment”). Payment for any percentage overage owed to Producer for tickets sold subsequent to calculation of the Pre-show Payment shall be made in the same manner the following business day of each Performance.

(c) The parties hereto agree that it is of the essence and fundamental to this Agreement that all monies due to Producer under this Agreement be paid within the time specified in this Agreement, in the manner specified in this Agreement, and without any deduction or diminution whatsoever, including but not limited to deduction or diminution based on claims arising under this Agreement, any other agreement between the parties hereto, or any other agreement between either of the parties hereto and any other person or entity. Any deviation from or failure to adhere to the afore-prescribed terms shall constitute a material breach of this Agreement and entitles Artist to full compensation. Artist shall not be compelled, coerced or forced to render performance in part or whole in any such instance.

3. Billing & Publicity

(a) Artist shall be referred to as DEE DEE BRIDGEWATER on all manner and forms of advertising in connection with the Engagement, including but not limited to tickets, paid newspaper advertising, publicity releases, programs, fliers, posters, signs, billboards and marquees. No other name or names shall be billed or used in connection with the Engagement without the express written consent of Producer. Promoters agrees that all photographs, biographical information and other material furnished by Producer to Promoter shall be and remain the property of Producer, and

shall be used only in connection with the promotion and publicity of the Engagement. Promoter agrees not to use any photographs or biographical information of Artist in connection with the Performance other than those furnished by Producer without the prior written approval of Producer. Promoter agrees not to use Artist’s name and/or likeness, directly or indirectly, to sponsor, endorse, promote, or otherwise suggest an affiliation with any commercial product, company or service, including but not limited to newspapers, magazines, and radio or television stations. Promoter agrees that there shall be no sign, banner, or notice of any kind on or near the stage at any time during the Performance advertising any commercial product, company or service, without prior written consent of Producer.

(b) Artist shall not be scheduled or committed to appear at any press conference, interview or signing without the express written consent of Producer and subject to Addendum F attached herewith.

(c) All press materials, requests, interviews, etc., must be direct to Artist’s publicist. No materials from other sources will be accepted.

4. Creative Control

Artist shall have sole control over all creative elements of the Performance including but not limited to material selection, lighting and sound direction.

5. Unauthorized Recording Prohibited

(a) PROMOTER MAY NOT RECORD, FILM, TAPE, VIDEOTAPE, BROADCAST, OR OTHERWISE CAPTURE, OR TRANSMIT WITHOUT CAPTURING, ANY PORTION OF THE PERFORMANCE OR ANY REHEARSAL THEREFORE WITHOUT THE EXPRESS PRIOR WRITTEN AGREEMENT OF PRODUCER. NO ARCHIVAL RECORDINGS ARE PERMITTED.

(b) In the event that Promoter and/or Promoter’s agents, servants, employees or contractors reproduce or cause to be reproduced the Performance in the form of film, audio tapes, video tapes, compact discs, or any other means of audio or visual reproduction, without prior written permission of Producer:

(i) Upon demand by Producer, Promoter shall deliver all such reproductions (together with any and all masters, negatives, and other means of reproduction thereof) to Producer at Promoter’s sole cost and expense; and

(ii) Promoter shall pay to Producer a sum equal to five times the total compensation provided for in the Agreement as liquidated damages, without prejudice to any other legal or equitable remedies that Producer may have.

(c) Promoter shall take all practicable measures to prevent unauthorized capture or transmission of the Performance by others. Such measures shall include, at a minimum:

(i) Posting signage prohibiting recording devices in the venue; and

(ii) Instructing security and ticket personnel to prohibit ticket-holders from entering the venue carrying audio or audiovisual recording devices.

6. Producer’s Right to Record

Producer shall have the right to record, film, videotape or otherwise capture all or part of the Performance or rehearsal therefore without charge to Producer. Promoter shall allow Producer such access to the facilities in which the Performance occurs as is reasonably necessary for Producer’s practical exercise of this right, without charge to Producer. Promoter shall have no right, title or interest in or to any recording, film, tape or other embodiment of the Performance or rehearsal therefore created pursuant to the terms of this paragraph 6.

