I, ________________________________, agree to all the following terms and conditions set forth by Jeremy Marcott as the
musical entertainment (DJ) for the date of __________________________________ initially between the hours
of ____________ and ____________, for the initial price of $400.00.
• A deposit of $200 is required to book the performance date. This payment can be made by cash, card, check or
money order. A receipt will be provided, and we accept Visa, MasterCard, or Discover.
• There is no restriction set for the amount of time that the DJ will perform, but after the initial FOUR HOUR
time frame, the payment for continued entertainment will be $50 PER HOUR, charged at the quarter hour (7:15
= $25, 7:45 = $50).
• All remaining balances - including the EXTRA TIME CHARGES AND TRAVEL EXPENSES are due after the
event, immediately after the DJ tears down the equipment. This payment may be made by cash, check, debit or
credit card. Any returned checks WILL obtain a $35 returned check charge.
• Set up and tear down are NOT part of the initial FOUR HOUR performance time slot, and you are only
required to pay the DJ for the time music was actually playing.
• A PRE-SET song list will be played at the event, one approved and set by the above stated patron TWO WEEKS
prior to the event. This meeting is NON-NEGOTIABLE, and required.
• Karaoke gear will be available on the night of the event, but will be available only for a set time. The time frame
of which karaoke will be available will be agreed upon by the DJ and the above stated patron TWO WEEKS
prior to the event.
• ABSOLUTELY NO ONE will be allowed near the DJ equipment, ESPECIALLY if the DJ has temporarily left the
table. Because of the pre-set song list, requests will not be taken; therefore no one need be near the equipment.
Any damages done BY ANYONE not contingent with this statement will be the responsibility of the above
stated patron.
• Any cancellations MUST be made MORE THAN SIX WEEKS prior to the event. Upon cancellation before
the cutoff date, $125 of the original deposit will be refunded (within two weeks of the cancellation); however, a
$75 CANCELATION FEE will be kept. If there is a cancellation, AFTER the six week cutoff date, the ENTIRE
deposit is NON-REFUNDABLE.
• For any traveling, depending on distance (20 miles or more), a $25 charge for gas and one meal reimbursement
will be required for event date. AFTER 60 miles, if needed, the patron is required to provide a hotel, as well as
$50 for gas and TWO meals reimbursement. The meals WILL be for two people, the DJ and his assistant. A
receipt WILL be provided to the patron for added expenses.
I have read, understand, and agree to all the terms and conditions set for the by Jeremy Marcott as the musical entertainment
for my event on the above stated date. Upon signing, we both agree on all the information and promise to fulfill our end of the
contract.
