100 Million Healthier Lives is an unprecedented collaboration

of change agents pursuing an unprecedented result: 100

million people living healthier lives by 2020. Together we are

fundamentally transforming the way the world thinks and acts

to improve health.

OUR THEORY OF CHANGE

We’re convinced that dramatic improvements in health and wellbeing are

possible if we can harness the power of unprecedented collaboration, adopt

a humble spirit of innovative improvement, and create catalytic system

transformation to remove barriers and accelerate progress.

OUR PRIORITIES

To determine our priorities, we sought recommendations from work groups

of cross-sector experts on what changes would be most critical to achieve

the audacious goal of 100 million people living healthier lives by 2020. Then

we combed through hundreds of member action plans to understand what

was important to them and what assets our members bring to the table. The

priorities described below emerged as the most critical for our shared learning

and action together. Topical hubs are forming to organize our work on these

priorities and individual members are encouraged to identify where they want

to focus their efforts. Priorities fall into two categories: whats and hows.

9 WHATS

1. Address equity gaps – this is the “price of admission” in 100 Million Healthier Lives.

2. Help all kids have a great start in life, with all the skills they and their families need to flourish from cradle to career.

3. Support veterans to thrive.

4. Reclaim the health, wellbeing, and dignity of indigenous communities.

5. Address the social and behavioral determinants of health across health care, the community, and social services, with a special focus on vulnerable populations.

6. Make mental health everybody’s job, across the continuum of health care, the community, public health, and social services.

7. Improve access to primary health care for all.

8. Create the best possible wellbeing in the elder years and at the end of life.

9. Engage everyone in improving their own health and in building a culture and practice of healthy living.

9 HOWS

1. Shift culture and mindset to one of partnership, co-design, and wellbeing.

2. Develop workforce strategies — engage students and youth as leaders in the transformation.

3. Integrate peer-to-peer support systems into every relevant initiative.

4. Integrate improvement and change methods at the community level.

5. Focus on the top chronic diseases in each commu-nity and core risk factors to build a continuum of health that spans the home, the community, public health, and health care.

6. Develop and adopt financing strategies that align funding at the community level.

7. Integrate data across silos (health care, community, public health, and social services).

8. Engage employers and businesses to improve workforce health and wellbeing.

9. Transform health care to be good at health and good at care.

MEMBERSHIP AND ENGAGEMENT

To join, individuals and organizations submit an

action plan committing to work toward the goal of

100 million people living healthier lives by 2020.

The action plan helps us understand their health

improvement priorities, the assets they bring, and the

people and communities they reach.

• Members: Individuals and organizations working

on the ground to improve health and who want to

be part of a community working together to create

the change that is needed.

• Partners: Individuals and organizations who are

ready and willing to play a leadership role in 100

Million Healthier Lives.

• Sponsors: Individuals and organizations who can

financially support 100 Million Healthier Lives.

Once you join 100 Million Healthier Lives, there are

lots of opportunities to engage in the work.

BECOME A CHANGE AGENT

This is a movement of leaders at every level working

together to make a difference. Opportunities to become

engaged as a change agent include:

• Working with your organization or community

to improve health, well being, and equity on

the ground.

• Connecting other change agents to the movement.

• Contributing to building our shared support system.

• Supporting transformation of policy and systems.

• Networking with other change agents on our

online learning community called HealthDoers

and by joining monthly momentum calls.

JOIN A HUB

Become involved in the work of Topical hubs, made

up of the people and organizations leading the

thinking, learning, and acting of 100 Million Healthier

Lives in specific priority areas such as Child Health,

Veterans Health, and Equity. Geographic hubs consist

of organizations with the capacity to engage local

health systems and communities and to coordinate

participation in 100 Million Healthier Lives for a

geographic area.

PARTICIPATE IN INITIATIVES RELATED TO A 100 MILLION HEALTHIER LIVES

100 Million Healthier Lives is committed to action

and demonstrating results on the ground through

large-scale, multi-partner initiatives such as these

flagship examples:

• Spreading Community Accelerators through

Learning and Evaluation (SCALE) is the first

community-based initiative of 100 Million

Healthier Lives. SCALE will help communities

develop capability to improve health and spread

effective community-driven approaches to

help build a culture of health. The two-year

SCALE initiative engages 24 communities from

across the US and is funded by the Robert Wood

Johnson Foundation (RWJF). Communities were

selected through an application process, visit ihi.

org/100mlives to view the full list of communities.

• Pathway to Pacesetter (P2P) is a multi-year program

(October 2015 through March 2017) to support

more than 100 communities in accelerating their

improvement journey. The goal of P2P is to support

local leaders at every level of a community to be

successful and multiply their effectiveness in

achieving their existing vision and goals. Please