7. Stage, Sound, and Lighting

Promoter agrees to supply, at Promoter’s sole cost and expense, the stage setup, sound equipment and personnel, lighting equipment and personnel, and other technical equipment and personnel set forth in Appendix A hereto, in the manner described in Appendix A hereto, which Appendix is hereby incorporated by this reference as though set forth in full herein.

8. Transportation and Lodging

Promoter agrees to supply, at Promoter’s sole cost and expense, the transportation, lodging, and other goods and/or services set forth in Appendix B hereto, in the manner described in Appendix B hereto, which Appendix is hereby incorporated by this reference as though set forth in full herein.

9. Meals and Catering

Promoter agrees to supply, at Promoter’s sole cost and expense, meals and catering, and other goods and/or services set forth in Appendix C hereto, in the manner described in Appendix C hereto, which Appendix is hereby incorporated by this reference as though set forth in full herein.

10. Tickets

(a) Promoter shall distribute no more than ten (10) complimentary tickets to the Performance, including tickets allocated for newspaper, radio and television personnel. Promoter shall supply Producer with an exact list of all complimentary tickets distributed by Promoter.

(b) Promoter shall provide Producer with twelve (12) complimentary tickets for each Performance. Any tickets not used by Producer will be returned to Promoter on the day of the Performance.

(c) Scaling

Promoter warrants that the tickets for each Performance will be scaled at the following prices:

Under no circumstances are any tickets to be discounted in any manner whatsoever without the prior express written consent of Producer.

(e) Reserved Seat Basis

Unless otherwise agreed to in writing by Producer, the place of performance will be scaled on a reserved-seat basis, with all tickets numbered by seat according to the house-seating chart.

11. Participation in Gross Receipts

With respect to any Performance where the Producer is being paid based on a percentage of gross receipts, the following shall apply:

(a) Promoter shall furnish Producer not later than fourteen (14) days before the scheduled performance date a ticket manifest and plot plan setting forth the number of tickets to be sold in each price category. All tickets shall be sold

accordingly with the prices as stated. Additionally, a printer's manifest of the house (a sworn and notarized-signed statement from the printer of tickets listing amount(s) of tickets printed at each price) shall be furnished to Producer’s representative the night of the performance.

(b) Promoter shall be obligated to pay Producer Producer’s percentage compensation for every seat sold and/or occupied within the place of performance.

(c) All percentage payments provided for herein shall be paid to the Producer or its representatives by intermission, and shall be accompanied by a signed written statement from the Promoter. Promoter's failure to furnish such statement on time shall excuse Producer from further performance hereunder, but any such nonperformance shall not relieve Promoter of any of Promoter's obligations hereunder.

(d) Producer must approve in writing any changes in the prices for tickets stated above or the scaling of the place of performance and any and all discounting of tickets.

(e) Promoter further agrees to have on hand at the place of performance the night of the show, for counting and verification by a representative of Producer, all ticket stubs and unsold tickets. Producer shall be compensated for the difference between the total of such stubs and unsold tickets and the number of tickets manifested. If the Promoter shall violate any of the preceding provisions of this paragraph, it shall be deemed that Promoter has sold a ticket for each seat in the place of performance (and permitted standing room) at the highest ticket price for which the place of performance is scaled.

(f) Promoter further agrees to give Producer’s representative the right to enter the box office at any time (during the performance and after the performance) and to examine and make extracts from the box office records of Promoter relating to the gross receipts of the Engagement. A written box office statement, certified and signed by the Promoter, will be furnished Producer prior to intermission.

(g) Promoter may not sell tickets to a performance herein as part of a series of other concerts without the prior written consent of Producer. All tickets printed under the manifest shall be of the one stub, one price variety.

Examples of tickets prohibited under this agreement are:

• One price for students and a different price for general admission on the same ticket.• One price for tickets bought in advance and a different price for tickets bought at the gate on the same ticket.

