Create and run your own Zika KAP survey with Kobo Toolbox

Follow these simple steps


STEP 1

Visit: http://www.kobotoolbox.org/

STEP 2

Scroll down, and choose between the two access portals:


STEP 3

Click on create an account (if you don’t have one), or insert your credentials and

access Kobo (if you are already registered)


STEP 4

To create an account fill in the form and follow the instructions. You will receive a confirmation email with an

activation link. Once you have a confirmed account, login (see previous slide).


STEP 5

Click on Projects, then on “new”, and then on “upload”.


STEP 6

Select the file “11112016_Kap_Survey_full.xls” that you have downloaded.


STEP 7

Click on “Edit” in the “Form Builder” page.


STEP 8

Give a name to the form on the top (“form name”) and click on SAVE.


STEP 9

Scroll down through all the survey and delete the questions you don’t want in your KAP survey by clicking on the red

“trashcan” icon on the side of every question.

N.B. it is also possible to delete entire groups of questions by clicking on the same icon on the side of the groups


STEP 10

Once you are done editing your form click on “SAVE”, and

quit the form clicking on the x next to the save button.

N.B. Remember to save often!!


STEP 11

Once your form is ready to be used, click on “deploy” and

start your survey.


STEP 12

Once you have deployed the form, you will have to choose between different ways of collecting

data. You can also install the Kobo app for android, if you plan to use an android device for your

data collection.


STEP 13

By clicking on the “choose an option” box it is possible to decide whether the data collection should be

conducted exclusively on-line (having an internet connection), or whether it should be possible both online

and offline. Additionally it is possible to choose “view only” to test the project without submitting data, or to

copy an “Embeddable web form code” which can then be inserted in web pages and websites.


STEP 14

To collect the data, then, the participant/data collector will have to fill in the form you created.


STEP 15

Once the participant/data collector has filled in the survey, the completed form can be submitted

by clicking on “Submit”.


STEP 16

To access Kobo automatic analysis tool, click on “Data”, and then on “Reports”.


STEP 17

The Kobo automatic analysis tool will produce graphs and tables based on your data.


STEP 17

It is also possible to:

- Display the collected data in a table,

- Access the gallery of pictures taken (if a “picture” question was included in the form)

- Download in different formats (including excel) the data collected.

- Visualize the geolocation of the data collection sessions on a map (if a “GPS” was included

in the form)
