CRANIAL NERVES HOW ELICITED NORMAL RESPONSE PATIENT’S RESPONSE I. Olfactory nerve Ask client to close eyes and identify mild aromas under one nostril with the occluded nares. Repeat with one other nostril. Perception of any odor, identifies scent correctly. II. Optic nerve Ask client to read a reading material Able to read words or recognize letters III. Occulomotor nerve Ask client to follow SN’s finger from left to right. Check client’s pupil reaction to light using a penlight. Able to follow direction of finger in left and right; pupils equally round and accommodation IV. Trochlear nerve Ask client’s to follow SN’s finger in six directions of gaze Able to follow direction of fingers in up and down; pupils equally round reactive to light and accommodation. V. Trigeminal nerve Assess client’s ability to chew and clench teeth. Able to move jaw up and down with appropriate strength. Able to distinguish light touch sensation. VI. Abducens nerve Assess movement of eyeball and directions of gaze Able to move eyeballs in left and right direction. VII. Facial nerve Ask the client to smile, raise eyebrows, frown, and puff out cheeks. Assess sense of taste. Able to demonstrate varied facial expressions. Able to distinguish light touch sensation. Able to distinguish different taste. VIII. Acoustic nerve Assess client’s ability to spoken words. Able to hear whispered word at approximately 2 feet away. IX.