(h) All tickets shall be printed by a bonded ticket house or, if the performance is at a college or university, the official printing department of the college or university.

(i) All ticket stubs and unsold tickets shall be retained by Promoter for a period of ninety (90) days from the date of the Performance, during which time a representative of the Producer shall have the right to count and examine them, and the further right to inspect the books and records of the Promoter with respect to the percentage compensation.

(j) Promoter is liable for any and all counterfeit tickets and under no circumstances will Producer assume a loss as the result of any such tickets.

12. Merchandising

Producer shall have the sole and exclusive right, but not the obligation, to sell merchandise in connection with and at the Performance, including but not limited to recordings, posters, T-shirts and other articles of apparel, souvenirs and programs, and Promoter shall have no interest in or claim to any receipts from such sales. Promoter agrees that no other product, recording or other item identified with Artist shall be sold or distributed by Promoter or its agents, employees, servants, or assigns without the prior written agreement of Producer.

Should Producer opt to have merchandise sold, Producer will provide Promoter with 25% of the net proceeds from sales. Promoter agrees to provide seller or space within Gift Shop or other Retail location within venue. All recorded materials provided by Producer must be SoundScan reported (form provided by Producer). If an outside Retailer is brought in by Promoter to handle retail, all sales must be SoundScan reported by Retailer.

13. Security

(a) Promoter agrees to provide security personnel before, during, and after the Performance adequate to insure the safety of Artist, Artist’s crew, and their property and equipment. Promoter agrees to provide adequate secured parking at all times for vehicles provided by Promoter to Producer and/or for vehicles belonging to Artist or Artist’s crew. Promoter assumes responsibility for any loss or injury resulting from lack of adequate protection backstage, on-stage and in the auditorium and/or on the property of the venue, as well as for loss of vehicles and/or their contents resulting from lack of adequate security for vehicles for which secured parking is provided. Promoter’s obligations under this paragraph 12 shall commence upon pick-up of Artist, equipment and crew at the airport and shall continue until Artist, equipment and personnel are returned to the airport.

(b) Promoter shall provide backstage passes to Producer. Producer agrees that backstage passes will be issued only to persons specifically designated by Producer.

(c) Promoter shall post signage indicating that all cellular phones and pagers must be switched off during the Performance.

14. Representations & Warranties of Promoter.

(a) Promoter represents and warrants that it is, or that before and on the Performance date will be, the owner, operator, representative or lessee (in its own name) of the place specified in the Agreement for the Performance to take place. No provision in any lease that is inconsistent with any provision of the Agreement or this Rider shall be binding on Artist or Producer unless Producer consents in writing thereto.

(b) The person executing the Agreement and this Rider represents and warrants that s/he has the right and authority to enter into and fully perform this Agreement and is of legal age to enter into a binding contract.

(c) Promoter represents and warrants that the execution and performance of this Agreement will not constitute a breach

of any agreement or restriction, if any, to which the Promoter is a party or by which it may be bound.

(d) Promoter assumes all responsibility for obtaining all necessary permits, licenses and authorizations from any and all governmental agencies, bureaus, and departments, and Promoter warrants and represents that all such permits, licenses and authorizations will be obtained for the Performance. Promoter’s failure to obtain any necessary permit, license or authorization for the Performance is a material breach of this Agreement.

15. Default By Promoter

If Promoter fails or refuses to perform any of its obligations contained in the Agreement and this Rider, including but not limited to failing or refusing to proceed with the presentation of the engagement which is the subject of the Agreement or to make any payments as and when required hereunder; or if the Performance is rendered impossible or unfeasible by any cause within Promoter's control or which Promoter, by the exercise of reasonable diligence, could have avoided, then:

(a) Producer may, in its sole discretion, terminate the Agreement without liability of any kind to Promoter;

(b) Artist shall have no obligation to perform;

(c) Producer shall retain all amounts paid to it by Promoter;

(d) Promoter shall remain liable to Producer for any additional compensation provided for in the Agreement; and

(e) Any relief or remedy provided for herein shall be without prejudice to any other or additional remedy that may be available to Producer or Artist at law or in equity.

