BCBS continues to provide the most affordable, comprehensive coverage and network of providers to our associates in all locations.

We are offering the same two PPO options, a base and a buy up.

Employees desiring a higher level of coverage with lower out-of-pocket exposure can buy up to the BlueOptions plan. If you would like to change plans, you need to do so now.

BlueCross provides international medical coverage through BlueCard Worldwide

Includes office visits, hospitalization, emergency care, etc.

Although you can search for in-network providers online at www.bcbs.com under “Locate health care providers outside the US”, they recommend that you coordinate any medical treatment you need by calling their Worldwide Service Center at 1-800-810-BLUE (2583) or collect at 1-804-673-1177, particularly for inpatient and emergency care.

You will likely be responsible for your deductible, copays and coinsurance up front and then can file the claim for reimbursement if the provider does not file directly with BlueCross.

Travel assistance is also provided by MetLife for 24/7 coverage for travel, insurance, medical and financial assistance when you are 100 miles away from home.

Dependent Coverage for Young Adults:Employers must provide dependent coverage to young adults until age 26. This includes married dependents but not grandchildren.

No Cost Sharing on Preventive Services:Requires health plans to provide a minimum of coverage without cost sharing for preventative services, immunizations recommended by the CDC, and preventive screenings for infants, children, adolescents and women. Preventive services covered are those classified into either the A or B category of services, but will be dependent upon age and type of condition.

Lifetime Limits:No lifetime dollar limits may be placed on coverage

Emergency Services do not require prior authorization out-of-network and must be paid at the in-network coinsurance level. Balance billing out-of-network still applies.

Over-the-counter Drugs Exclusion:

Over-the-counter drugs not prescribed by a doctor are excluded from being reimbursed through a Flexible Spending Account (FSA) after 12/31/10.

Plan Rescission:

Plans cannot rescind coverage when an individual files a claim unless there is fraud or misrepresentation of material facts.

HSA Tax Increase:

Increases the tax on non-qualified distributions from an HSA from 10% to 20%.

Set aside on a pre-tax basis up to $5000 in anticipated and qualified health expenses and $5000 in dependent care costs through your flex plan on an annual basis.

This is changing to $2500 in 2013, so if you have something expensive you would like to have done, such as LASIK, you may want to plan that for 2012.

As a reminder, effective 1/1/11, over-the counter drugs were no longer reimbursable under your flex without a prescription. So do not expect to use those this year for unused money in your flex.

You and your dependents do not need to be enrolled in Summit’s benefits to be reimbursed for qualified expenses under our FSA. However, you cannot claim the same expenses that are reimbursed through another plan, such as a spouse’s flex or HSA account.

Dependent care expenses will be reimbursed once you have accumulated the money in your account to cover that expense/ claim for reimbursement.

You have until March 31st of the following calendar year to turn in your receipts for qualified expenses incurred between January 1st through December 31st. Money accumulated in your account but not reimbursed in time will be forfeited.

Make sure you incur your expenses for 2011 by 12/31/11!

Advantages:

The “Unreimbursed Medical” acts like an interest free loan. You can incur and be reimbursed for the expenses in January, for example, but only make small payments throughout the year through payroll deductions to cover that expense.

Your payroll deducted flex elections and insurance premiums are deducted pre-tax to save you money. Be advised that you cannot make plan changes throughout the year unless you have a qualifying event. If you would prefer to not have your deductions taken out pre-tax please let me know.

Expenses you pay out in prescriptions, copays, deductibles, coinsurance etc. on your medical, dental, and vision plans are qualified expenses to be reimbursed through your flex.

Pre-x:Your medical plans have a pre-existing condition clause in which they will not cover pre-existing conditions for 12 months. However, this can be eliminated or reduced if you can provide a Certificate of Coverage showing that you have had 12 months of continuous coverage prior to becoming eligible under our plan.

Enrollment Changes/ Pre-x:All enrollment changes must be made during the month of December. Otherwise you can only make a change during the year if you have a qualifying event.
