    About our service:

    The Crisis and Intensive Support Team (CaIS) are a specialist multi-professional

    health team within the Doncaster Community Health Team. They support

    individuals with severe challenging behaviours, mental health needs and

    forensic backgrounds in need of intensive support or in times of crisis.

    There is a nurse on duty from 8am to 8pm, Monday to Friday.

    It is a community based service able to support people where they live, or

    working with service users in community settings alongside carers and support

    workers.

    If a period of hospital admission is required the CaIS Team support the service

    user from admission to discharge.

    The team work with service users for a focused period of time with the aim of

    preventing a hospital admission, or service users being placed in out-of-area

    placements. The recovery pathway is followed and work towards discharge

    begins from the onset of admission.

    The team also work in line with Continuous Service Improvement Portfolio

    Guidance, working closely alongside Doncaster Clinical Commissioning Group

    (DCCG) to reduce lengthy admissions to hospital by providing structured

    monitoring and personalised support.

    The team can request other professionals within the service, or from outside

    agencies, to work with individual service users to prevent hospital admission.

    • Psychiatrists who specialise in learning disabilities

    • Psychologists who specialise in learning disabilities

    • Occupational therapists who specialise in learning disabilities and sensory

    processing difficulties

    • The Health Action Team who work in partnership with primary care to

    enable clinicians to support people with learning disabilities to access

    mainstream services by making reasonable adjustments

    • Community Learning Disability Nurse Team

    • Section 117 Lead Nurse.

    Outside agencies:

    • Children services

    • Social work team

    • General practitioners

    • Various care providers and respite services (local authority, private and NHS).

    Link Nurse Role:

    The team also provide a link nurse to Doncaster based providers; this role has

    been developed to encourage communication between Doncaster providers

    and the Adult Learning Disability Service. Regular contact takes place between

    the nurses and the providers, allowing early identification of any individuals

    who may need specialist input. The team work flexibly to implement

    immediate interventions or signpost referrals to relevant professionals for

    specialist input. The aim is to prevent a crisis and support recovery.

    CaIS pathway:

    The CaIS pathway indicates different levels of interventions and reaction times

    from the duty nurse who aims to respond in a timely manner.

    RED +++ Immediate DangerSerious and immediate risk to self and others which may require assistance

    from emergency services.

    RED - High RiskTo have contact within one hour and face to face contact within one day.

    Amber - Medium RiskTo have contact within one hour and face to face contact within two days.

    Green - Low RiskTo have contact within three days and face to face contact within one week.