16. Anticipatory Breach

If, on or before the date of any scheduled performance hereunder the financial standing or credit of Promoter has been called into question, Promoter shall provide upon demand satisfactory documentation of its ability to perform its obligations under this Agreement. If Promoter is unable to provide such documentation, Producer shall have the right to demand payment

forthwith of the total compensation specified in the Agreement (excluding percentages). If Promoter fails or refuses to make such payment forthwith, Producer shall then have the right to terminate the Agreement without liability of any kind to Promoter, and shall retain any moneys theretofore paid to it by Promoter.

17. Cancellation

If Artist is engaged to provide services in connection with a motion picture, television program, stage play, Nevada or Atlantic City or foreign concert tour, and if Artist, in Artist's sole discretion, determines that appearing at the Performance would conflict with such other engagement, or with any rehearsal or preparation therefore, Producer may cancel this Agreement by giving Promoter written notice mailed to the address set forth in the Agreement at least sixty (60) days prior to the Performance.

18. Force Majeure

In the event of any illness, accident or medical emergency affecting Artist or Artist’s immediate family member(s) (i.e.: mother, father, spouse or significant other, sibling or children), or if the Performance is rendered impossible, unfeasible, or dangerous to Artist’s physical safety as a result of any accident, fire, riot, war, civil disturbance, strike, lockout or other labor dispute, act of God, order, rule, regulation or act of any governmental authority, absence of power or other essential services, failure of technical facilities, failure or delay of transportation facilities, the parties hereto specifically agree that: (a) Producer’s obligation to provide Artist’s services shall be waived as to the Performance affected; and (b) neither party hereto shall have any claim for loss or damages against the other party hereto with respect to the Performance affected.

Provided that Artist is ready, willing and able to perform pursuant to the terms of the Agreement, the fact that inclement weather may render the Performance impossible or unfeasible does not abrogate or diminish Promoter’s obligation to pay the compensation specified in the Agreement unless the Agreement expressly reserves to Promoter the right to cancel without obligation in the event of inclement weather and the specific paragraph containing such reservation of right is initialed by Producer.

19. Indemnification

Promoter shall indemnify and hold Producer and Artist harmless from and against any and all liability, claims, demands, costs, expenses, loss and damage (including reasonable attorneys' fees) arising out of any breach of this Agreement by Promoter or in connection with any bodily injury, death, or loss of or damage to property which occurs in connection with any performance rendered or to be rendered by Artist during this engagement (unless same is caused by the tortious conduct of Artist or any employee or agent of Artist). Promoter represents and warrants that it presently carries, and will have in force at the time of the Engagement and for a reasonable time thereafter, (i) a policy of insurance against property damage in an amount not less than the full replacement cost of the venue in which the performance occurs, and (ii) a Commercial General Liability policy of insurance protecting Promoter against claims for bodily injury, personal injury or personal advertising injury, and property damage based upon, involving, or arising out of Promoter’s ownership, use, occupancy, or maintenance of the venue in which the Performance occurs and all areas appurtenant thereto. Such insurance shall be on an occurrence basis providing single limit coverage in an amount not less than $2,000,000.00 per occurrence. Promoter shall provide Producer with appropriate certificates of insurance evidencing such coverage not less than thirty, and not more than sixty, days before the scheduled date of the Performance. The parties agree that Promoter’s inability to provide adequate evidence of insurance coverage in the amounts set forth herein is a material breach of Promoter’s obligations under this Agreement.

20. Governing Law

This Agreement shall be construed in accordance with the laws of the State of California as applicable to agreements entered into and fully performed therein.

21. Arbitration

Any dispute or controversy arising out of or in connection with this Agreement shall be submitted to and determined by arbitration before a single arbitrator in the State of California under the rules then obtaining of the American Arbitration Association and judgment on any award rendered may be entered in any court having jurisdiction thereof.

22. Entire Agreement

This Agreement, including this Rider and its appendices, constitutes the entire understanding and agreement of the parties relative to the subject matter hereof, and any and all prior agreements, understandings, or representations are hereby terminated and cancelled in their entirety and are of no force or effect.

