Hello. Good morning. How may I address you ? My name is Ho Yong Wai. I am a first year medical student at International Medical University. Today I have been asked by my consultant to ask you a few questions regarding your condition. Is that okay with you ? (May I have your permission to do ?)

Site - I see , would you please point to me where the pain is ?

Onset

Character : Can you describe the nature of the pain ?

Radiation : Does the pain radiate to anywhere else ?

Associated symptoms : Have you had any other symptoms associated with the pain ? For example , nausea , vomiting , sweating , anything like that ?

- what about difficulty in breathing or coughing ?

Time / Duration - How long did the chest pain last ?

Exacerbating / Relieving factors -

Severity - On a scale of 1 - 10 , how would you rate your pain.
