Understanding Your Vision Care Needs...is the first step in identifying how a vision plan will best fit into a healthcare package for you and your family. With more peopleneeding some sort of vision correction growing each year, having the insurance of a quality vision benefit can ensure that youroutinely check for any changes in your eyesight and potentially save you hundreds of dollars in unexpected expenses.

How the Vision Plan Works

The Avesis plans provide covered members with a completevision examination, a pair of spectacle lenses, and a frame.If the member elects to receive contact lenses in place ofspectacle lenses and frames, Avesis will provide a generousallowance of $110 or $130 depending on the plan selected.

With a quality Vision Plan from Avesis, you have theconfidence of knowing that you and your family member’svision care needs are covered. Plans are flexible to givemembers choices; affordable, giving members one of thebest values in healthcare.

Spectacle Lenses include: (Standard single vision, bifocal, trifocal orlenticular lenses). Lens options are discounted when using aparticipating provider by as much as 20% off of retail pricing.Participating providers will carry an assortment of frames withinthe allowance for each Avesis plan. Contact Lenses are in lieu offrames and spectacle lenses and are not subject to a co-pay. Co-paysmay apply for vision exams and optical materials.

Giving members more choices is a continuing goal forAvesis. In addition to our in-network coverage listed above,Avesis provides reimbursement coverage for members whoelect to go to non-participating providers.

The popularity of laser technology to correct certain visionconditions continues to grow. Avesis provides its memberswith a LASIK allowance of $100 or $150 depending onwhich program is selected (Enhanced or Plus). In addition,Avesis’ national network of providers will discount theirservice up to 25% prior to applying the LASIK allowance.

Vision care benefits are typically available to all activeemployees as defined by your group. Benefits are extended toinclude spouses and dependents (see your Benefits Managerfor group dependent eligibility rules).

The Avesis plans are designed to give each member acomplete solution for vision correction. If the memberchooses, add-ons can be purchased at a substantial savingsof up to 20% off of retail pricing. Even after membershave exhausted their covered benefits for the plan period,they will still be able to take advantage of deep discountson additional eyewear from participating providers.