23. Captions

The captions appearing at the commencement of the paragraphs hereof are descriptive only and for convenience in reference. Should there by any conflict between any such caption and the paragraph at the head of which it appears, the paragraphs and not such caption shall control and govern in the construction of the document.

24. Construction

For all purposes of interpretation or construction of this Agreement, the singular shall include the plural and the plural shall include the singular, and the masculine gender shall include the feminine and/or neuter and vice versa.

25. Waiver

The waiver by any party to this Agreement of a breach of any provision of this Agreement shall not be deemed a continuing waiver or a waiver of any subsequent breach, whether of the same or another provision of this Agreement by such party, unless the same shall be made in writing.

26. Severability

If any one or more of the provisions of this Agreement shall for any reason be held invalid or unenforceable in whole or in part, such invalidity or unenforceability shall not affect any other provision hereof, and this Agreement shall be construed as if such invalid or unenforceable provision was not contained herein, or was contained herein only to the extent the same was enforceable.

27. Confidentiality

All terms of this Agreement are deemed confidential and shall not be divulged, described, duplicated, disseminated or shared in part or in whole by any means or methods, including, but not limited to press releases, interviews, social media, online outlets, etc. Release of any such information shall constitute a material breach of confidentiality and shall entitle Artist to any and all legal and financial remedies.

The Confidentiality provisions of this agreement shall survive the termination of this Agreement. Promoter and all of its agents, designees, employees, sub-contractors, independent contractors and unpaid employees or interns are bound to hold all terms of this Agreement in confidence in perpetuity.

No alterations, amendments or exclusions to any part of the technical rider are permitted without the express prior written consent of Artist’s Sound Engineer and require counter-signature for approval.

Promptly following execution of the Agreement, Promoter shall provide Artist's office or agent, via fax or U.S. mail, the following information:

1. Name and phone number of the technical manager of the venue;2. Precise address of and a map to the venue; and3. Stage-plot and complete description of the venue.

Promoter agrees to provide, at Promoter’s sole cost and expense, as follows and according to the following specifications:

1. Promoter’s Technical Crew

During settings and Performance:

• Technical manager of the venue;• Electric engineer in charge of the venue powering;• A lighting designer;• Two (2) follow spot operators;• A monitoring sound engineer.

2. Stage

Minimum size is 26' wide x 23' depth. The stage floor must be covered with a dark dance carpet. Black background. The stage must have a small black round table with a nice drinking glass (no plastic), a carafe, and a small digital clock. Two large fans should be available if needed. The stage should be set up with Nine (9) small dark towels and twelve (12) one-liter bottles of Evian mineral water.

3. Private Dressing Rooms

From 2:00 p.m. on, Promoter will provide four (4) separate clean and comfortable dressing rooms, heated or cooled to about 68° F (20° C), which lock by key (keys will be given to the road manager upon arrival), as follows:• For Ms. Bridgewater, the “Star” dressing room with a toilet and shower, furnished with a sofa and armchairs, a makeup/

dressing table with a large lighted mirror (with an electrical outlet) for applying makeup, hangers, wastebasket, large towels and soap, a full-length mirror, and a working telephone (for local or calling card calls). Please have an electric boiling pot organic Ginger or Organic Ginger-Lemon tea, organic fresh unsliced lemons with knife, tea cups, drinking glasses and a water carafe / pitcher (No Plastic Please). One (1) bottle of top-quality Cabernet Sauvignon. One (1) bottle of Veuve Cliquot Champagne with two (2) glass champagne flutes. Four (4) local micro-brews on ice with drinking glasses.

• For the crew, one dressing room near the stage, furnished with armchairs, a table, a mirror, large towels and soap.• Meals must be available in the dressing room as provided in Appendix C.

