1. Is there a structured and accredited CME/CPD program dedicated to physicians in your country? If yes, who is in charge of organizing/providing accreditation for it? The National Anaesthesiology Society? The National Medical Organization/Council? Another entity? In the Czech Republic accredited CME/CPD program is legally the responsibility of Czech Medical Chamber. It is in charge of organizing it and providing accreditation. Czech Medical Chamber provides accreditation for any CME/CPD events provided by other bodies (i.e. learned Societies).

2. Is there a structured CME/CPD program dedicated to anaesthesiologists? If yes, who is in charge of organizing/accrediting it at the National Level? At the local (hospital/department) level? There is no CME/CPD structured program dedicated to anaesthesiologists at the National level. Such programs mostly run on a hospital or Departmental level. However, scientific and educational activities of the Czech Society of Anaesthesiology and Intensive Care Medicine are frequent, popular, well attended and besides CME also contain the elements of CPD.

3. Are there incentives/sanctions if a CME/CPD program is followed/not followed? Legally, every physician is bound to be engaged in CME/CPD. It is considered his/her professional duty. Incentives or sanctions do not apply.

4. How frequently is accreditation being performed: every year?; every few years? Educational activities within CME/CPD are accredited ad hoc. Credits vary according to duration, provider and scientific content.

5. Based on the attached articles, please rank the highest most frequent level of CME/CPD in anaesthesiology in your country as it is ranked at the graph below: According to the graph supplied vast majority of CME/CPD activities are levels 1-3 (Participation, Satisfaction, Learning). Level 4 (Competence) appears sporadically.

6. Given the definitions in the attached JAMA articles and in this letter, would you consider that in your country, anaesthesiologists are involved more in a CME or in a more complex CPD program? Vast majority of anaesthesiologists in the Czech Republic are involved in CME activities. More complex CPD programs/opportunities are lacking.

7. What are the two most limiting factors for implementation/adoption of CME/CPD programs in anaesthesiology in your country? Two most limiting factors are a lack of positive motivation as well as missing legislative „push“.

8. What solutions do you propose to correct these limiting factors? Any solution would have to be considered within the context of a local health care system and its manpower. In the situation, when anaesthesiologists (as well as most physician) feel seriously underpaid and when they are lacking in numbers (with ongoing considerable brain drain to more affluent neighbouring countries) applying stringent criteria would be counterproductive.

9. Please cite the 2 most relevant indicators on which you would consider that a CME/CPD program in anaesthesiology is successful The ultimate goal of our profession is safety, well-being and satisfaction of patients. Indicators should reflect this, i. e. one indicator (or a group of) should reflect safety of patients, another measure their satisfaction during the contact with our profession.

10. Do you think ESA could help improve CME/CPD programs in anaesthesiology in your country? Certainly, ESA could bring an inspiration in all aspects of CPD/CME.

11. What would be the most efficient educational tools that ESA could provide in order to improve CME/CPD programs in your country/National Anaesthesiology Society? a. Classical conferences as organized during Euroanaesthesia? All educational tools a-d are useful . It is important to present system which is educationally worthwhile, flexible and presenting widest range of activities.

12. Please provide suggestions on complimentary elements concerning CME/CPD programs in general and relevant for ESA. CME/CPD program is useful, indeed essential. It should rest on several foundations: one is a motivation of a physicians, who see their profesional growth as one of their life-long ambitions. Another one is a legislation. Another important is opportunity to join in and engage. These should be in balance.
