RISK ASSESSMENT FORM

Title of projectName and NumberAddress

Complete the following table rating each risks severity on scale from Negligible (N), Low risk (L), Moderate risk (M), Severe (S),

Very severe (V) each risks likelihood on scale from Very unlikely (VU), Unlikely (U), Possible (P), Likely (L),

Very likely (VL) Determine the risk factor from the table overleaf.

32 Other risks NCalculate the Risk Factor:

Very Unlikely Unlikely Possible Likely Very LikelyVery severe 3 4 4 5 5Severe 2 3 3 3 5Moderate 2 2 3 3 3Slight 1 2 2 3 3Negligible 1 1 2 2 3

Decide the action to take:

5 Very severe Take immediate action4 Severe High priority3 Moderate Risk Programme for action2 Low Risk Action may be required1 Negligible Probably acceptable

Then list each risk identified

Hazard no: 3 Risk Factor (1-5) 2Description AUDIENCES

Person(s) exposed (detail if cast/crew (C), outside company (O) or public (P) )

CREW, AUDIENCE

Action to take There will be an audience at the exhibition. The main risk of this is the potential of them tripping over wires around the exhibition space. The wires will be hidden as best as possible, and the audience will be encouraged not to stand in front of the projector.

To be completed at agreed later date -Exposed person(s) informed? (y/n)

Y Agreed action taken? (y/n) Y Risk removed? (y/n)

Hazard no: 4 Risk Factor (1-5) 2Description WIRES AND CABLES

Person(s) exposed (detail if cast/crew (C), outside company (O) or public (P) )

CREW, AUDIENCE

Action to take The wires leading up to the projector and laptop create a large tripping hazard for the audience in the area, so the wires must be hidden as best as possible.

Hazard no: 5 Risk Factor (1-5) 2Description CONFINED SPACES

Person(s) exposed (detail if cast/crew (C), outside company (O) or public (P) )

CAST & CREW

Action to take The exhibition space is fairly small, so the area must be made safe for walking through. The projector must also be set up so it does not connect with anybody’s eyes.

Hazard no: 19 Risk Factor (1-5) 1Description RADIATION

Person(s) exposed (detail if cast/crew (C), outside company (O) or public (P) )

CREW, AUDIENCE

Action to take The video art will be projected onto a radiator via a projector. The projector radiates a light that is potentially harmful if stared at. The radiator will most likely be turned off, but if it is turned on, this will also produce heat. The projector will face away from people to reduce risk, while the radiator presents little problem.