4. Light System

Promoter will provide a complete light system with minimum of 36 PC spotlights and a 1200-watt HMI follow-spot. Soft colors such as blue, pink and amber will be used (no green). Lighting will be mainly made with showers and backlight with minimum front light. You can focus on musician positions according to the stage plot. Promoter will also provide a Clear Comm. System between front desk-monitors, follow spot and light board. Setting up and operating must be done by Promoter’s technical team under the supervision of Artist and her crew.

5. Sound System.

This system will have to be ready, all lines checked, under the responsibility of a technical supervisor. He will especially have to take care of the PA disposition, the electrical powering quality, and the location of the mixing board. Please provide a comfortable seat next to the sound engineer reserved for the management or guest.

The mixing board cannot be under a balcony. It must be in the center and in the middle of the audience. The stage board must be on even side. We will not accept any other disposition! DDB Production, Inc.’s representative shall have the sole and absolute authority in mixing and controlling all sound equipment with which Dee Dee Bridgewater performs. If a part of mixing desk must be used for other artists, it must be done with our agreement. Some channels cannot be moved.

IMPORTANT: Please do NOT set any equipment (sound or instruments) on stage if it's needed to move on stage to focus lights!

Ms. Bridgewater travels with a management, musicians and technical crew as outlined in attached preceding Rider. Number of Airfares, Rooms and Meals must be completed prior to signature. Promoter agrees to provide, at Promoter’s sole cost and expense, for Ms. Bridgewater and her crew as follows:

1. TRANSPORTATION (a) Airfares:

(i) Itineraries:Producer and/or Producer’s management must approve air travel schedules and carriers.

(ii) International Travel:Overweight: Overweight or cartage charges for instruments and/or equipment are to be reimbursed to Producer by Promoter.

(iii) Visas: Work and/or travel visas necessary for band or crew must be coordinated through Producer’s management office. All visa processing fees to be covered by Promoter. Visas must be arranged so that a third party representative may present and collect passports as band members may be unable to submit themselves.

(b) Ground TransportationGround transportation includes, but is not limited to the transport of all band and crew to and from airport(s), train station(s), venue(s), Promoter arranged publicity and engagement related events. In locations with restrictive airline access, Promoter agrees to extend ground transportation to the nearest major airport or point of access. (i) A comfortable, air-conditioned 2013 Mercedes Benz S-Class, 2013 Lincoln Town Car, or equivalent, with

driver, readily available on short notice at Ms. Bridgewater’s request from the time of her arrival at the airport until her departure; and

(ii) A comfortable air-conditioned, 15-person passenger van, with driver for musicians and crew.Note: Van must be reconfigurable to allow back seats to be removed if necessary to create sufficient space for luggage, a double bass and backline. Van should be readily available on short notice at Ms. Bridgewater’s crew’s request from the time of the first crew’s arrival at the airport until the last crew’s departure.

2. LODGING (a) Five-star hotel convenient to the venue, with free WIFI, two (2) restaurants on the premises, 24 hour room service

and telephone service, mini bars in all rooms, and WINDOWS THAT OPEN, full suite(s) on club/executive level and single rooms for night(s). All hotel accommodations are subject to Producer’s approval. Note: All rooms must have Western-style KING beds.

• In instances when travel to performance location requires travel on the day prior to the event, Promoter agrees to cover hotel room and tax charges for those travelers. If travelers must stay on beyond performance date, Promoter agrees to arrange for the extension of any negotiated rates to Producer so that Producer may arrange room and tax payments accordingly.

(b) Breakfast for persons at the hotel each morning.

Note:Ms. Bridgewater frequently travels with a small, certified service dog.

Documentation provided in Service Animal Addendum.Vaccinations are kept current and may not be reflected in the enclosed documentation.

3. Dinner: For Persons (Or $50.00 USD per person buy-out = Total $ USD CASH ONLY)• Consisting of salad, entrée (protein), vegetable, starch (rice, potato or pasta), bread, beverages and a dessert. • NO SHELLFISH.• One (1) white-meat meal (fish or chicken)• One (1) vegetarian meal.• Three (3) bottles of quality Cabernet Sauvignon• One (1) ½ gallon 2% Milk

4. One-Hour Before Performance:• Twelve (12) 1-liter bottles of Evian water.• Twelve (12) local micro brews (stouts and ales preferred) on ice.• Three (3) bottles quality Cabernet Sauvignon.• One (1) small glass carafe with glass (no plastic). Artist maintains the sole and exclusive rights over all aforementioned materials, name and likeness in all formats and mediums.

1. MATERIALS:(a) All publicity, marketing and promotional materials must be solely obtained from Artist’s publicist. This includes, but is

not limited to biographies, discographies, photos, posters, EPKs, video and audio footage. Artist’s publicist maintains the most updated materials and information.

(b) Any materials obtained from any unapproved source shall not be permissible for use in any manner. (c) All posters, programs, marketing and promotional materials (printed and online) must be submitted to Artist’s publicist for

prior written approval. Artist’s publicist will not unreasonably withhold approval. (d) All printed and online materials must cite photographer credit in all instances.

2. REQUESTS:(a) All publicity requests including, but not limited to phone interviews, in-person interviews, television or radio appearances

must be submitted to Artist’s publicist with no less than seventy-two (72) hours advance notice. (b) All requests must be submitted in writing via email and no later than seven (7) days prior to appearance. (c) In instances when Artist is touring, all requests are subject to Artist’s schedule.

3. INTERNET, SOCIAL NETWORKS, ONLINE:(a) Artist reserves all rights to any online streaming video and/or audio content. (b) No still photos, footage or audio may be posted to any website without Artist’s publicist’s prior written consent.

4. PHOTOGRAPHY:(a) Any press photography to occur during Artist’s performance require prior consent from Artist’s publicist or Management

and may only occur during the first two (2) songs of the performance. (b) No flash photography permitted.(c) No video recording permitted.

Purpose of FormA person who is required to file an information return with the IRS must obtain your correct taxpayer identification number (TIN) to report, for example, income paid to you, real estate transactions, mortgage interest you paid, acquisition or abandonment of secured property, cancellation of debt, or contributions you made to an IRA.

Use Form W-9 only if you are a U.S. person (including a resident alien), to provide your correct TIN to the person requesting it (the requester) and, when applicable, to:

1. Certify that the TIN you are giving is correct (or you are waiting for a number to be issued),

2. Certify that you are not subject to backup withholding, or3. Claim exemption from backup withholding if you are a U.S. exempt

Ms. Bridgewater commonly travels with a Certified Service Animal (small dog - Maltese).

The US A.D.A. provides legal protections and provisions for such animals and their owners. The nature of the service animal’s role is CONFIDENTIAL and may not be disclosed to any parties under any circumstances.

No establishments may prohibit the dog’s presence or limit its access to areas the Artist accesses. Additionally, no establishments may charge any additional fees, restrict access or cause undue hardship or discrimination due to the Service Animal’s presence. This includes, but is not limited to hotels, airports, theaters, ground transportation, etc.

A copy of this Appendix and the attached US Department of Justice Commonly Asked Questions may be provided to any establishment’s unfamiliar with the ADA regulations. Copies of Physicians Letters shall remain CONFIDENTIAL. Any service providers, vendors or establishments unaware of the provisions afforded Certified Service Animals should be encouraged to familiarize themselves with the ADA guidelines related to this issue.

In instances of international travel, the service animal guidelines prevail in accordance with that nation’s regulations. Ms. Bridgewater’s dog has all necessary US and International travel documentation and vaccinations. Copies will be provided upon written request.

COMMONLY ASKED QUESTIONS ABOUT SERVICE ANIMALS IN PLACES OF BUSINESS

Q: What are the laws that apply to my business?A: Under the Americans with Disabilities Act (ADA), privately owned businesses that serve the public, such as restaurants, hotels, retail stores, taxicabs, theaters, concert halls, and sports facilities, are prohibited from discriminating against individuals with disabilities. The ADA requires these businesses to allow people with disabilities to bring their service animals onto business premises in whatever areas customers are generally allowed.

Q: What must I do when an individual with a service animal comes to my business?A: The service animal must be permitted to accompany the individual with a disability to all areas of the facility where customers are normally allowed to go. An individual with a service animal may not be segregated from other customers.

Q: I have always had a clearly posted "no pets" policy at my establishment. Do I still have to allow service animals in?A: Yes. A service animal is not a pet. The ADA requires you to modify your "no pets" policy to allow the use of a service animal by a person with a disability. This does not mean you must abandon your "no pets" policy altogether but simply that you must make an exception to your general rule for service animals.

Q: My county health department has told me that only a guide dog has to be admitted. If I follow those regulations, am I violating the ADA?A: Yes, if you refuse to admit any other type of service animal on the basis of local health department regulations or other state or local laws. The ADA provides greater protection for individuals with disabilities and so it takes priority over the local or state laws or regulations.

Q: Can I charge a maintenance or cleaning fee for customers who bring service animals into my business?A: No. Neither a deposit nor a surcharge may be imposed on an individual with a disability as a condition to allowing a service animal to accompany the individual with a disability, even if deposits are routinely required for pets. However, a public accommodation may charge its customers with disabilities if a service animal causes damage so long as it is the regular practice of the entity to charge non-disabled customers for the same types of damages. For example, a hotel can charge a guest with a disability for the cost of repairing or cleaning furniture damaged by a service animal if it is the hotel's policy to charge when non-disabled guests cause such damage.

Q: Can I exclude an animal that doesn't really seem dangerous but is disruptive to my business?A: There may be a few circumstances when a public accommodation is not required to accommodate a service animal--that is, when doing so would result in a fundamental alteration to the nature of the business. Generally, this is not likely to occur in restaurants, hotels, retail stores, theaters, concert halls, and sports facilities. But when it does, for example, when a dog barks during a movie, the animal can be excluded.

Right Breed for You?Even though the Maltese is a very small dog, they tend to be brave and without fear. They areone of the most gentle mannered of all little dogs, but are also full of energy and veryplayful, making them great family dogs. Maltese should be brushed daily, and groomed oftento prevent mats from forming in their long, white coat.

black rims enhancing the gentle yet alert expression. The muzzle is of medium length, fineand tapered but not snipy.The nose is black. The teeth meet in an even, edge-to-edge bite, orin a scissors bite.

NeckSufficient length of neck is desirable as promoting a high carriage of the head.

BodyCompact, the height from the withers to the ground equaling the length from the withers tothe root of the tail. Shoulder blades are sloping, the elbows well knit and held close to thebody. The back is level in topline, the ribs well sprung. The chest is fairly deep, the loins taut,strong, and just slightly tucked up underneath.

TailA long-haired plume carried gracefully over the back, its tip lying to the side over the quarter.

Legs and FeetLegs are fine-boned and nicely feathered. Forelegs are straight, their pastern joints well knitand devoid of appreciable bend. Hind legs are strong and moderately angulated at stifles andhocks. The feet are small and round, with toe pads black. Scraggly hairs on the feet may betrimmed to give a neater appearance.

Coat and ColorThe coat is single, that is, without undercoat. It hangs long, flat, and silky over the sides ofthe body almost, if not quite, to the ground. The long head-hair may be tied up in a topknotor it may be left hanging. Any suggestion of kinkiness, curliness, or woolly texture isobjectionable. Color, pure white. Light tan or lemon on the ears is permissible, but notdesirable.

SizeWeight under 7 pounds, with from 4 to 6 pounds preferred. Overall quality is to be favoredover size.

GaitThe Maltese moves with a jaunty, smooth, flowing gait. Viewed from the side, he gives animpression of rapid movement, size considered. In the stride, the forelegs reach straight andfree from the shoulders, with elbows close. Hind legs to move in a straight line. Cowhocks orany suggestion of hind leg toeing in or out are faults.

TemperamentFor all his diminutive size, the Maltese seems to be without fear. His trust and affectionateresponsiveness are very appealing. He is among the gentlest mannered of all little dogs, yethe is lively and playful as well as vigorous.
